A Body-Temperature-Triggered In Situ Softening Peripheral Nerve Electrode for Chronic Robust Neuromodulation.

Implantable peripheral nerve electrodes are crucial for monitoring health and alleviating symptoms of chronic diseases. Advanced compliant electrodes have been developed because of their biomechanical compatibility. However, these mechanically tissue-like electrodes suffer from unmanageable operating forces, leading to high risks of nerve injury and fragile electrode-tissue interfaces. Here, a peripheral nerve electrode is developed that simultaneously fulfills the criteria of body temperature softening and tissue-like modulus (less than 0.8 MPa at 37 °C) after implantation. The central core i

Stimulating the Cochlear Apex Without Longer Electrodes: Preliminary Results With a New Approach.

OBJECTIVE: To investigate a new surgical and signal processing technique that provides apical stimulation of the cochlea using a cochlear implant without extending the length of the electrode array. PATIENTS: Three adult patients who underwent cochlear implantation using this new technique. INTERVENTIONS: The patients received a cochlear implant. The surgery differed from the standard approach in that a ground electrode was placed in the cochlear helicotrema via an apical cochleostomy rather than in its typical location underneath the temporalis muscle. Clinical fitting was modified such tha

Spiral Ganglion Neuron Explant Culture and Electrophysiology on Multi Electrode Arrays.

Spiral ganglion neurons (SGNs) participate in the physiological process of hearing by relaying signals from sensory hair cells to the cochlear nucleus in the brain stem. Loss of hair cells is a major cause of sensory hearing loss. Prosthetic devices such as cochlear implants function by bypassing lost hair cells and directly stimulating SGNs electrically, allowing for restoration of hearing in deaf patients. The performance of these devices depends on the functionality of SGNs, the implantation procedure and on the distance between the electrodes and the auditory neurons. We hypothesized, that

Transnasal ventral brainstem stimulation reverses anesthesia-induced unconsciousness in sheep.

BACKGROUND: Brainstem reticular formation (BRF) is crucially involved in regulating arousal, making it a potential target for reversing general anesthesia. The transnasal approach provides a minimally invasive access to the ventral brainstem. However, whether this approach can effectively modulate arousal remains unknown. OBJECTIVE: To evaluate the efficacy of transnasal BRF electrical stimulation in reversing anesthesia-induced unconsciousness. METHODS: Three 12-month-old male sheep (40-50 kg) were implanted epidural/bone electrodes close to ventral brainstem via a transnasal approach. Unde

Regional cerebral blood flow and tissue oxygenation during hypocarbia in geese.

Very low arterial CO2 tension (PaCO2) experienced by birds during high-altitude flight may result in cerebral vasoconstriction and reduced cerebral O2 delivery. To examine this possibility, we measured regional cerebral blood flow (CBF) and tissue PO2 in pentobarbital-anesthetized geese (Anser domesticus). Twenty-five-micrometer Teflon-coated platinum electrodes for H2-clearance measurements of local blood flow or tissue PO2 were implanted in the cerebral cortex in 11 geese. Tissue H2 and O2 were measured by voltage clamping the electrodes at +0.30 and -0.5 V, respectively. Washout kinetics of

Cortical Brain-Computer Interface for Closed-Loop Deep Brain Stimulation.

Essential tremor is the most common neurological movement disorder. This progressive disease causes uncontrollable rhythmic motions-most often affecting the patient'sdominant upper extremity-thatoccur during volitional movement and make it difficult for the patient to perform everyday tasks. Medication may also become ineffective as the disorder progresses. For many patients, deep brain stimulation (DBS) of the thalamus is an effective means of treating this condition when medication fails. In current use, however, clinicians set the patient's stimulator to apply stimulation at all times-wheth

Nucleus 21-channel auditory brainstem implant in patients with previous tumour removal.

Three patients with neurofibromatosis type 2 who had undergone previous cochlear nerve tumour removal were implanted with the Nucleus 21-channel auditory brainstem implant (ABI). The time intervals between tumour removal and implantation were 4, 5, and 7 years, respectively. Total bilateral deafness was confirmed before implantation. One patient was also blind after acute intracranial hypertension. The translabyrinthine approach was used in all cases. The choice of side for implantation depended on pre-operative magnetic resonance imaging study, the facial nerve function, the presence of recur

Comparison of the sutureless and suture-type epicardial electrodes in pediatric cardiac pacing.

Fourteen consecutive patients with the suture-type electrode (Group 1) were compared with the previous 14 patents having the sutureless electrode (Group 2). five patients in Group 1 had had failure with the sutureless electrode, for a total of 23 patients. In Group 1, no exit block was encountered. One patient had sensing problems corrected simply by increasing the pacer rate. One patient had generator failure and another, lead fracture. In 5 patients in Group 2, exit block developed early (less than 6 months after implantation); 3 patients had the problem twice, making a total of eight early

[Electric and mechanical activities of the upper esophageal sphincter in rabbits].

