Magnetic resonance imaging of guinea pig cochlea after vasopressin-induced or surgically induced endolymphatic hydrops.

OBJECTIVE: To investigate the ability to detect the in vivo cochlear changes associated with vasopressin-induced and surgically induced endolymphatic hydrops using MRI at 3 tesla (T). STUDY DESIGN: Prospective, animal model. SETTING: Animal laboratory. SUBJECTS AND METHODS: In group 1, five guinea pigs underwent post-gadolinium temporal bone MRI before and after seven and 14 days of chronic systemic administration of vasopressin by osmotic pump. In group 2, five guinea pigs underwent temporal bone MRI eight weeks after unilateral surgical ablation of the endolymphatic sac. Three-tesla high-…

[Clinical electrocochleography in Menière's disease].

Transtympanic electrocochleography (ECochG) was performed on 137 definite Menière ears, 29 normal ears and 383 ears with other diseases. The … gr of glycerol (50% solution, 100ml) and isosorbide (70% solution, 2.0gr/kg of body weight). After endolymphatic sac surgery a decrease of SP/AP of 10% or more was found in 5 ears, an increase of SP/AP in 3 ears, and no change of SP/AP in 3 ears. In the patients showing no change of SP/AP after the surgery, numerical values of vertigo by AAOHNS were higher than in …

Comparative genomic profiling of SLC26A4-expressing cells in the inner ear and other organs.

Pendred syndrome and autosomal recessive non-syndromic hearing loss, type 4 (DFNB4), are associated with mutations in SLC26A4 that encodes the anion transporter SLC26A4 (pendrin). SLC26A4 is expressed in mitochondria-rich cells of the endolymphatic sac, spindle and root cells in the cochlear lateral wall, transitional cells in the vestibular organs, follicular cells in the thyroid, and type B intercalated cells in the kidney. This study aimed to assess the gene profiles of murine Slc26a4-expressing cells to better understand the regulatory mechanisms and functions of SLC26A4. Publicly availabl…

[Selection of surgical methods for Méniére's disease].

119 cases (cars) of Meniere's disease operated on with different surgical methods were reevaluated with the criteria (Chinese ENT Association, 1996). 48 cases in second stage underwent endolymphatic sac shunt (ESS); 32 cases in third stage underwent scarpa ganglionectomy (SGN) and 12 cases in the same stage ESS; 16 cases in fourth stage vestibulocochlear neurectomy(VCN) and others ESS(7 cases) and SGN(4 cases). The results showed that 1. ESS successfully relieved vertigo in 79.2 per cent of the patients in second stage (reversible stage), and their hearing tended to stabilize(35.4%); 2. SGN wa…

Meniere's disease at the University of Iowa: 1973 to 1980.

The goals of treatment in Meniere's disease are hearing preservation and control of vertigo. Controversy abounds with regard to the most expeditious and efficacious means of attaining these goals. Low-salt diet, diuretic therapy, endolymphatic-mastoid shunt, and middle cranial fossa vestibular neurectomy are all modes of treatment employed for Meniere's disease at the University of Iowa. This review of our experience from 1973 to 1980 summarizes the efficacy of each therapeutic option.

What I think of sac surgery in 1989.

The senior author has performed shunt procedures routinely for medically refractory cases of Meniere's disease starting in 1964 and ending in 1984. Five hundred and sixty-eight consecutive patients were operated on and followed from 1970 to 1984. Combining the cases of C. G. Jackson, there have been a total of 676 procedures done at the Otology Group. Initially endolymphatic-subarachnoid (ES) shunts were used, then endolymphatic-mastoid shunts (EM), and most recently, the Denver valve has been employed. No significant difference between these procedures was noted in the control of vertigo or s

[Contribution to the therapy of Menière's disease (author's transl)].

A case report is presented about a patient with typical Menière's disease of the left inner ear. The surgical therapy by means of saccotomy brought only slight improvement. After consultation of the maxillofacial surgeon an impact upper left wisdom tooth was discovered and surgically treated. since this time the symptoms of the Menière's disease have disappeared. Possible connections concerning the localization of the wisdom tooth and the neighbouring anatomical structures are discussed and give a possible clue to a new form of therapy.

Diabetes mellitus and hearing loss: clinical and histopathologic relationships.

