[Comparative analysis of modern methods of preparing donor material for endothelial keratoplasty].

PURPOSE: To evaluate various methods of stripping Descemet's membrane of a donor corneoscleral flap for Descemet's membrane endothelial keratoplasty (DMEK). MATERIAL AND METHODS: The study included 60 corneoscleral flaps (mean donor age was 62.9 years) from the eye bank of Research Institute of Eye Diseases, which were not suitable for clinical use due to positive serological results. Four alternative methods of donor material dissection in preparation for DMEK were compared: SCUBA (group A), our newly suggested method utilizing intracapsular ring (group B), 'liquid bubble' (group C), 'big bu

Increased clusterin expression in Fuchs' endothelial dystrophy.

PURPOSE: To investigate the differential expression of the glycoprotein clusterin/apoJ (CLU) in normal and Fuchs' endothelial dystrophy (FED) corneal endothelium and to compare the expression of various forms of CLU in normal and FED tissue. METHODS: FED and pseudophakic bullous keratopathy (PBK) corneal buttons were removed during transplantation, and normal corneas were obtained from tissue banks. Human corneal endothelial cells and Descemet's membrane (HCEC-DM) complex was dissected from the stroma. Proteins were separated on 2-D gels and subjected to comparative proteomic analysis. Relati

Transplantation of ocular tissue from a donor with sporadic Creutzfeldt-Jakob disease.

BACKGROUND: One definite, one probable and several possible transmissions of sporadic Creutzfeldt-Jakob disease (sCJD) have followed corneal transplantation. We report an incident in the UK in 1997 in which both corneas and scleras from a donor, subsequently confirmed to have had sCJD, were transplanted. The final clinical outcome for two surviving recipients is still not yet known. CASE REPORT: In 1997, a 56-year-old woman died from biopsy-proven carcinoma of the bronchus. Both eyes were donated for transplantation. Shortly before she died, she had developed neurological symptoms thought to

Easy-to-prepare autologous platelet-rich plasma in the treatment of refractory corneal ulcers.

As platelets are rich in growth factors for tissue regeneration, autologous platelet-rich plasma (PRP) has been used to treat some refractory corneal defects. Although PRP is effective, the cost of its preparation is very high. This article presents three cases of refractory corneal ulcer under the prescription of autologous PRP. The autologous PRP used in these cases was easily prepared in the blood bank laboratory. In this paper, we collected three patients with refractory corneal ulcer who were unresponsive to conventional treatment. The patients presented with neurotrophic ulcer, exposure

A portable microkeratome-based anterior corneal surface harvesting device.

PURPOSE: To determine the reproducibility of anterior sclerokeratectomy using a portable nonelectric microkeratome-based device capable of harvesting the entire anterior corneal surface for lamellar transplantation. METHODS: A modified gas turbine-driven microkeratome (LSK One, Moria/Microtech, Doylestown, PA) with a redesigned head large enough to incorporate the whole human anterior corneal surface in a pass and was coupled to a manual vacuum pump. This instrument was tested on 25 fresh porcine globes divided into 2 groups (170-microm and 200-microm head). To assess cut reproducibility the

Craniofacial neurosurgical approach for extensive hyperostotic meningioma.

We present a patient with a craniofacial meningioma that had been considered inoperable for 8 years but has now been successfully treated using the combined neurosurgical and craniomaxillofacial approach. The tumor extensively involved the meninges, skull, face, left orbit, and maxilla. Vision and mild proptosis of the left eye had worsened over the last several years. Computed tomography and magnetic resonance imaging scans revealed an extensive intraosseous meningioma with intracranial and extracranial extension. An angiogram revealed an intensely vascularized mass supplied by a markedly enl

Freeze-fracture label of functional and dysfunctional human corneal endothelium.

Previous freeze-fracture results from our laboratory have shown a reduction in a population of intramembrane particles in the lateral endothelial membranes from dysfunctional human corneas. The size range of these intramembrane particles corresponds to that which has previously been reported for the glycoprotein enzyme Na, K-ATPase in enzyme enriched freeze-fractured membranes. In order to investigate glycoconjugate changes potentially related to the particle reduction, wheat germ agglutinin (WGA), which has been shown to bind to the sugar residues in the ATPase subunit, was used to label thre

Incidence of presumed iatrogenic graft failure in Descemet stripping automated endothelial keratoplasty.

