Morphology of the proximal tibiofibular joint does not affect the type of lateral malleolus fracture and syndesmotic ligament injuries: A computed tomography-based study.

INTRODUCTION: The Proximal Tibiofibular Joint (PTFJ) is a synovial joint with anatomical variations located between the anteromedial surface of the fibula and the posterolateral facet of the tibia. These anatomical variations are believed to contribute to different biomechanical behaviors in both the knee and ankle joint and play a role in both physiological and pathological movements. Therefore, the morphology of the proximal tibiofibular joint may play a decisive role in the characteristics of ankle fractures and syndesmotic ligament injuries. MATERIALS AND METHODS: In this retrospective co

Management of ipsilateral distal tibia and ankle fractures.

This study reviews 24 patients with ipsilateral fractures of the distal tibia metaphysis and ankle joint. All fractures were evaluated and categorized by the mechanism of injury--that is, bending force versus torsion. All--tibial fractures in this series were managed by a statically locked intramedullary nail with appropriate stabilization of the ankle injury as indicated by the fracture or injury pattern. This treatment protocol resulted in an excellent clinical result with only 3 patients requiring a secondary procedure: 2 dynamizations and 1 exchanged intramedullary nail. The results indica

Bone fragility of the peripheral skeleton during fluoride therapy for osteoporosis.

Bone fragility during fluoride therapy for osteoporosis was observed in 24 (37.5%) of 64 patients treated with sodium fluoride, calcium, and vitamin D for 2.5 years who developed episodes of lower-limb pain during treatment. Eighteen (28%) of these patients had clinical and roentgenographic features of 41 stress fractures and 12 new spinal fractures. There were 26 periarticular, six femoral neck, three pubic rami, three tibia and fibula, one greater trochanter, and two subtrochanteric fractures. Vertebral fractures appeared first, then periarticular, then femoral neck, and lastly long-bone sha

Current Evidence: Plate Versus Intramedullary Nail for Fixation of Distal Tibia Fractures in 2016.

Displaced distal tibia shaft fractures are effectively treated with standard plates and intramedullary nails. Plate fixation performed with meticulous soft tissue handling results in minimal risks of infection and poor wound healing. Standard plates have high rates of primary union, whereas locking plates may delay union because of increased stiffness. Tibial healing may also be delayed after plating of the fibula, although fibula reduction and fixation may aid accuracy of reduction of the tibia. Malalignment occurs more often with infrapatellar intramedullary nailing versus plates, and early

When are CT angiograms indicated for patients with lower extremity fractures? A review of 275 extremities.

BACKGROUND: Computed tomography angiogram (CTA) is frequently utilized to detect vascular injuries even without examination findings indicating a vascular injury. We had the following hypotheses: (1) a CTA for lower extremity fractures with no clinical signs of a vascular injury is not indicated, and (2) fracture location and pattern would correlate with the risk of a vascular injury. METHODS: A retrospective review was conducted on patients who had an acute lower extremity fracture(s) and a CTA. Their charts were reviewed for multiple factors including the presence or absence of hard or soft

Staged Tibialization of the Fibula for a Large Segmental Tibial Defect: A Case Report.

CASE: A 15-year-old adolescent boy sustained a Gustilo-Anderson type III-B tibiofibular open fracture after a motorcycle collision. This injury and subsequent debridement resulted in a 15-cm defect in the tibial bone and skin. He underwent skin grafting and staged tibialization of the fibula to reconstruct a segmental tibial bone defect, stabilized using a double internal locking plate. At 10 months postoperatively, bony union was achieved, and the patient demonstrated favorable limb function. CONCLUSION: Staged tibialization of the ipsilateral fibula as a pedicle graft is a viable option for

[Typical pitfalls in the definitive reconstruction of pilon fractures].

The definitive reconstruction of a pilon fracture can be technically demanding and requires meticulous preoperative planning. Besides selecting the appropriate surgical approach and implant, particular attention must be paid to the reduction strategy, including the sequence of reconstruction, the required reduction maneuver and the necessary instruments. Successful execution of the surgical plan also depends on recognizing suboptimal reduction during the definitive reconstruction, a task that can be particularly challenging in the presence of extensive metaphyseal comminution. Various pitfalls

Malreduction of the tibiofibular syndesmosis in ankle fractures.

BACKGROUND: Diagnosis and reduction of syndesmosis injuries in ankle fractures can be challenging. Previous studies have demonstrated that standard radiographic measurements used to evaluate the integrity of the syndesmosis are inaccurate. The purpose of this study was to determine the adequacy of standard postoperative radiographic measurements in assessing syndesmotic reduction compared to CT and to determine the prevalence of postoperative syndesmotic malreduction in a patient cohort. METHODS: Twenty-five patients with ankle fractures and syndesmotic instability who had open reduction and

Indications and Surgical Treatment of Acute and Chronic Tibiofibular Syndesmotic Injuries with and Without Associated Fractures.