Electrical and mechanical activities of the rabbit muscles in different zones of the esophageal cervical part were examined on free-moving rabbits with chronically implanted electrodes and force transducers under conditions of hunger and food intake. It is shown that the functional role of the circular muscles of the cranial end of the esophagus is determined by their participation in activity of the superior esophageal sphincter.

Targeting accuracy of robot-assisted deep brain stimulation surgery in childhood-onset dystonia: a single-center prospective cohort analysis of 45 consecutive cases.

OBJECTIVE: Deep brain stimulation (DBS) is an established treatment for pediatric dystonia. The accuracy of electrode implantation is multifactorial and remains a challenge in this age group, mainly due to smaller anatomical targets in very young patients compared to adults, and also due to anatomical abnormalities frequently associated with some etiologies of dystonia. Data on the accuracy of robot-assisted DBS surgery in children are limited. The aim of the current paper was to assess the accuracy of robot-assisted implantation of DBS leads in a series of patients with childhood-onset dyston

Towards an artificial peripheral nerve: Liquid metal-based fluidic cuff electrodes for long-term nerve stimulation and recording.

Nerve cuff electrodes have been used for decades as peripheral nerve interfacing devices in the fields of neural science, neural disease, and brain-machine interfacing. The currently-used cuff electrode is commonly based on rigid materials whose flexibility and tensile properties are far different from those of biological nervous tissue. Herein, a fluidic cuff electrode using a gallium-based liquid metal (LM) conductor is developed as a prototype artificial peripheral nerve. The results indicate that the LM cuff electrode has high flexibility and maintains outstanding conductivity. After impla

Magnetic Resonance Image-Guided Focused Ultrasound Ventral Intermediate Nucleus Thalamotomy with Indwelling Globus Pallidus Internus Deep Brain Stimulation Electrodes: A Case Report.

INTRODUCTION: Deep brain stimulation (DBS) is the gold-standard surgical treatment for essential tremor (ET) and tremor-predominant Parkinson's disease (TdPD). However, despite appropriate electrode placement and programming, patients may develop tremor recurrence due to disease progression. MRI-guided focused ultrasound (MRgFUS) thalamotomy is an alternative treatment to DBS and has been shown to yield durable tremor control in both ET and TdPD patients. However, MRgFUS thalamotomy in a patient with indwelling DBS electrodes has not been previously reported. CASE PRESENTATION: We present the

A Liquid-Metal-Based Magnetoactive Slurry for Stimuli-Responsive Mechanically Adaptive Electrodes.

Electrical communication between a biological system and outside equipment allows one to monitor and influence the state of the tissue and nervous networks. As the bridge, bioelectrodes should possess both electrical conductivity and adaptive mechanical properties matching the target soft biosystem, but this is still a big challenge. A family of liquid-metal-based magnetoactive slurries (LMMSs) formed by dispersing magnetic iron particles in a Ga-based liquid metal (LM) matrix is reported here. The mechanical properties, viscosity, and stiffness of such materials rapidly respond to the stimulu

Medical, surgical and audiological complications of the first 100 adult cochlear implant patients in Birmingham.

Of the first 100 patients implanted on the Midland Cochlear Implant Programme the commonest aetiologies of deafness were idiopathic 31 per cent, meningitis 28 per cent and cochlear otosclerosis 16 per cent. The major complication rate was three per cent. The most severe was one individual who post-operatively developed a cerebral infarct and subsequently died. The minor complication rate was 39 per cent, all of which successfully resolved, and included 11 cases of wound infection, nine cases of vertigo, three transient facial palsies and two post-operative bleeds. Older patients and men were m

Attenuation of self-stimulation from substantia nigra but not dorsal tegmental noradrenergic bundle by lesions of sulcal prefrontal cortex.

Rats that self-stimulated from electrodes implanted in either the substantia nigra pars compacta (SNC) or the dorsal tegmental noradrenergic bundle (DTB) received bilateral electrolytic lesions of the prefrontal cortex dorsal to the rhinal sulcus. Immediately after the lesions, animals with SNC electrodes (n = 9) showed significant and permanent reductions in self-stimulation rates. The average reduction in the SNC group was 67% of prelesion bar-pressing scores. In contrast, rats with DTB electrodes (n = 11) were not significantly affected by similar sulcal lesions. Subsequent examination of t

The Safety of Electroconvulsive Therapy in Patients With Implanted Deep Brain Stimulators: A Review of the Literature and Case Report.

Currently there is no consensus statement about the safety of electroconvulsive therapy in patients who have implanted electrodes for deep brain stimulation. We present a summary of the existing literature on this topic, consisting of 21 cases, and then report a case performed at the University of Maryland Medical Center. Notably, with appropriate safety precautions and careful patient selection, there were no adverse events reported in the literature that were related to the presence of the deep brain stimulation device in any of the cases. Based on our review of the literature and the case w