We compared audiometric and clinical histories with findings in temporal bones of eight diabetics and ten normal controls matched for age and sex. The group with diabetes mellitus had significantly more hearing loss than the normal control group (p less than .01). Only patients with diabetes had microangiopathy. Patients with microangiopathic involvement of the endolymphatic sac had significantly greater hearing loss than patients without such involvement (p less than .01). Microangiopathy in the stria vascularis was highly significant in the diabetics (p less than .001); however, they did not…

Hormonal aspects of Ménière's disease on the basis of clinical and experimental studies.

CONCLUSIONS: Endolymph homeostasis is thought to be mediated by the vasopressin-aquaporin-2 (VP-AQP2) system in the inner ear. Endolymphatic … sin type 2 receptor were expressed in the stria vascularis of the cochlea and the epithelium of the endolymphatic sac, and that the volume of the endolymphatic compartment was mediated by the activity of the VP-AQP2 system in the inner ear.

Th1: mediator lymphocytes in experimental autoimmune labyrinthitis.

A previous study on experimental autoimmune labyrinthitis (EAL) consistently demonstrated transient infiltration of lymphocytes only into the inner ear of mice. To clarify the profile of lymphocytes in the initiation of EAL, the present study investigated cell surface antigens, as well as cytokines, from Day 4 to Day 35, using immunohistochemical techniques. Many CD4+ cells mainly infiltrated the endolymphatic sac as early as Day 4 and gradually spread to the rest of the inner ear. Infiltration peaked on Day 12 and persisted in most animals until Day 35, although the number of cells gradually …

Expression of intercellular adhesion molecule-1 during inner ear inflammation.

This study was designed to investigate the expression of intercellular adhesion molecule-1 (ICAM-1) on the spiral modiolar vein (SMV) with its collecting venules (CVs) and the venules of the endolymphatic sac during inner ear inflammation. These data will further elucidate the role of adhesion molecules in extravasation of inflammatory cells from blood vessels during an inner ear immune response. Labyrinthitis was induced in rats by inoculation of keyhole limpet hemocyanin into the scala tympani of animals who had been systemically sensitized to it. Expression of ICAM-1 was examined with a mou…

History of Meniere's disease and its clinical presentation.

The term Meniere's disease is used to define either the classic triad of vestibular and cochlear symptoms and aural pressure from known or unknown causes or its clinical variants, vestibular and cochlear Meniere's disease. Some variants evolve after years into typical forms, whereas others do not. Some symptoms (positional vertigo) have been long underestimated in previous reports. The more we study our patients and correlate clinical findings and the natural history with pathologic studies on temporal bones and laboratory research, the more we will understand Meniere's disease and its causes.

Inner ear pathologies impair sodium-regulated ion transport in Meniere's disease.

Meniere's disease (MD), a syndromal inner ear disease, is commonly associated with a pathological accumulation of endolymphatic fluid in the inner ear, termed "idiopathic" endolymphatic hydrops (iEH). Although numerous precipitating/exacerbating factors have been proposed for MD, its etiology remains elusive. Here, using immunohistochemistry and in situ protein-protein interaction detection assays, we demonstrate mineralocorticoid-controlled sodium transport mechanisms in the epithelium of the extraosseous portion of the endolymphatic sac (eES) in the murine and human inner ears. Histological …

[Quantification of endolymphatic hydrops in experimental animal model].

OBJECTIVE: To investigate a method of quantification of the endolymphatic hydrops in experimental animal model. METHODS: Thirty guinea pigs were divided into three groups at random. In control group, there were ten guinea pigs without operation on both ears. Endolymphatic hydrops was induced by endolymphatic sac obliteration through extradural posterior cranial fossa approach in right ear, including 4-week postoperative group(n = 10) and 8-week postoperative group(n = 10). The area of scala vestibuli (SV) and scala media(SM) of each turn on both cochlear midmodiolar sections was measured, res…

A preliminary report on the correlation of vestibular Meniére's disease with electrocochleography and glycerol test.

Objective evaluation of vestibular Meniére's disease has been difficult due to the lack of audiometric findings. Thus inevitably, the existe … confirmed if either test result was positive. A retrospective collection of 24 cases who underwent endolymphatic sac surgery was made to evaluate in retrospect the existence of endolymphatic hydrops indirectly proved by successful surgery. Results showed a positive SP/AP ratio in 25 cases (62.5%) of which 7 had bilateral positive responses. The glycerol test was p …
