Professional, structural and organisational interventions in primary care for reducing medication errors.

BACKGROUND: Medication-related adverse events in primary care represent an important cause of hospital admissions and mortality. Adverse events could result from people experiencing adverse drug reactions (not usually preventable) or could be due to medication errors (usually preventable). OBJECTIVES: To determine the effectiveness of professional, organisational and structural interventions compared to standard care to reduce preventable medication errors by primary healthcare professionals that lead to hospital admissions, emergency department visits, and mortality in adults. SEARCH METHOD

Receipt of needed care coordination and emergency department utilization among US children with chronic physical pain: a pooled analysis of the 2022-2024 National Survey of Children's Health.

BACKGROUND: Children with chronic physical pain often require complex, multidisciplinary services across medical, behavioral, and community systems. However, whether receipt of needed care coordination is associated with acute healthcare utilization and care access in this population remains poorly characterized. METHODS: We conducted a pooled cross-sectional analysis of the 2022-2024 National Survey of Children's Health (NSCH). The analytic cohort included US children aged 6-17 years with caregiver-reported chronic physical pain who had an identified need for care coordination. The exposure

The portrayal and perceptions of cesarean section in Mexican media Facebook pages: a mixed-methods study.

BACKGROUND: Mexico has one of the highest rates of cesarean sections globally at over 45%. There is limited research about social factors influencing these rates. This study explores the portrayal and perceptions of cesarean section in Facebook media pages to better understand the socio-cultural context of childbirth in Mexico. METHODS: This is a mixed-methods social media analysis using two data sources. First, to study the portrayal of cesarean section, we identified ten Mexican media Facebook pages with the largest audiences (based on number of page "likes"). We searched these pages for ar

Control of human parasitic diseases: Context and overview.

The control of parasitic diseases of humans has been undertaken since the aetiology and natural history of the infections was recognized and the deleterious effects on human health and well-being appreciated by policy makers, medical practitioners and public health specialists. However, while some parasitic infections such as malaria have proved difficult to control, as defined by a sustained reduction in incidence, others, particularly helminth infections can be effectively controlled. The different approaches to control from diagnosis, to treatment and cure of the clinically sick patient, to

Management of minor burns during the COVID-19 pandemic: A patient-centred approach.

INTRODUCTION: The UK government introduced lockdown measures on 23 March 2020 due to the first wave of the COVID-19 pandemic. A restructuring of clinical services was necessary to accommodate mandatory changes while also maintaining the best possible standards for patient care. The present study explored the initial management, follow-up and patient-reported outcomes of burn injuries <15% total body surface area (TBSA) during the height of the COVID-19 lockdown at a tertiary burns centre. METHODS: A retrospective review of all adult patients with burns <15% TBSA during the national lockdown (

Maturity assessment of KenyaEMR: advancing digital health solutions through continuous improvement in Kenya.

BACKGROUND: Kenya Electronic Medical Records (KenyaEMR) has improved data availability for patient services and programmatic purposes in Kenya. Maturity models provide a framework to assess health information systems (HIS) capabilities across maturity levels to ensure they are effective. This study aimed to determine KenyaEMR's current and goal maturity status, highlight gaps, and establish priorities for continuous improvement. METHODS: The HIS Stages of Continuous Improvement (SOCI) Toolkit guided KenyaEMR's maturity assessment across five HIS domains and five maturity stages: 1-emerging/ad

Towards decentralization of Salmonella serotyping and risk assessment in poultry production environments with nanopore sequencing.

Salmonella enterica is a highly diverse group of organisms with over 2,500 serovars described to date. However, not all serovars are pathogenic to humans and animals. Many serovars typically cause self-limiting gastrointestinal infections, but certain host-adapted serovars, as well as those expressing specialized virulence factors (e.g., Vi capsule) have an increased potential to cause bacteremia and trigger systemic responses, necessitating immediate medical attention. Timely identification of Salmonella at the serovar level is, therefore, critical for informing risk assessments, guiding clin

Attitudes and Willingness to Participate in Drug Clinical Trials Among Patients With Cancer: Multistage Qualitative Study.

BACKGROUND: Cancer clinical trials are essential for advancing therapeutic innovations; however, patient enrollment remains a persistent challenge globally. Understanding the attitudes and willingness of patients with cancer to participate in clinical trials is critical for improving recruitment strategies. While previous studies have explored barriers and facilitators, few have integrated multiple data sources or used emerging analytical approaches, such as large language models (LLMs), to capture the multidimensional nature of patient decision-making. Furthermore, limited research has examin

On the occurrence of mirror modes at Mercury and their comparison with magnetic holes and ion cyclotron waves.

In the context of this review, civil unrest is defined as disharmony, expressive dissatisfaction and/or disagreement between members of a community, which leads to a situation of competitive aggression that may find expression as disruption of organisation, conflicts, damage to property and injuries. Such a breakdown of harmonious relationships, which may result in property damage and human injuries that may be threatening to life, varies in magnitude from participation of a very few individuals up to the involvement of large crowds of people, which may evolve into a full-scale riot. It is the

Use of automated external defibrillators in cardiac arrest: an evidence-based analysis.

