From injury to reconstruction, utilizing staged technique in a self-inflicted facial gunshot wound: a case report.

BACKGROUND: Facial gunshot injuries are rare and complex, with mortality and complication rates of up to 15% and 30%, respectively. Management of facial gunshot wounds has proven to be challenging for surgeons because the treatment requires multidisciplinary evaluation and planning. We present a rare case of a gunshot wound to the face that resulted in extensive deformities, requiring multiple reconstructive procedures. CASE PRESENTATION: A 19-year-old male of South Asian (Pakistani) origin presented to the emergency department with a 3-day-old gunshot wound to the floor of the mouth, causing

Successful Surgical Management of Intracranial Carotid Artery Trauma Following Penetrating Facial Injury: A Case Report.

BACKGROUND Carotid artery injury has an incidence of 0.2% in the National Trauma Data Bank. The true incidence of intracranial carotid injury is unknown but can be estimated at less than one in 1000 trauma-related inpatient admissions in America. Operatively managed penetrating carotid trauma has a mortality rate approaching 20%, and the selection of the appropriate operative approach is not straightforward. We present a case of penetrating carotid trauma successfully managed via combined approach by neurosurgery and otolaryngology teams. CASE REPORT A 74-year-old woman fell into a honeysuckle

Localized application of SAG21k-loaded fibrin hydrogels for targeted modulation of the hedgehog pathway in facial nerve injury.

Given the broad biological effects of the Hedgehog (Hh) pathway, there is potential clinical value in local application of Hh pathway modulators to restrict pathway activation of target tissues and avoid systemic pathway activation. One option to limit Hh pathway activation is using fibrin hydrogels to deliver pathway modulators directly to tissues of interest, bypassing systemic distribution of the drug. In this study, we loaded the potent Hh pathway agonist, SAG21k, into fibrin hydrogels. We describe the binding between fibrin and SAG21k and achieve sustained release of the drug in vitro. SA

Effect of Metformin on the Functional and Electrophysiological Recovery of Crush Injury-Induced Facial Nerve Paralysis in Diabetic Rats.

The impact of metformin on the rat facial nerve following crush injury has only occasionally been documented to date. The purpose of the current investigation was to use functional and electrophysiological evaluations to investigate the effects of metformin administration on recovery following crush injury to the rat facial nerve. The rats were randomly divided into four groups: the nonDM/PBS group (n = 4), the nonDM/metformin group (n = 4), the DM/PBS group (n = 4), and the DM/metformin group (n = 4). Diabetes was generated by an intraperitoneal injection of streptozotocin. Facial nerve paral

Brief Electrical Stimulation Improves Functional Recovery After Femoral But Not After Facial Nerve Injury in Rats.

Brief low-frequency electrical stimulation (ES, 1 h, 20 Hz) of the proximal nerve stump has emerged as a potential adjunct treatment for nerve injury. Despite available experimental and clinical data, the potentials and limitations of the ES therapy still have to be defined using different animal models, types of nerves, and clinical settings. Here, we show that brief ES of the proximal stump of the transected rat femoral nerve causes, as estimated by motion analysis, enhanced functional recovery reaching preoperative levels within 5 months of injury, in contrast to the incomplete restoration

Brief electrical stimulation and synkinesis after facial nerve crush injury: a randomized prospective animal study.

BACKGROUND: Recent studies have examined the effects of brief electrical stimulation (BES) on nerve regeneration, with some suggesting that BES accelerates facial nerve recovery. However, the facial nerve outcome measurement in these studies has not been precise or accurate. Furthermore, no previous studies have been able to demonstrate the effect of BES on synkinesis. The objective of this study is to examine the effect of brief electrical stimulation (BES) on facial nerve function and synkinesis in a rat model. METHODS: Four groups of six rats underwent a facial nerve injury procedure. Grou

Nimodipine improves vocal fold and facial motion recovery after injury: A systematic review and meta-analysis.

INTRODUCTION: Nimodipine is a calcium channel blocker that has been used to treat hypertension and vasospasm. Emerging evidence in the literature suggests that it is neuroprotective by reducing cellular apoptosis after neuronal injury and promoting axonal sprouting at the nodes of Ranvier. OBJECTIVES: To conduct a systematic review of the usage of nimodipine in cranial nerve injury and to perform a meta-analysis to estimate the efficacy of nimodipine on functional recovery of the injured cranial nerves. METHODS: Literature search was performed in eight databases using preferred reporting ite

The injury of marginal mandibular branch unexpectedly promotes the repair of buccal branch of facial nerve in a rat model.

