A patient with multiple myeloma developed gaze paresis to the left with slowed saccades and gaze-paretic nystagmus, which alternated with abduction palsy in the left eye (with preserved oculocephalic deviation) and dissociated adducting nystagmus in the right eye, suggesting so-called posterior internuclear ophthalmoplegia. At autopsy multiple small infarcts were found with partial destruction of the left paramedian pontine reticula formation (PPRF) extending towards the abducens nucleus, which was involved only in its inferior pole. The medial longitudinal fasciculus and other oculomotor stru

Saccadic intrusions disrupted forward fixation in four patients with bilateral internuclear ophthalmoplegia (INO). Three had multiple sclerosis and one had brainstem infarction. Sporadic bursts of monocular abducting saccades were recorded in either eye. Each saccade was followed by a slower return to midposition. The presence of the INO prevented conjugate saccades of the fellow eye. The adducting phases were faster than convergence eye movements or the glissadic smooth eye movements that sometimes follow saccades. Dynamic overshoot of the abducting saccadic phase may contribute to the relati

See-saw ocular movements are described in two patients, one having obstructive hydrocephalus and the other a thalamic infarct. Electro-oculographic studies demonstrated that the eye movements in patient 1 moved in and out of phase at irregular intervals, in both a horizontal and a vertical direction. The disconjugate eye movements were exaggerated in bright light and less evident during fixation. We suggest a lesion impairing the function of that circuitry of cells thought to include the nucleus centromedianus of the thalamus, the zona incerta, the interstitial nucleus of Cajal and eye movemen

Analysis of the ENG records of 150 patients with well substantiated neurologic disease involving the structure of the posterior fossa revealed a high incidence of diminished optic fixation inhibition (DOFI) of caloric induced nystagmus. DOFI is defined as less than 50 percent decrease of speed of the slow phase when eyes are open and fixated. The whole group was subdivided into seven subgroups according to type and location of disease and the incidence of DOFI was found to vary significantly in the different subgroups. The highest incidence was found in the group of cerebellar mass lesions and

OBJECTIVE: Rapid-Eye-Movement (REM) sleep behaviour disorder (RBD) is an early predictor of Parkinson's disease, dementia with Lewy bodies, and multiple system atrophy. This study investigated the use of a minimal set of sensors to achieve effective screening for RBD in the population, integrating automated sleep staging (three state) followed by RBD detection without the need for cumbersome electroencephalogram (EEG) sensors.
METHODS: Polysomnography signals from 50 participants with RBD and 50 age-matched healthy controls were used to evaluate this study. Three stage sleep classification wa

BACKGROUND: Patients with multiple sclerosis (MS) suffer from the repetitive yawning and sleep problems. Yawning is observed in MS and other central nervous system disorders and yawning and swallowing may be controlled by the network of the brain stem. Therefore it is important to investigate the MS patients with various clinical and radiological locations in order to understand the role of brainstem on the yawning mechanisms. One hour polygraphic recording would be crucial method for this purpose, because it is easy to observe spontaneous yawning (SY) and spontaneous swallowing (SS) together

BACKGROUND: Animal experiments demonstrated that there are vestibular cortical areas at the parietal cortex. Moreover, in humans, recent functional neuroimaging studies revealed that caloric stimulation activated the parietoinsular vestibular cortex and optokinetic stimulation activated the parieto-occipital cortex. These activations indicate that the parietal vestibular areas play some role in nystagmus generation or in spatial information processing in the eye movement tasks.
AIMS OF THE STUDY: The aim of this communication was to present a patient giving some information about parietal cor

OBJECTIVES: The objectives of this study were to compare (1) the sensitivity of simulated driving to self-report measures, nocturnal sleep latency tests (SLTs), and an auditory vigilance task and (2) urban and motorway driving.
METHODS: Healthy males 18 to 35 years maintained wakefulness for one night and were tested at 2400, 0230, 0500 and 0730 h. In Study 1 (n=11), the SLTs were followed by auditory vigilance and simulated driving tasks; in Study 2 (n=18), the SLTs were preceded and followed by simulated driving on motorway and urban routes.
RESULTS: In Study 1, speed variability, tracking

BACKGROUND: The ratios of abducting to adducting eye movements (versional dysconjugacy index, VDI) for saccadic velocity and acceleration have been useful measures by which to objectively characterize internuclear ophthalmoparesis (INO). Amplitude measures of dysconjugacy have been less useful, given that many patients maintain the ability to ultimately reach a centrifugal fixation target and that traditional amplitude measures of VDI have focused on this 'final amplitude' (FA) position.
METHODS: We utilized infrared oculography to define a new amplitude measure of dysconjugacy in 42 multiple

OBJECTIVES: The current gold standard for diagnosis of benign paroxysmal positional vertigo (BPPV) is the Dix-Hallpike maneuver. However, because of fatigability, the Dix-Hallpike is often falsely normal. The objective of this study was to evaluate the utility of vestibular autorotation testing in the diagnosis of BPPV.
METHODS: The charts of 210 patients at a tertiary referral center for vertiginous disorders were reviewed. All patients underwent clinical evaluation, Dix-Hallpike testing, audiometry, electronystagmography, and vestibular autorotation testing. The vestibular autorotation resu

We investigated the activation of frontal and occipital cortical areas in 14 normal volunteers during voluntary saccades in light or dark and during imagined saccades using functional magnetic resonance imaging (FMRI) with electro-oculogram monitoring. Voluntary saccades in light or dark and imagined saccades led to a significant activation (p < 0.005) of the precentral and posterior medial frontal gyrus (frontal eye field). The medial part of the superior frontal gyrus (supplementary eye field) also showed significant activity during voluntary saccades in all subjects, but only in four subjec

We used functional magnetic resonance imaging to explore, in individual subjects, the properties of the kinetic occipital (KO) region, which previous position emission tomography studies have shown to be involved in the processing of kinetic boundaries. The KO region was significantly activated in 23/25 subjects tested in the subtraction of uniform motion from kinetic gratings. The KO region is genuinely specialized for processing kinetic boundaries since it is significantly more activated by kinetic gratings than by luminance-defined gratings, uniform motion or transparent motion. This leaves
