[Fetal uropathies and pulmonary hypoplasia].

The author discusses the role of fetal uropathies in the development of pulmonary hypoplasia. The predisposing abnormalities may occur at various locations, including the kidneys and the efferent urinary ducts (ureters, urocyst, urethra). Following a brief general overview of the normal development of the urinary system, the developmental abnormalities are discussed. These anomalies interfere with the secretion of urine or, alternatively, may hinder or obstruct its elimination through the urinary ducts, thus leading to oligohydramnios. Inadequate production of amniotic fluid hinders the respir

[Spectral analysis of fetal heart rate variability].

OBJECTIVE: To assess power spectral densities and power spectrum of FHR variability in two categories of fetal activity, quiescence and breathing movements, in pregnancies complicated by diabetes mellitus or intrauterine growth retardation. MATERIAL AND METHODS: The study group included 70 women, 35 in uncomplicated pregnancies and 35 with above-mentioned complications. The spectral analysis of FHR variability in fetal quiescence or breathing activity was performed. The power spectrum was analyzed at frequencies 0-1.0 Hz. Successively, the power spectral density was calculated. It has been pr

Prenatal diagnosis of fetal left transverse venous sinus thrombosis with intracranial hemorrhage: imaging findings and short-term outcome.

This report presents the prenatal imaging findings and short-term clinical outcome of a rare case of fetal cerebral sinovenous thrombosis, along with a brief review of the literature. A 31-year-old pregnant Afghan woman at 34 weeks of gestation (gravida 3, para 0, abortion 2) presented to the hospital with complaints of decreased fetal movement. Fetal cranial ultrasonography revealed a thick-walled, avascular, hypoechoic, mass-like lesion (17×18×18 mm) extending from the left foramen of Monro to the superior aspect of the thalamus and displacing the body of the left lateral ventricle. Fetal cr…

Overrepresentation of multiple birth pregnancies in young infants with four metabolic bone disorders: further evidence that fetal bone loading is a critical determinant of fetal and young infant bone strength.

UNLABELLED: The frequency of multiple birth pregnancies, mostly twin pregnancies, was overrepresented in four different groups of young infants with fractures and bone abnormalities. This finding suggests that fetal bone loading through fetal movement is an important determinant of fetal bone formation and its resultant bone strength. INTRODUCTION: It has been suggested that intrauterine confinement related to the multiple birth pregnancy (MBP) may lead to an increased risk for fragility fractures in young infants as a result of decreased fetal bone loading. METHODS: To objectively test this…

Candida chorioamnionitis associated with late stillbirth: A case report.

Vulvovaginal candidiasis during pregnancy is common, but serious complications, including chorioamnionitis, are infrequent. A 41-year-old woman presented at 37 weeks of gestation with reduced fetal movements, and fetal death in utero was subsequently confirmed on ultrasound. Histopathology of the cord and placenta revealed Candida infection and microabscesses on the umbilical cord. Overall, these features are suggestive of ascending infection, consistent with Candida as the causative organism. To the best of our knowledge, this is the first reported case of late stillbirth due to Candida chori…

Fetal akinesia deformation sequence presenting with increased nuchal translucency in the first trimester of pregnancy.

Prenatal sonographic diagnosis of fetal akinesia deformation sequence (FADS) is usually achieved during the second and third trimester of pregnancy and relies on the demonstration of contractures of multiple joints, paucity of fetal movements and nuchal edema or generalized hydrops. We present the early diagnosis of FADS in a 13-week fetus presenting with nuchal translucency thickness (NT) of 5.1 mm, bilateral talipes and fixed flexion deformity of the fingers and wrists. We also review cases of this condition diagnosed during the first trimester and discuss the role of increased NT in the ear…

Spontaneous abortion.

As a result of improved investigation and treatment techniques, understanding of all aspects of spontaneous abortion has improved. The term "threatened abortion" is often a misnomer, for the fate of the pregnancy is decided when bleeding occurs. A dilated cervix indicates that abortion is inevitable. If the cervix is not dilated, pelvic ultrasonography will usually show the following: a live fetus, an anembryonic pregnancy, incomplete abortion or missed abortion. If fetal heart movement is seen, no further treatment is needed and the prognosis is excellent (98% fetal survival). In other cases,

Expanded Möbius syndrome.

A woman presented at 33 weeks gestation with reduced fetal movements and a nonreactive nonstress test. Fetal ultrasound examination revealed a peculiar unilateral arm tremor. At emergency cesarean section, performed for fetal indications, a 1,672-gm male infant was delivered requiring intubation for feeble respiratory effort. After delivery the neonate was transiently hypertonic and later hypotonic. Continuing ventilatory support at minimal settings was necessary. The work-up for aneuploidy, metabolic disorders, and infection was negative. The infant died after being removed from ventilatory s…

Pheochromocytoma in a pregnant patient.

Pheochromocytoma accounts for only 0.1% of hypertension found in adults between 40 and 70 years of age. Although it is extremely rare in pregnancy, if it occurs and is unrecognized in pregnant women, pheochromocytoma can have catastrophic effects. For instance, maternal fatal hypertension can be precipitated by anesthesia, vaginal delivery, uterine contractions, or even vigorous fetal movements. Fetal growth retardation is often seen secondary to decreases in uteroplacental perfusion. Fetal hypoxia or death can also occur with maternal episodes of headache, palpitations, and diaphoresis relate…

Fetal monitoring using a wearable ultrasound patch for high-risk pregnancies.