PURPOSE: To evaluate presumed iatrogenic graft failure (PIGF) in Descemet stripping automated endothelial keratoplasty (DSAEK). METHODS: Deidentified data were collected retrospectively from the Illinois Eye Bank between April 2007 and May 2010. PIGF was defined as cases in which a repeat corneal transplant was performed <8 weeks after an initial DSAEK. Data collected for each case included days between initial DSAEK and regraft, regraft type (DSAEK vs. penetrating keratoplasty), precut versus surgeon-cut tissue, and number of DSAEKs distributed to individual surgeons. Ninety-three cases of P

Preloaded Descemet membrane endothelial keratoplasty: A systematic and descriptive review of global experimental and clinical outcomes.

Preloaded Descemet membrane endothelial keratoplasty (plDMEK), in which eye-bank validated donor tissues are prepared, folded, and loaded into delivery devices prior to shipment, has emerged as a promising strategy to standardize and expand access to endothelial keratoplasty. Protocols developed with both endothelium-in (endo-in) and endothelium-out (endo-out) loading configurations have shown to improve surgical efficiency, reproducibility, and tissue utilization while lowering procedural barriers. We compiled current laboratory and clinical evidence on plDMEK and compared endo-in versus endo

Epigenetic Modulation Directs Recovery Post LASIK and SMILE Surgery: An Experimental Study.

PURPOSE: refractive surgery, such as LASIK and SMILE, induces a wound healing response that leads to significant corneal stromal remodeling. We have shown that the protein profile in the stroma changes dramatically immediately post-surgery. However, the methylation status of the DNA post-refractive surgery remains unknown. DESIGN/PARTICIPANTS: DNA methylation study. Refractive surgery (SMILE/LASIK) performed on donor eye globes. METHOD: we investigated the epigenetic changes post-surgery in relation to long term ECM remodeling in an experimental ex vivo study design. Donor globes (n = 19) we

Penetrating keratoplasty and Verisyse iris-claw lens--is it safe for corneal graft?

The loss of the corneal endothelial cells, especially in a first postoperative year, has been observed in patients who underwent penetrating keratoplasty (PK). The implantation of new generation of "iris claw" phakic IOL (Verisyse) in refractive cases has been shown to cause clinicaly insignificant endothelial cell loss. In our prospective case series we investigated the endothelial cell loss and clinical outcome in patients that either underwent PK and implantation of PCIOL or PK and implantation of Verisyse IOL. In the first group of 9 patients scheduled for PK, implantation of Verisyse was

Combined use of bulbar conjunctival pedicle flap and labial mucous membrane graft for porous orbital implant exposure: Long-term outcome.

Implant exposure is the most common serious complication of porous orbital implants, and often requires surgical repair. This study aims to describe a new repair technique using a bulbar conjunctival pedicle flap and a labial mucous membrane patch graft, as well as to report its long-term results. A retrospective chart review was performed on all patients whose porous orbital implant exposures were repaired using this technique from 1995 to 2014. Twenty-three patients were included. The maximal defect dimension ranged from 2 to 18 mm. Sixteen patients (70%) also received a banked human scleral

Prodromal Clinical Markers of Parkinson disease in Gaucher Disease Individuals.

Heterozygous mutations in the glucocerebrosidase (GBA) gene have been reported as a common risk factor for the development of Parkinson's disease (PD) in Gaucher disease (GD) patients and in heterozygous GBA mutation positive carriers. In this study, we analyzed the occurrence of prodromal markers of PD in an Argentinean cohort with type 1 GD. After signed informed consent, we evaluated 26 patients with type 1 GD under enzymatic replacement therapy from a cohort of the Hospital Ricardo Gutierrez GD Study Group in Buenos Aires City, Argentina. We performed an extensive neurological examination,

Gene therapy for corneal dystrophies and disease, where are we?

PURPOSE OF REVIEW: We assess the studies on vector systems for delivery of transgenes to the cornea that have been published over the last year and summarize new work on the identification of specific transgenes for corneal diseases. RECENT FINDINGS: Adeno-associated viral vectors are increasingly being successfully applied to the cornea, although transgene expression requires corneal epithelial debridement or intrastromal injection of the vector. Gene delivery platforms based on nanoparticles of chitosan or gold also show promise. Overexpression of vasoinhibin-1 or decorin, or siRNA-mediated
