Capillary refill for the assessment of circulatory dysfunction in sepsis: A scoping review.

BACKGROUND: Sepsis remains a significant and difficult diagnosis in US Emergency Departments and Intensive Care Units. OBJECTIVE: The objective of this scoping review was to understand the state of the literature regarding the performance of Capillary Refill Time [CRT] as a method to assess circulatory dysfunction in sepsis. DESIGN: This study followed PRISMA Guidelines for Scoping Reviews. Included studies used CRT as a physical exam finding 'measuring the time of return of color to a blanched nailbed' and was limited to Emergency Department and Intensive Care Unit settings. Care of the Patient With Asthma.

Nearly 8% of the U.S. population is diagnosed with asthma, leading to more than 5 million office visits and 1 million emergency department visits annually. Both outpatient and inpatient internal medicine clinicians treat asthma frequently, but nuances in diagnosis and management have emerged. This article highlights many of these developments.

Delirium Prevention and Management in Older Adults in the Emergency Department.

Delirium, acute brain dysfunction, is present in 10% to 35% of older adults in the emergency department (ED) but unrecognized in ∼80% of cases leading to significant adverse outcomes. Thus, routine screening for delirium is vital to improve prevention and management in the ED. The treatment of delirium focuses on addressing the underlying cause. For agitation, nonpharmacologic measures using the Tolerate, Anticipate, and Don't Agitate (TADA) approach and the Assess, Diagnose, Evaluate, Prevent, and Treat (ADEPT) tool are prioritized for management. If unsuccessful, only the lowest effective do

Benzodiazepine-refractory status epilepticus: A narrative review.

INTRODUCTION: Seizures are a common complaint in the emergency department, with status epilepticus (SE) associated with significant morbidity and mortality. While the typical first-line therapy for seizures includes benzodiazepines, patients with SE may be refractory to benzodiazepines. Seizures of longer duration are harder to break, and SE becomes a self-sustaining cycle; thus, benzodiazepine-refractory SE is a particularly challenging condition facing emergency clinicians. OBJECTIVES: This narrative review provides a focused evaluation of SE refractory to first-line therapy, highlighting p

Fever in the Not-So-Typical Pediatric Patient.

Fever is one of the most common chief complaints for patients in the pediatric emergency department (ED). Most of the time, these patients are diagnosed with a virus and treated with supportive care; however, there are a subset of patients with chronic conditions or special circumstances that require a more thorough approach when they enter the ED with a fever. This article will go into detail about why we must be on high alert when these children present with a fever and how we should approach them medically.

Atraumatic Back Pain.

Atraumatic back pain is a common condition affecting a significant portion of adults. Approximately 80% of individuals will experience it during their lifetime, making it a frequent reason for primary care and emergency visits. In the United States, about 4.4 million emergency department (ED) visits in 2021 were due to back pain, representing 2% to 4% of all ED visits. While most cases are benign, serious spinal pathologies occur in 2.5% to 5.1% of ED presentations, requiring prompt diagnosis and management. This review categorizes causes into emergent, "worst first" conditions and less urgent

Primary Headache.

Most patients presenting with headaches to the emergency department (ED) are experiencing benign exacerbations of chronic conditions. The ED focus is on identifying dangerous secondary causes of headaches and providing pain relief. Given the high recurrence rate, emergency physicians must optimize acute treatment, address unmet needs, and ensure appropriate outpatient follow-up.

The Role of Troponin Testing in Patients with Supraventricular Tachycardia, Systematic Review and Meta-Analysis.

BACKGROUND: Supraventricular tachycardia (SVT) is commonly evaluated in the emergency department (ED). While troponin has been shown to be elevated in SVT, its usefulness for predicting coronary artery disease and future adverse cardiovascular outcomes has not been shown. OBJECTIVES: We aimed to evaluate the prognostic utility of troponin measurement as part of SVT management in the ED. METHODS: We performed a literature search in the PubMed and Scopus databases from inception to August 30, 2023, including all studies reporting troponin measurements in adult patients (age > 18 years) present

Systematic review of parenteral ketamine for managing acute agitation in emergency settings.

INTRODUCTION: Agitation, a significant psychiatric issue often linked to conditions like schizophrenia, bipolar disorder, and major depression, invariably pose challenges in emergency settings. Acute agitation occurs in 2.6 % of emergency cases and up to 12.2 % among patients with schizophrenia or bipolar disorder. With limited available options for effective management of acute agitation, ketamine is emerging as a fast-onset alternative to antipsychotics like haloperidol. METHODS: This systematic review, following PRISMA guidelines, searched databases in April 2024 for studies on parenteral

[Typical causes and clinical presentations of traumatic and nontraumatic soft tissue and organ haemorrhage in emergency departments].

Acute bleeding often occurs in a wide variety of forms in clinical emergency medicine. In traumatic bleeding, soft tissue injuries to the head or extremities are common, while severe bleeding is less common in chest injuries or pelvic trauma. In nontraumatic emergencies, gastrointestinal bleeding is the leading cause, but many other bleeding entities are possible. Structured management helps to detect the critically ill patient, identify the possible source of bleeding and provide appropriate radiological diagnostics and therapy. Clinical examination, blood gas analyses and emergency ultrasoun

Consensus paper on the management of acute isolated vertigo in the emergency department.

Acute vertigo is defined as the perception of movement of oneself or the surroundings in the absence of actual motion and it is a frequent cause for emergency department admissions. The utilization of medical resources and the duration of hospital stay for this kind of symptom is high. Furthermore, the efficiency of brain imaging in the acute phase is low, considering the limited sensitivity of both CT and MRI for diagnosing diseases that are the causes of central type of vertigo. Relying on imaging tests can provide false reassurance in the event of negative results or prolong the in-hospital
