Mercury: major issues in environmental health.

In the past, methylmercury compounds were manufactured as fungicides or appeared as unwanted byproducts of the chemical industry, but today the methylation of inorganic mercury in aquatic sediments and soils is the predominant if not the sole source of methylmercury. This form of mercury is bioaccumulated to a high degree in aquatic food chains to attain its highest concentrations in edible tissues in long-lived predatory fish living in both fresh and ocean waters. It is well absorbed from the diet and distributes within a few days to all tissues in the body. It crosses without hindrance the b

The mutagenicity of organotin compounds as environmental pollutants.

The mutagenicity of 14 organotin compounds which have been reported to be environmental pollutants, their environmental metabolites and inorganic tin (SnCl4) was studied. The experiments were carried out by a modification of the conventional Salmonella assay. Each tested chemical was removed by washing the tested strain with phosphate buffer, before the strain with top agar was poured onto minimal glucose agar. By this method, we were able to estimate the mutagenicity of organotin compounds which had antibacterial activity. It was apparent that mono-n-butyltin oxide, n-butyltin trichloride, di

Vitamin A homeostasis endangered by environmental pollutants.

Normal vitamin A function depends on adequate stores of the vitamin, a finely regulated supply of the vitamin to target tissues, and an ability of cells to generate functionally active forms of the vitamin. Both endogenous and exogenous factors can adversely affect vitamin A homeostasis. Polyhalogenated aromatic hydrocarbons are ubiquitous environmental pollutants and cause severe disturbances in vitamin A metabolism, manifested by an accelerated metabolism and breakdown of vitamin A and its metabolites and a depletion of vitamin A from the body; this sequence of events accounts for the vitami

Genetic and environmental factors in systemic sclerosis.

The occurrence of autoantibodies in patients with systemic sclerosis has suggested a role for immune dysregulation in this disease. Recent genetic studies have concentrated on the major histocompatibility complex-encoded antigens and found an association of particular HLA-DQ alleles with anticentromere antibodies. Although the role of major histocompatibility complex antigens and autoantibodies in the pathogenesis of systemic sclerosis remains unclear, determination of major histocompatibility complex alleles may have clinical value in identifying patients who are at increased risk for develop

[Pseudofluorosis: psychologic aspects of environmental pollution].

A middle-aged woman who had been living in close surroundings of a fluorine-emitting factory for many years, was examined because she claimed to be suffering from skeletal fluorosis. Radiological, histological and laboratory findings did not give evidence of fluorosis. Fluoride concentrations were normal in samples of blood, bone, pooled urine, nails and hair. Even this proof to the contrary did not shake her conviction of many years that she was suffering from fluorosis. Thus, psychological aspects have to be considered in the assessment of health implications of pollutants.

Environmental and psychosocial determinants of sudden death.

The risk factors for sudden coronary heart disease (CHD) death have been well described. Sudden CHD deaths should be classified as those occurring in individuals with and those in individuals without a prior history of clinical heart disease. The extent of coronary artery disease, left ventricular dysfunction, and cardiac arrhythmias are the primary pathophysiologic determinants of ventricular fibrillation and sudden death. Psychosocial factors influence the threshold of response to the numerous physical and social environmental stimuli that can precipitate sudden death. The degree of patholog

Environmental factors in the etiology of Parkinson's disease.

Parkinson's disease (PD) has been proposed to result from the interaction of aging and environment in susceptible individuals. Defective metabolism of debrisoquine, inherited as an autosomal recessive, has been associated with this susceptibility. In 35 PD patients and 19 age-matched controls, no significant differences in debrisoquine metabolism were found, although a trend to impaired metabolism was noted in patients with disease onset less than or equal to 40. Foci of PD patients were associated with rural living and well water drinking, or rural living coupled with market gardening or wood

Landfill gas - a potential environmental hazard.

