Survival of ovarian adenocarcinoma patients depends on the size of the largest residual nodule at the end of debulking surgery. The argon beam coagulator (ABC) delivers radiofrequency current traveling in a beam of argon gas which facilitates tumor destruction and hemostasis. We used the ABC as an aid to cytoreduction in seven consecutive patients with stage III/IV ovarian cancer. Despite extensive disease, optimal debulking was achieved in all seven patients: four (57%) had no gross residual cancer; three had residual nodules of 2-3 mm. Ten anesthetized dogs were studied in an attempt to provoke partial alterations in atrioventricular (AV) conduction by high-frequency current (HFC) transcatheter ablation. Trigeminal neuralgia nearly always can be relieved initially with drug therapy. Long-term relief, however, is achieved in only 25 percent of patients. When medical therapy fails, surgery may be necessary. Percutaneous rhizotomy is an effective technique for pain relief, with few medical contraindications. Posterior fossa exploration and neurovascular decompression or sensory root sectioning is reserved for those patients who do not improve after rhizotomy and for selected younger patients.

Twenty-five patients with malignant vulvar melanoma have been treated with radical vulvectomy using the warm knife-open wound technique. Following preoperative radiation therapy, dissection of the groin and, in some cases, pelvic lymphadenectomy, were performed as a second procedure. Crude five-year survival was 9/21 (approximately 43%). Patients with tumours of Clark level II--IV have been found to have a good prognosis (7/9), while those with Clark V-tumours have had an extremely poor prognosis (2/10). The minimum voltage needed to perform laparoscopic sterilization was studied to learn more about prevention of bowel burns. Using a "CUT" waveform at very low levels, it is practical to coagulate fallopian tubes with as little as 100 volts peak. If electrosurgical cutting of fulguration (charring) is desired, the nimimum voltage is about 700 volts peak. The changes in electrical resistance of tissue were measured during coagulation of sample tissues and actual sterilizations. A discussion of basic electrosurgical principles is included.
