The problem in differentiation between psoriatic-related polyenthesitis and fibromyalgia.

The recognition of the primacy of enthesitis in animal models of spondyloarthritis and the prevalence of clinically occult enthesopathy in psoriatic subjects and of persistent joint pain in PsA subjects who have ostensibly good reduction of joint swelling under biological therapy has highlighted the potential impact of polyenthesitis in psoriatic disease. In daily practice, the formal demonstration of enthesitis is challenging for the following reasons: the relatively avascular nature of enthesis, often leading to the absence of overt clinical inflammatory signs; the frequent lack of elevation…

Prevalence, presentation and treatment of lower limb pathologies in juvenile idiopathic arthritis: A narrative review.

Juvenile idiopathic arthritis is a chronic, autoimmune, inflammatory joint disease. It is the most common arthritis in children and adolescents. This paper reviews the presentation and treatment of lower limb pathologies in juvenile idiopathic arthritis from an allied health perspective. Common lower limb pathologies include: synovitis causing swelling, tenderness and pain; persistent inflammation leading to flexion contractures; limb length discrepancies; muscle atrophy; enthesopathies such as plantar fasciitis and Achilles tendonitis; and tenosynovitis. Allied health professionals may use a

Brief communication: paleopathology of the Kiik-Koba 1 Neandertal.

The Kiik-Koba 1 Neandertal partial skeleton (canine, partial hands, partial leg, and feet), of a approximately 40-year-old probable male, exhibits a suite of pathological lesions, including hypercementosis, minor fibrous ossifications, pedal phalangeal fracture, and pronounced enthesopathies on the patella and calcanei in the context of no articular degenerations. The first two sets of lesions are related to age in the context of advanced dental attrition and physical strains. The third lesion joins a series of healed minor traumatic lesions among the Neandertals. The last represents either pr

Musculoskeletal manifestations in inflammatory bowel disease.

Musculoskeletal manifestations are the most common extra-intestinal complication of inflammatory bowel disease (IBD). They are part of the clinical spectrum of spondylarthropathies and include different articular manifestations. In addition to axial symptoms, peripheral findings such as seronegative oligoarthritis, dactylitis, and enthesopathy commonly occur, sometimes representing the only manifestation. Wide ranges of prevalence have been reported, depending on the criteria used to define spondyloarthropathy and on the selection of patients. In an inceptional cohort of newly diagnosed IBD pa…

[Anatomical study of the yellow ligament of the spine with special reference to its ossification].

In thoracolumbar spine, the interlaminar and the capsular portion of the yellow ligament differed in their location and direction of elastic … findings. Small ossification in the yellow ligament of a cadaver was considered to be "degenerative enthesopathy (enthesophyte)" originating from "enthesis". But in operative cases, the ossification had the same direction and location as those of the small ossification seen in a cadaver, but endogenous factors were strongly suggested because of the extensive c …

Concave impressio ligamenti costoclavicularis ("rhomboid fossa") and its prevalence and relevance to clinical practice.

PURPOSE: The incidence of fossa rhomboidea (RF) and its relevance to clinical practice were retrospectively studied in 1,017 cases of patien … s to RF. The bilateral RF was asymmetrical in its appearance. One of the patients suffered from LCC enthesopathy. The difference of the distribution of RF throughout the genders was 10.5% in females and 89.5% in males, marking a difference that is statistically significant (p < 0.0001). Only in 0.3%, the fracture line occurred in RF, which did not prove the RF …

Patient-reported complications associated with Campylobacter jejuni infection.

This study aimed to investigate the occurrence of complications, especially musculoskeletal symptoms, after sporadic Campylobacter jejuni en … n was most commonly reported (81%). The incidence of reactive arthritis was 4% and that of Achilles enthesopathy and/or heel pain was 9%. Stomach ache during enteritis was associated with the later development of joint pain. Antimicrobial treatment was common but did not prevent complications.

Sonographic analysis of the ankle in patients with psoriatic arthritis.

Foot involvement is very frequent in patients affected by psoriatic arthritis (PsA). However, evaluation of the painful foot can be problematic, because it is often difficult to distinguish between arthritis, tenosynovitis, and enthesopathy. Plain radiographs can show bone erosion or other features of joint involvement, but give little information about the soft tissues. We therefore studied foot involvement in 31 PsA patients using high resolution sonography, and compared the results with the findings on x-ray and clinical examination. Ultrasound revealed pathological findings in a large prop…

Effects of secukinumab on synovitis and enthesitis in patients with psoriatic arthritis: 52-week clinical and ultrasound results from the randomised, double-blind ULTIMATE trial with open label extension.