Syndesmosis injury may occur in a wide variety of clinical scenarios. Accurate diagnosis and anatomic reconstruction are necessary for optimizing clinical outcomes. The management considerations of syndesmotic injuries with associated proximal fibula fractures are reviewed. Methods to improve the accuracy of syndesmotic reduction are outlined. The management of fractures of the posterior malleolus, Chaput tubercle, and Wagstaffe tubercle is discussed with an emphasis on their contributions to syndesmotic stability. The evolving role of flexible fixation for syndesmosis injuries is discussed. C…

[Ankle dislocation without osseous lesions].

Approximately 7-10% of all emergency department patients are diagnosed with distortion of the upper ankle joint. A dislocation of the upper ankle joint without fracture is described in the literature as being very rare. In 33-50% the ankle dislocations are at least first-degree open and associated with a fracture of the tibia, the fibula or the talus. We report the course, treatment and results 12 months after trauma of a posteromedial first-degree open dislocation of the upper ankle joint without fracture. To our knowledge, there is no record of this pathology mentioned in the German language

Irreducible fracture dislocation of the ankle caused by tibialis posterior tendon interposition.

A patient with severe irreducible open fracture dislocation of the ankle was admitted to our emergency department. After wound irrigation and debridement, skeletal traction was applied to the calcaneus to minimize soft tissue injury and swelling. The patient was followed in traction for 1 week, after which reduction and fixation of the fibula was attempted but not achieved. We extended the incision distally, visualized the ankle, and located the tibialis posterior tendon between the distal tibia and fibula, thereby inhibiting the reduction. The tendon coursed into the tibiotalar joint anterior

Infected Nonunion of the Tibia Due to Paenibacillus turicensis in a Healthy Young Adult After an All-Terrain Vehicle Accident.

We present the case of a 19-year-old man with an open fracture of the tibia and fibula secondary to an accident with an all-terrain vehicle. He underwent operative excisional irrigation, debridement, and fixation on the day of injury. His course was complicated by nonunion of the tibia fracture. Infection is a common factor in fracture nonunion, even in patients who receive appropriate surgical and antimicrobial management. Paenibacillus turicensis, an organism adapted to survive in the environment via spore formation, was responsible for nonunion in our patient. A brief discussion of this unu

[Rotational physeal fracture of the distal tibia : where a keen clinical eye changes everything].

Ankle fractures are among the most common pediatric fractures, especially during the transitional period. They can lead to growth arrest, which is why careful attention is required when interpreting radiographs. This article discusses the case of an 11-year-old patient who sustained a rotational injury to the left ankle. Initially protocoled as negative, a second review of the radiographs supplemented by a CT scan revealed a Salter-Harris type II fracture of the distal tibia with 90° external rotation, associated with a non-displaced fibula fracture. Treatment consisted of a closed reduction a

Treatment of Radiation-Associated Fractures: A Critical Analysis Review.

»: Fractures that result from radiation-induced bone damage are a recognized adverse effect of radiation therapy (RT). »: The mechanisms of damage from RT are thought to be related to impaired vascularity, aberrations in osteoclast and osteoblast-mediated bone turnover, and compromise of cortical and trabecular microarchitecture. »: Treatment of radiation-associated fractures is challenging, with rates of delayed union and nonunion of >50%. »: Fracture management strategies, each with its own profile of risks and benefits, include prophylactic intramedullary nailing of long bones, open redu

The infected nonunion of the tibia.

Ununited fracture of the tibia complicated by infection is not only a complex surgical problem but also a chronic and at times debilitating condition. The principle methods used to diagnose and stage posttraumatic tibial osteomyelitis are described. Infected nonunions of the tibia are characterized by the extent of bony loss and the presence of a functional ipsilateral fibula. Using this tibial staging criteria, a series of 37 infected nonunions of the tibia are reviewed. Twenty patients were male and 16 were female, with an average age of 33 years. The distal third of the tibia was involved i

Innovative approach to intramedullary nailing of the fibula: a technical note.

Traditionally unstable ankle fractures are surgically managed using open reduction and internal fixation (ORIF) with plate and screws. However, the operative management has gained an innovative technique. In the last decade, intramedullary (IM) nailing was introduced in local guidelines as a treatment for a selective group of elderly patients with compromised soft-tissues, as this technique is minimally invasive and less prone to wound complications including infections. Based on the authors' experience with IM nailing of the fibula using an intramedullary locking fibula nail, common technical
