Isolated epiglottic lymphatic malformation in a 2-year-old: Diagnosis and treatment.

Airway obstruction in children has a wide differential diagnosis that includes laryngomalacia, infectious processes, paralysis, extrinsic compression, and other rare anatomic anomalies. Isolated laryngeal lymphatic malformations are rare developments that can manifest with clinically significant airway obstruction. To the best of our knowledge, there have been fewer than 20 reported cases. These laryngeal mucosal lesions are best managed with radiofrequency ablation or laser ablation. We present a case of a 2-year-old child who presented with airway obstruction, initially diagnosed with laryng

Histoplasma epiglottitis in a patient with Crohn's disease maintained on infliximab, prednisone, and azathioprine.

Crohn's disease is associated with treatment and non-treatment infectious complications. Among the treatment-related infectious complications, Histoplasma infection is interesting because of significant overlap between its symptoms and Crohn's disease exacerbation. It is often mistaken as Crohn's disease exacerbation. We present a case of disseminated histoplasmosis presenting as Histoplasma epiglottitis in a patient with Crohn's disease maintained on infliximab, prednisone, and azathioprine.

First presentation of sarcoidosis with severe obstructive sleep apnoea and epiglottic involvement.

Sarcoidosis of the upper respiratory tract (SURT) is rare but well documented. We present a patient whose first presentation with sarcoidosis was with dysphonia, upper airways obstruction and severe obstructive sleep apnoea (OSA) . Systemic steroids and hydroxycholoroquine led to improvement in phonation, endoscopic appearance and upper airway obstruction with significant improvement in Apnoea Hypopnoea Index (AHI) on polysomnography.

Successful use of videolaryngoscopy in an adult patient with acute epiglottitis: a case report.

Acute epiglottitis is a potentially life-threatening infection of the supraglottic structures, which can lead to sudden, fatal airway obstruction. Different techniques have been described to facilitate tracheal intubation in acute epiglottitis. We describe the successful intubation, with the help of the videolaryngoscope, of a 60-year-old female with acute epiglottitis. On admission the patient was ill and severely distressed, sitting in the upright position, drooling saliva, showing severe inspiratory stridor, hyperventilation, but no trismus. Intubation was performed in OR conditions with di…

Acute epiglottitis: a report of the two different methods of airway management in adults.

The authors present two cases of acute epiglottitis with upper airway obstruction that urgently needed artificial airways. In case 1, a 55-year-old woman with DM presented with severe upper airway obstruction and sepsis. The tracheostomy was chosen for airway management, antibiotic was given, and fluid and inotropic drug (dopamine) were used for cardiovascular support. She received continuing care in the intensive care unit for several days. In case 2, a 40-year-old man presented with acute airway obstruction when he breathed forcefully. The prophylaxis tracheal intubation was done by using se…

Epiglottic histoplasmosis presenting in a nonendemic region: a clinical mimic of laryngeal carcinoma.

Histoplasma capsulatum is a dimorphic fungus endemic to North America. Histoplasmosis is primarily an inhalation-acquired mycosis that is encountered rarely outside of endemic regions. In nonendemic regions, histoplasmosis may present a diagnostic challenge and both clinical and laboratory vigilance are required to accurately identify infection. Unusual clinical presentations with limited physical findings may compound the difficulty in diagnosis. We describe a 78-year-old retired soil science professor who presented with an eroded epiglottic mass secondary to disseminated histoplasmosis in a

Experiences in treatment of epiglottal entrapment using a hook knife per nasum.

Thirty eight horses with epiglottal entrapment were examined. In 29 the dorsally displaced mucosa was split axially with a curved hook knife, per nasum. The condition was also treated by subepiglottal mucosal resection (4 horses) and solely by the administration of an anti-inflammatory throat spray (4 horses). All surgical cases were treated with an anti-inflammatory throat spray and parenteral phenylbutazone. Those horses treated via a laryngotomy incision were box rested until the laryngotomy incision healed. The other horses were restricted to walking and trotting exercise for 10 days befor

[The role of the hyoid bone in the act of deglutition after horizontal epiglottic laryngectomy].

Results of ++roentgeno-cinematographic studies of the deglutition act in patients after horizontal supraglottic laryngectomy are presented. The analysis of the material has been expected to provide an answer to what an extent the removal or the preservation of hyoid bone does influence the disorders that involve the swallowing. The entire elaborated material comprises 51 deglutition acts, having been divided into 3 groups. In the I group of studied subjects it was observed that there was a more sizable shortening of the pharynx, as well as a greater upward and forward movement of the remaining

Acute epiglottitis in adults. An eight-year experience in the state of Rhode Island.