OBJECTIVES: In the ULTIMATE study with an open label extension, we assessed the long-term effect of secukinumab at tissue level on synovitis and enthesitis, and across all psoriatic arthritis (PsA) manifestations, using both clinical evaluations and power Doppler ultrasonography (PDUS). METHODS: This randomised, placebo-controlled, Phase 3 study (ULTIMATE) included biologic-naïve patients with PsA with active PDUS synovitis and clinical enthesitis, and inadequate response to conventional synthetic disease-modifying antirheumatic drugs. The study consisted of 3 treatment periods; in the first

Osteoarthritis, entheses, and long bone cross-sectional geometry in the Andes: Usage, history, and future directions.

Akin to approaches encouraged by Verano (1997) in the Andes, and Ortner (2011, 2012) for general paleopathological studies, this article focuses on accurate descriptions and definitions of osteoarthritis, entheses, and long bone cross-sectional geometry. By evaluating these conditions as part of biological responses to abnormal skeletal changes and biomechanical stress, this research discusses each condition's pathogenesis. Further, this article emphasizes a "small data" approach to evaluating these conditions in ancient culturally and biologically related human populations, where the study sa

Clinical practice recommendations for the diagnosis and management of X-linked hypophosphataemia.

X-linked hypophosphataemia (XLH) is a rare metabolic bone disorder caused by pathogenic variants in the PHEX gene, which is predominantly expressed in osteoblasts, osteocytes and odontoblasts. XLH is characterized by increased synthesis of the bone-derived phosphaturic hormone fibroblast growth factor 23 (FGF23), which results in renal phosphate wasting with consecutive hypophosphataemia, rickets, osteomalacia, disproportionate short stature, oral manifestations, pseudofractures, craniosynostosis, enthesopathies and osteoarthritis. Patients with XLH should be provided with multidisciplinary ca

Psoriatic arthritis. When the heterogeneity requires normality.

Psoriatic arthritis (PsA) is characteristically associated with a large spectrum of disorders, some of which are peculiars, such as enthesopathy, dactilytis, osteitis and axial involvement. Due to the heterogeneity of its expression, definition and classification of PsA have been unsatisfactory until recent years, with consequences on the reliability of epidemiological studies. Other confounding factors for diagnosis and classification of PsA are the radiological changes, sometimes found in asymptomatic patients with psoriasis, and the frequent normality of acute phase response indices, in par…

Review of the use of ultrasound for the diagnosis and monitoring of enthesitis in psoriatic arthritis.

Musculoskeletal ultrasound is rapidly advancing, with contemporary machines offering exquisite high-resolution pictures of superficial struc … e framework of an enthesis organ concept. There are limited studies that search for the presence of enthesopathy in patients with psoriatic arthritis. Most of these examine lower extremity entheses with heterogeneous scoring methods. Similar indices have been used to examine patients with psoriasis to elucidate signs of early or occult psoriatic enthesitis. Al …

Pathogenetic and clinical rationale for TNF-blocking therapy in psoriatic arthritis.

The classical definition of psoriatic arthritis (PsA) as an inflammatory arthritis associated with psoriasis reflects only in part the large spectrum of musculoskeletal disorders found in patients with psoriasis. In particular, enthesopathy, dactilytis, osteitis and axial involvement are frequently neglected and probably account for the unsatisfactory response of PsA to traditional drugs, such as NSAIDs, steroids and DMARDs. Furthermore, these drugs showed only a partial ability to influence radiographic progression and psoriasis. The new anti-TNF agents, in particular etanercept but also infl…

Biochemical Predictors in the Blood Serum of Children with TORCH Encephalopathy and Autism.

TORCH-associated encephalopathy in children with autism spectrum disorder (ASD) is accompanied by a pronounced neurometabolic syndrome characterized by neuronal damage, mitochondrial energy deficiency, and disturbances in methylation processes. In the present study, children with ASD and confirmed TORCH-related central nervous system injury demonstrated significantly elevated serum levels of neuron-specific enolase (NSE), lactate, ammonia, and homocysteine, reflecting neuronal injury, mitochondrial dysfunction, and metabolic dysregulation. These alterations were more pronounced than in childre
