OBJECTIVES: To evaluate the efficacy of general anesthesia during epileptic surgery.
MATERIAL AND METHODS: A retrospective study of 64 patients who received general anesthesia during epileptic surgery. In the preoperative period, anticonvulsive medication was adjusted in accordance with plasma levels and withdrawn entirely 8 hours before surgery. After premedication with droperidol and fentanyl, a balanced anesthetic technique was applied, based on pentothal, pancuronium (or vecuronium), fentanyl, N2O and isoflurane. Continuous monitoring of ECG, arterial blood pressure, pulse oximetry, ET CO

Reference is made to the recommendation of the International League Against Epilepsy concerning driving-permission of epileptics, publicated 1981; only minor attention was payed to the german translation, published in 1984. Different comments on this recommendation as well as our own experiences are quoted. We refer to the rarity of traffic-crashes due to epileptic seizures, although we assume that approximately half of the seizure-patients drive, partly under concealing their disturbance. We present 51 collected own cases of traffic-accidents due to sudden attacks of unconsciousness. Only 7 o…

Epilepsy is the commonest manifestation of neurocysticercosis. Epilepsy was observed in 127 of 150 cases (84.7%) of neurocysticercosis seen over a period of 17 years. The basis of diagnosis was clinical presentation and concomitant evidence of extraneural cysticercosis in the pre-computed tomography (CT) scan era, and typical CT findings in later years. Eighty one cases (54%) who primarily presented as epilepsy without any neurological deficit have been analysed in detail. In the pre-CT period the occurrence of epilepsy as a presenting feature in neurocysticercosis was 43.5% whereas in later y…

In a large family with maternally inherited mitochondrial disease, a mild defect in the NADH-ubiquinone oxidoreductase step (complex 1) in the respiratory chain was found. Epilepsy was seen in nine (22%) of the 37 family members. Five of them, belonging to one branch of the family, had myoclonus epilepsy and EEG abnormalities consistent with this. The remaining four patients, belonging to other branches of the family tree, had partial epilepsy. Neurological symptoms also varied in different parts of the family. Possible explanations for the differences in phenotypic expressions are discussed.

The clinical differentiation of epileptic seizures from hysterical pseudo-epileptic seizures is sometimes difficult. The routine inter-ictal … ient post-ictal elevations in serum prolactin as a diagnostic test in aiding the differentiation of epilepsy from hysterical pseudo-epilepticseizures, In the past decade many reports have documented post-ictal elevations in serum prolactin after tonic-clonic and complex partial seizures. Peak prolactin levels were observed 15 to 20 minutes after the ictus …

Sixty-six patients with epilepsy and depression were studied. Thirty-four had a family history of psychiatric illness; depression was the most common condition. Assessed using standardised rating scales, the severity of the depression was moderate and was endogenous in approximately 40% of patients. Attendant features were high state and trait anxiety and hostility. The EEGs of the patients and a control group were not significantly different. Patients receiving phenobarbital (PB) were more depressed, whereas those taking carbamazepine (CBZ) were both less depressed and less anxious. The pheno…

The EEG is useful in evaluating seizure disorders by establishing or confirming the diagnosis of a seizure disorder, determining the type and focus of origin of the seizure, and helping make the distinction between an epileptic attack and a nonepileptic condition. The EEG also may show other abnormalities, such as focal slowing, that give a clue about the underlying disease process. A negative EEG does not exclude the diagnosis of epilepsy. Activating procedures such as hyperventilation, photic stimulation, and recording during sleep may help bring out epileptiform activity. Occasionally, repe…

Background: Brain regions positively correlated with the epileptogenic zone in patients with temporal lobe epilepsy vary in spread across the brain and in the degree of correlation to the temporal lobes, thalamus, and limbic structures, and these parameters have been associated with pre-operative cognitive impairment and seizure freedom after epilepsy surgery, but negatively correlated regions have not been as well studied. We hypothesize that connectivity within a negatively correlated epilepsy network may predict which patients with temporal lobe epilepsy will respond best to surgery. Method…