We retrospectively reviewed the medical records on all cases of acute epiglottitis in adults from each of the hospitals in Rhode Island and from the state medical examiner's office over an eight-year period (1975-1982). We found 56 cases--an annual incidence of 9.7 cases per million adults. A significant increase occurred in the last two study years. Indirect laryngoscopy proved to be more reliable in making a diagnosis than did x-ray films of the neck; all of 41 indirect examinations were performed without complications. Twenty-three percent (6 of 26) of patients in whom blood cultures had be…

Diode Laser Epiglottidectomy (DLE) for management of epiglottic disease in 35 dogs.

Veterinary literature on epiglottic disorders and their treatment in dogs is limited. The objective of this study is to report the clinical features, diagnosis, surgical treatment, and outcomes of dogs with epiglottic conditions treated using diode laser epiglottidectomy (DLE). This was a single-institution observational prospective study. Thirty-five dogs diagnosed with epiglottic disease underwent DLE. In all cases, an epiglottic disease was documented with combined laryngoscopic examination, when needed, fluoroscopy, computed tomography (CT) scan, and biopsy. Sub-total (SDLE) and total (TDL

An Uncommon Epiglottic Cyst Presentation in an Adolescent: A Case Report and Literature Review.

Epiglottic cysts are benign lesions of the larynx that are relatively rare beyond infancy age. A 17-year-old adolescent male patient presented to the outpatient specialized oropharyngeal clinic with inspiratory stridor, chronic sore throat, and progressive dyspnea symptoms over the past eight months. Examination by a headlight and a tongue depressor showed a large cystic lesion arising from the hypopharynx. A neck computed tomography (CT) scan revealed a 4 cm oval cyst attached to the lingual epiglottic surface. The relatively large epiglottic cyst was drained directly in the clinic and was la

Types of Epiglottic Collapse in Breathing Sleep Disorder and Their Impact in Clinical Practice.

Introduction  Obstructive sleep apnea (OSA) is a severe form of sleep-disordered breathing (SDB) that is strongly correlated with comorbidities, in which epiglottic collapse (EC) and other contributing factors are involved. Objectives  To evaluate the occurrence of EC in OSA patients through drug-induced sleep endoscopy (DISE) and to determine the factors contributing to EC. Methods  A retrospective study of 37 adult patients using medical history. Patients were assessed for laryngopharyngeal reflux (LPR) and lingual tonsil hypertrophy (LTH) using reflux symptom index and reflux finding score

Subtotal Laryngectomy with Epiglottic Reconstruction for Glottic Carcinoma: A Single Institutional Experience.

AIM: Laryngeal cancer is a common form of head and neck cancer in Vietnam where the current treatment is surgery. Subtotal laryngectomy with epiglottic reconstruction, a conservative surgery, allows removal of anterior commissure including thyroid cartilage and paraglottic space and provides a maximum restoration of the anatomical structure of the larynx. PURPOSE: To evaluate the results, the safety and effectiveness of patients who were treated with subtotal laryngectomy with epiglottic reconstruction. MATERIAL AND METHOD: From January 2012 to July 2017, 42 patients (41 male, 1 female, medi

Paraglottic and Pre-epiglottic Space Abscess in Adults: Report of Two Cases.

INTRODUCTION: Acute epiglottitis or supraglottitis is a rapidly progressing upper respiratory tract infection that can often threaten the airway patency. Epiglottic abscess that expands to the paraglottic (PGS) or preepiglottic (PES) space and acute airway obstruction constitute rare complications, exclusively presented in adults. CASE REPORT: We report two cases. In the first case flexible fiberoptic Rhino-Pharyngo-Laryngoscopy showed epiglottitis that was obstructing the airway and abscesses on the lingual surface of the epiglottis. Abscesses were opened using laser CO2. In the following da…

Point-of-care ultrasound for prompt diagnosis and treatment monitoring of acute epiglottitis.

A 72-year-old woman presented with a 1-day history of acute throat pain, hoarseness. On cervical ultrasonography (transverse view), the epiglottis was swollen with anterior-posterior diameter of 3.5 and 3.8 mm at right and left edges, respectively. Laryngeal endoscopy showed marked swelling of the epiglottis, and acute epiglottitis was diagnosed.

A systematic review and meta-analysis of predictors of airway intervention in adult epiglottitis.

OBJECTIVE: Epiglottitis is typically considered a pediatric disease; however, there is growing evidence that the incidence of adult epiglottitis has changed since the introduction of the Haemophilus influenzae vaccine. The literature is composed of multiple small series with differing findings. To date, there has been no attempt to collaborate evidence on predictors of airway intervention in this disease. METHODS: The population of interest was adults with a diagnosis of epiglottitis. The primary outcome in this review was incidence of airway intervention. A comprehensive literature search wa…