OBJECTIVE: The objectives were to identify the components of a program to deliver early defibrillation that optimizes the effectiveness of automated external defibrillators (AEDs) in out-of-hospital and hospital settings, to determine whether AEDs are cost-effective, and if cost-effectiveness was determined, to advise on how they should be distributed in Ontario. CLINICAL NEED: Survival in people who have had a cardiac arrest is low, especially in out-of-hospital settings. With each minute delay in defibrillation from the onset of cardiac arrest, the probability of survival decreases by 10%.

Beyond the Algorithm: The ERC Guidelines 2025 and the Maturity of Resuscitation Medicine.

The European Resuscitation Council Guidelines 2025, officially launched as the Guidelines on Cardiopulmonary Resuscitation 2025 on 22 October 2025 in Rotterdam and published in Resuscitation (1) as a thematic collection of guideline papers, represent far more than a scheduled update of resuscitation algorithms. That reading would be too narrow. The ERC Guidelines 2025 are better understood as a portrait of a discipline that has entered a new phase in its understanding of the continuum of resuscitation care. In these guidelines, resuscitation medicine is not reduced to the dramatic moment of ca

A pilot randomised controlled trial of community-led ANtipsychotic Drug REduction for Adults with Learning Disabilities.

BACKGROUND: Data suggest that approximately 50,000 adults with learning disabilities (LDs) in England and Wales are currently prescribed antipsychotic medication. Illness in this population is common, including significant rates of challenging behaviour and mental illness, but there is particular concern over the use of antipsychotics prescribed for reasons other than the treatment of psychosis. Control of challenging behaviour is the primary reason why such medications are prescribed despite the absence of good evidence for any therapeutic effect for this purpose. OBJECTIVES: To assess the f

Implementing the 2009 Institute of Medicine recommendations on resident physician work hours, supervision, and safety.

Long working hours and sleep deprivation have been a facet of physician training in the US since the advent of the modern residency system. However, the scientific evidence linking fatigue with deficits in human performance, accidents and errors in industries from aeronautics to medicine, nuclear power, and transportation has mounted over the last 40 years. This evidence has also spawned regulations to help ensure public safety across safety-sensitive industries, with the notable exception of medicine. In late 2007, at the behest of the US Congress, the Institute of Medicine embarked on a year

Immunization coverage levels among 19- to 35-month-old children in 4 diverse, medically underserved areas of the United States.

BACKGROUND: The National Immunization Survey demonstrates that national immunization coverage in 2002 remained near the all-time highs achieved in 2000. However, that survey cannot detect whether coverage is uniformly high within relatively small areas or populations. The measles resurgence in the early 1990s revealed that coverage was low in some areas, particularly among inner-city children from racial and ethnic minority groups. Today, identifying areas with low childhood-vaccination coverage remains important, particularly if these areas are at risk for the introduction of disease. In 1995

A comprehensive review of prioritised interventions to improve the health and wellbeing of persons with lived experience of homelessness.

BACKGROUND: Homelessness has emerged as a public health priority, with growing numbers of vulnerable populations despite advances in social welfare. In February 2020, the United Nations passed a historic resolution, identifying the need to adopt social-protection systems and ensure access to safe and affordable housing for all. The establishment of housing stability is a critical outcome that intersects with other social inequities. Prior research has shown that in comparison to the general population, people experiencing homelessness have higher rates of infectious diseases, chronic illnesses

On the Occasion of the 80th Anniversary of the Faculty of Medicine of Charles University in Hradec Králové.

Last autumn we commemorated the 80th anniversary of the founding of the Faculty of Medicine of Charles University in Hradec Králové. A number of events were held and several publications were released, including a special issue of the Časopis lékařů českých entirely dedicated to this anniversary and to the faculty. At the ceremonial assembly of the academic community on 8 October 2025, the dean of the faculty, Professor Manďák, delivered an extensive speech in which he clearly described the historical circumstances of the faculty's foundation, its beginnings, development, and growth through va

Implementation strategies for health systems in low-income countries: an overview of systematic reviews.

BACKGROUND: A key function of health systems is implementing interventions to improve health, but coverage of essential health interventions remains low in low-income countries. Implementing interventions can be challenging, particularly if it entails complex changes in clinical routines; in collaborative patterns among different healthcare providers and disciplines; in the behaviour of providers, patients or other stakeholders; or in the organisation of care. Decision-makers may use a range of strategies to implement health interventions, and these choices should be based on evidence of the s

Pediatric Simulation-Based Prehospital Training Course in Botswana.

AUDIENCE: This simulation-based training focuses on the most common and high risk pediatric prehospital scenarios in low- and middle-income countries (LMIC). The curriculum was developed based on a needs assessment to train Ministry of Health and Wellness (MOHW) prehospital providers in Botswana specifically for pediatric resuscitation and could be used for emergency medical services (EMS) providers in other LMIC. After participating in this curriculum, providers should enhance their assessment and interventions in acutely ill pediatric prehospital patients. LENGTH OF CURRICULUM: The entire c

The path to healthy ageing in China: a Peking University-Lancet Commission.

UNLABELLED: Around the world, populations are ageing at a faster pace than in the past and this demographic transition will have impacts on all aspects of societies. In May 2020, the UN General Assembly declared 2021–2030 the Decade of Healthy Ageing, highlighting the importance for policymakers across the world to focus policy on improving the lives of older people, both today and in the future. While rapid population ageing poses challenges, China’s rapid economic growth over the last forty years has created space for policy to assist older persons and families in their efforts to improve he