Methane derived from the decomposition of organic material contained within a landfill may escape beyond the site boundary where it can pose an explosion or fire hazard. Methods are described to prevent die occurrence of such lateral gas migration. Problems due to the accumulation of gas in buildings, erected on landfill sites, have occurred and techniques are now available to overcome these at some sites. However, it is recognized that at other sites, redevelopment should not be allowed to take place on die grounds of safety.

Loss of topographic familiarity. An environmental agnosia.

Sixteen patients manifested the syndrome of loss of environmental familiarity. The syndrome is characterized by an inability to recognize familiar surroundings in spite of relatively intact verbal memory, cognition, and perception. In addition to the loss of environmental familiarity, other clinical disturbances, including central achromatopsia, prosopagnosia, palinopsia, visual hallucinations, dressing disturbances, or impaired revisualization, were present in several cases. Radiologic studies revealed that all patients had right medial temporo-occipital lesions; three had additional left-sid

Clinical and environmental aspects of arsenic toxicity.

Arsenic is widely distributed throughout the animal and plant kingdoms and our environment where sources can be natural or anthropogenic. Agricultural uses of arsenic have declined recently, but it still has well-defined roles in industry. Small amounts of arsenic are metabolized in a variety of ways and are largely rapidly methylated and excreted by man and animals. Poisoning can occur and may follow an acute or chronic course. Toxic manifestations in man occur at the cellular level and may appear in many organ systems. Specific effects can often be demonstrated in the skin and in the vascula

Geographic and environmental factors in pediatric cancer.

It is important to determine the relative contributions of genetic and environmental factors to the etiology of childhood cancer in order to elucidate the pathogenic mechanisms involved and to develop effective means of primary prevention. Geographic differences in cancer incidence as well as changes in incidence over calendar time have long been used to generate clues to possible etiologic agents. The important role of genetic factors in childhood cancer is clear, and is exemplified by the observations in retinoblastoma. The importance of the contributions of environmental factors in general

Chromium: a review of environmental and occupational toxicology.

The following topics are covered in this brief review on the environmental and occupational toxicology of chromium: occurrence, production and uses of chromium and chromium compounds; experimental toxicology; chromium toxicity for man; hygienic and ecologic aspects of chromium contamination of the environment. The review provides a conclusive evidence which suggests that chromium, especially its hexavalent form, is both toxic and carcinogenic, but its trivalent form is physiologically essential in the metabolism of insulin. It is also emphasized that among the major sources of environmental ch

Environmental asbestos exposure and cancer mortality.

From 1970 to 1980, mortality from cancer of the lung and stomach was analyzed in a town with asbestos deposits and in a town with asbestos processing. The populations of the entire country, the province, the district, and of all communities of the same size and agricultural index were used as references. In the town with asbestos contamination of air and water from natural tremolite deposits and an endemic occurrence of pleural plaques, no increased risk for lung or stomach cancer was found. In the town with asbestos cement production lung cancer rates were lower in males and higher in females

Environmental hazards in milk and infant nutrition.

Epidemiologic studies have provided much of the knowledge concerning adverse side effects of exposure to drugs and other xenobiotics transmitted via breast milk. Adverse effects can be immediate and usually recognizable or long-term and difficult to define. Acute effects may be due to individual idiosyncrasy, allergic reaction, or manifestation of acute toxicity. We need further human clinical investigations to determine actual dosing to nursing infants and to ascertain the existence of subtle and presently unrecognized long-term effects.

Environmental and drug factors in hepatic porphyria.

Numerous drugs and environmental chemicals are capable of influencing the clinical expression of human hepatic porphyria primarily by interfering with the orderly regulation of heme synthesis in the liver. Some agents trigger the disease in otherwise normal individuals whereas others exacerbate an underlying genetic abnormality leading to disease expression. In both instances careful avoidance of exposure to these drugs and chemicals can largely prevent the development of manifest disease. The mechanisms whereby these agents impair the normal regulation of hepatic heme synthesis have been care