The rapid and complex nature of fetal development requires meticulous prenatal monitoring to ensure optimal pregnancy outcomes1. Cardiotocography, which continuously records the fetal heart rate and uterine contractions, often leads to inaccurate diagnoses and unnecessary interventions2. Ultrasonography is a cornerstone of fetal monitoring and diagnosis, but it is highly dependent on specialized sonographers, limiting its availability3. Additionally, current ultrasound methods provide only snapshot evaluations4. Even in very high-risk pregnancies, it is rare to have fetal ultrasound assessment

Oxygen consumption, acid-base status, and behavior during and after acute, severe hemorrhage in fetal lambs.

The metabolic and behavioral effects of 40-45% hemorrhage (at approximately 1%/min) were studied in nine fetal lambs in utero (130-135 days of gestation) until 2 days posthemorrhage. Umbilical blood flow (Qum) fell from 192 +/- 14 to 100 +/- 9 ml.min-1.kg-1 at the end of blood loss, and on the day of hemorrhage it was linearly related to blood volume. However, on the posthemorrhage days, Qum was restored even though blood volume was still reduced. Fetal O2 delivery (DO2) fell with the decrease in Qum and later due to anemia (decrease in hematocrit from 33.4 +/- 1.2 to 23.6 +/- 1.2%), from 946

The efficacy of rapid biophysical profile in predicting poor pregnancy outcomes in suspected intrauterine growth restriction fetuses: preliminary study.

OBJECTIVE: To determine the efficacy of the rapid biophysical profile (rBPP), which includes sound-provoked fetal movement detected by ultrasonography, and amniotic fluid index for antepartum fetal well-being assessment in suspected intrauterine growth restriction (IUGR) fetuses. MATERIAL AND METHOD: One hundred seven tests of rBPP were antenatally performed in 30 IUGR fetuses who attended the ANC unit at Maharaj Nakorn Chiang Mai Hospital between October 2009 and July 2010. Pregnancy outcomes were prospectively recorded for subsequent analysis. The accuracy of rBPP was based on poor outcomes…

Fetally-injected drugs for immobilization and analgesia do not modify fetal brain development in a rabbit model.

OBJECTIVE: During fetal surgery, fetuses receive medication (atropine-fentanyl-curare) to prevent fetal pain, movement and bradycardia. Although essential there has been no detailed review of potential side effects. Herein we aimed to assess the effects of this medication cocktail on fetal brain development in a rabbit model. METHODS: Pregnant does underwent laparotomy at 28 days of gestation. Two pups of each horn were randomized to an ultrasound guided injection with medication (atropine-cisatracurium-fentanyl, as clinically used) or saline (sham). The third pup was used as control. At term

[A Case of Congenital Gastroschisis Repair of a Newborn Following the Cesarean Section].

A 21-year-old woman was admitted at 35 +4 weeks due to disappearance of fetal movement for one day. The ultrasound from other hospital indicated fetal gastroschisis. Abdominal ultrasound in our hospital confirmed that the discontinuity of the fetal abdominal wall by 4.5 cm. The stomach and part of the intestine were herniating into the amniotic cavity. Considering the possibility of gastroschisis and fetal distress, electronic fetal monitoring was implemented continuously and consultations of relevant departments were called immediately, including anesthesiology, neonatology and general pediat…

Magnetocardiographic rhythm patterns at initiation and termination of fetal supraventricular tachycardia.

BACKGROUND: Using fetal magnetocardiography (fMCG), we characterize for the first time the electrophysiological patterns of initiation and termination of reentrant fetal supraventricular tachycardia (SVT), the most common form of life-threatening fetal arrhythmia. METHODS AND RESULTS: In contrast to the expectation that reentrant SVT is initiated by spontaneous premature atrial contractions (PACs) and is terminated by spontaneous block, 5 distinct patterns of initiation and 4 patterns of termination were documented, with the most common patterns of initiation involving reentrant PACs. Wavefor

Three cases of massive fetomaternal hemorrhage presenting without clinical suspicion.

Fetomaternal hemorrhage (FMH) is a common obstetrical occurrence most often associated with small volumes of blood transferred across the placenta. Fetomaternal hemorrhage leads to alloimmunization of Rh D-negative mothers, resulting in an increased risk of hemolytic disease of the newborn. Massive FMH involving volumes of blood greater than 30 mL can cause substantial fetal morbidity and mortality. Massive FMH may present with signs and symptoms such as decreased movement, sinusoidal heart rhythms, or fetal anomalies. We present 3 cases of clinically unexpected massive FMH of 206, 88, and 155

Exploration of 3D fetal cardiac MRI in the late second trimester.

BACKGROUND: Fetal cardiovascular magnetic resonance imaging (CMR) is restricted to late gestation due to fetal movement and vessel size while initial clinical assessment of the fetal heart is conducted mid-trimester. This study aimed to assess feasibility and diagnostic potential of automated motion-correction tools for 3D fetal CMR of late second trimester fetuses. METHODS: Black-blood fetal CMR datasets of controls and fetuses with congenital heart disease (CHD) (20 - 27+6 weeks' gestation) were collected and reconstructed to 3D volumes. Image quality, nineteen cardiovascular structures and…

Artificial Intelligence-Assisted Fetal Ultrasound in Low-Resource Settings: Opportunities, Challenges, and Future Directions.

Artificial intelligence (AI) has emerged as a promising strategy to improve access to fetal ultrasound in low-resource settings, where shortages of trained personnel, limited infrastructure, and unequal access to diagnostic imaging continue to compromise maternal and fetal healthcare. This narrative review synthesizes current evidence on AI-assisted fetal ultrasound, focusing on its clinical applications, implementation experience, challenges, and future directions. The reviewed evidence demonstrates that AI can support multiple stages of the fetal ultrasound pathway, including gestational age