CONCLUSIONS: The results indicate that the injury of the marginal mandibular branch improved the recovery of the buccal branch in a rat model. OBJECTIVE: The aim of this study was to investigate whether the injury or intactness of the marginal mandibular branch affects the regeneration of the facial nerve buccal branch in a rat model. METHODS: This experiment was conducted on 30 adult rats, which were randomly and equally divided into two groups. The buccal branch of the facial nerve was transected and reconstructed, with the marginal mandibular branch damaged (group A) or intact (group B).

Trigeminal Inflammatory Compression (TIC) injury induces chronic facial pain and susceptibility to anxiety-related behaviors.

Our laboratory previously developed a novel neuropathic and inflammatory facial pain model for mice referred to as the Trigeminal Inflammatory Compression (TIC) model. Rather than inducing whole nerve ischemia and neuronal loss, this injury induces only slight peripheral nerve demyelination triggering long-term mechanical allodynia and cold hypersensitivity on the ipsilateral whisker pad. The aim of the present study is to further characterize the phenotype of the TIC injury model using specific behavioral assays (i.e. light-dark box, open field exploratory activity, and elevated plus maze) to

Accuracy of high-resolution computed tomography in locating facial nerve injury sites in temporal bone trauma.

In this study, high-resolution, multislice computed tomography findings are compared with surgical findings in terms of the fracture location in patients with traumatic facial paralysis. Patients with traumatic facial paralysis with grade VI House-Brackmann scale who met the criteria for surgical decompression between 2008 and 2012 were included in this study. All the patients underwent a multislice high-resolution, multislice computed tomography (HRCT) using 1-mm-thick slices with a bone window algorithm. The anatomical areas of the temporal bone (including the Fallopian canal) were assessed

Subthreshold continuous electrical stimulation facilitates functional recovery of facial nerve after crush injury in rabbit.

We sought to determine whether electrical stimulation (ES) with subthreshold, continuous, low-frequency impulses is a viable clinical method for improving functional recovery after facial nerve crush injury. In 10 rabbits, bilateral crush injuries were made on the facial nerve by compression for 30 s with mosquito forceps, causing complete facial paralysis. Subthreshold continuous direct current ES with 20-Hz square-wave pulses was applied to the proximal stump on one side for 4 weeks. Vibrissae movement returned significantly earlier on the ES side, with a less variable recovery time. Electro

[Selective injury of the vestibular and facial nerves following microvascular decompression procedures for hemifacial spasm: A case report].

Microvascular decompression procedures were carried out to relieve right hemifacial spasm in a 54-year-old man. His facial spasm was satisfactorily cured but he complained of severe vertigo, and slight facial palsy was also shown after the operation. Electronystagmograph disclosed spontaneous horizontal and rotatory nystagmus toward the non-operated (left) side. The postoperative caloric test showed profound dysfunction on the operated side. His hearing was well maintained postoperatively. These findings indicated, in addition to facial nerve injury, that the peripheral portion of the vestibul

Estimating the National Burden of Potentially Avoidable Interfacility Transfer Among Patients with Isolated Facial Injury in the US.

BACKGROUND: Interfacility transfers for facial trauma are common but often avoidable. However, the magnitude of these potentially avoidable interfacility transfers (PAITs), which can strain trauma systems and incur avoidable costs to the patients, remains unknown. This study aimed to quantify the burden of PAITs for isolated facial trauma in the US. STUDY DESIGN: A retrospective analysis using the 2022 Nationwide Emergency Department Sample was performed. Patients aged 16 years and older with isolated facial injuries were identified using ICD-10-CM codes and the Abbreviated Injury Scale. A mu…

Extracapsular dissection versus superficial parotidectomy: real-world comparison of postoperative complications and facial nerve injury.

BACKGROUND: Superficial parotidectomy (SP), involving routine facial nerve (FN) dissection, is the standard approach for benign superficial parotid tumors. Extracapsular dissection (ECD) offers a less invasive alternative by avoiding formal FN identification, but large-scale comparative data remain limited. OBJECTIVE: To compare postoperative complication rates between ECD and SP. METHODS: This retrospective study used the TriNetX database to identify adults with benign superficial parotid tumors who underwent ECD or SP. Propensity score matching (PSM) was performed to balance baseline covar

The Efficacy of Thermal Mechanical Fractional Injury System for Facial Rejuvenation: Subjective, Objective and Image Analysis Study.

BACKGROUND: Facial aging is the result of a number of factors, including the resorption of skeletal tissue, the atrophy of fat pads, and the laxity of connective tissue. These factors contribute to a reduction in facial volume and the development of skin changes. Thermal mechanical fractional injury (TMFI) represents a minimally invasive solution, stimulating collagen production and enhancing the delivery of topical formulations via epidermal microcraters. AIM: This study seeks to evaluate the effects of TMFI on skin quality and introduce quantifiable methods to assess improvements. PATIENTS
