BACKGROUND: Retinoblastomas account for 4% of malignancies in children, 1-2% of which are diffuse infiltrating retinoblastomas. BACKGROUND: Sympathetic ophthalmia is a rare form of autoimmune uveitis and manifests in 90% of cases within the first year after penetrating injuries or surgical interventions.

A new method is proposed for evisceroenucleation with formation of a locomotive stump permitting to remove complications connected with anophthalmos. In experiment, morphologic investigation of changes in the implant (allocartilage) in dynamics from 20 days to 1 year have shown that the implant preserves its form and initial fixation for the whole period of observation. This provides good prosthetic conditions. Ultrasonic scanning is recommended as an effective method to control the implant adaptation and locomotive stump formation.

Eyelid and orbital deformities caused by various defects, such as tumor resection and trauma, whether congenital or acquired, are extremely complex and atypical. The case of a 77-year-old woman with trauma after a fall is reported. On clinical examination, an avulsion injury was found in the left eye with dual optic nerve dissection. An INR of 3.3 was determined in a perioperative blood sample. The given conditions forced the rare but urgent indication for primary enucleation with hemostasis. Oral anticoagulation presented perioperative high risk, especially due to the potential risk of bleeding from the ophthalmic artery.

Histopathologic examination of a blind eye from a 12-year-old black girl revealed an unsuspected adenocarcinoma of the nonpigmented ciliary epithelium and evidence of previous penetrating trauma. The association between adenocarcinoma of the ciliary epithelium and ocular trauma has been well documented in elderly patients but not in children. The histopathologic distinction between a pleomorphic adenocarcinoma of the ciliary epithelium and a uveal melanoma may be difficult. Signs of previous penetrating trauma should heighten suspicion of an adenocarcinoma, even in young children.

A study of 11 consecutive patients with anterior uveal melanomas from one institution demonstrated the wide variation in the clinical and histopathologic manifestations of the tumors. The intraocular pressure in the involved eye compared with the fellow eye was elevated in five cases, equal in four, and reduced in two. Fifty-two unilaterally enucleated patients were studied for symptoms, signs, and actual presence of bacteria in the socket and fellow eye. The majority (85%) of asymptomatic sockets were found to have a mixed culture of pathogenic organisms. Sockets had a significantly (P = .001) greater frequency of pathogenic organisms than the fellow eye. Cross contamination does not appear frequently but is a definite possibility. We recommend culture and treatment of the asymptomatic socket before surgery on the remaining eye.

Penetrating eye injury is an important cause of ocular morbidity in India, especially in the pediatric and adolescent age group. Sometimes these injuries are associated with retained intraorbital foreign body. We report herein for the first time a child who had an open globe injury with a retained unusually large intraorbital foreign body (iron nut).
