Willingness to pay for risky lifestyles: results from the Pay for Others (PAY4O) study, Italy.

OBJECTIVES: We assess the individual willingness to pay for diseases arising from risky lifestyles and investigate the personal factors that influence such willingness. STUDY DESIGN: We conducted an online survey with 821 respondents in Italy. The questionnaire was distributed via Facebook® in July and August 2016. The questionnaire covered sociodemographic characteristics, health status, behaviour and psychological attitudes, economic status, and opinion about covering the healthcare costs related to overeating, unhealthy diet, sedentary lifestyles, alcohol abuse, tobacco smoking, driving un

Fleming Fund Implementation in Kenya: What Have We Learned About Strengthening One Health Antimicrobial Resistance Surveillance for Decision-making?

BACKGROUND: Kenya has moved from fragmented, pilot-based antimicrobial resistance (AMR) surveillance toward a coordinated One Health system, and its experience, pathways of change, and remaining gaps are relevant for other low- and middle-income settings working to operationalize One Health AMR surveillance. METHODS: Here, we report a narrative overview of programmatic learning using 2018 as the pre-Fleming Fund baseline, drawing on national and global surveillance reports, monitoring frameworks from the Quadripartite indicators, qualitative evidence on governance, implementation and policy e

The 'survival job' pathway: Risk-focusing and occupational challenges among Canadian racialized and immigrant adults during the COVID-19 pandemic.

The impacts of the COVID-19 pandemic were not equally distributed, with emerging research demonstrating that racialized and immigrant populations in Canada have been disproportionately affected. Drawing on the risk-focusing hypothesis, a theoretical lens that explicates how risk burden concentrates disproportionately in marginalized populations, government mitigation strategies such as shutdowns can be seen as exacerbating existing social and economic challenges, making individuals more susceptible to COVID-19 and its adverse outcomes. This paper examines the economic and occupational challeng

Municipal structural adjustment: For an institutional analysis of global development finance.

In this paper I examine climate-aligned development finance initiatives (DFIs) that foster private-sector investment in 'sustainable' urban infrastructures. I am concerned with the ways in which they work to create the institutional preconditions for such investment. I develop the notion of Municipal Structural Adjustment (MSA) as a lens for examining the fields of intervention and modes of policy-making through which these initiatives seek to reform municipalities' governance structures, innovate planning tools and introduce financial instruments. Empirically, this paper focuses on initiative

Potential of a hand transplantation program in Colombia.

The protocols and published results on hand transplantation show acceptable results of this technology. None of the registered groups, however has a sufficient number of patients to allow continuity of the process. In Colombia the main problems are violence, drug traffic, and guerrillas. Thanks to the profitability of drug traffic, guerrillas, who initially were created based on ideologic differences, have become terrorist groups whose main source of income is drug traffic. From that interest comes the use of landmines to protect illicit crops. Colombia is the most mined country in the world,

Brazil: public health genomics.

Brazil represents half of South America and one third of Latin America, having more than 186 million inhabitants. After China and India it is the third largest developing country in the world. The wealth is unequally distributed among the states and among the people. Brazil has a large and complex health care system. A Universal Public Health System (SUS: Sistema SPACEnico de Saúde) covers the medical expenses for 80% of the population. The genetic structure of the population is very complex, including a large proportion of tri- hybrid persons, genetic isolates, and a panmictic large majority.

Towards mission-driven investment in new antimicrobials? What role for Chinese strategic industrial financing vehicles in responding to the challenge of antimicrobial resistance?

BACKGROUND: Antimicrobial resistance (AMR) causes high levels of global mortality. There is a global need to develop new antimicrobials to replace those whose efficacy is being eroded, but limited incentive for companies to engage in R&D, and a limited pipeline of new drugs. There is a recognised need for policies in the form of 'push' and 'pull' incentives to support this R&D. This article discusses China, a country with a rapidly emerging pharmaceuticals and biotech (P&B) sector, and a history of using coordinated innovation and industrial policy for strategic and developmental ends. Evaluation of the use of modelling in resource allocation decisions for HIV and TB.

INTRODUCTION: Globally, resources for health spending, including HIV and tuberculosis (TB), are constrained, and a substantial gap exists between spending and estimated needs. Optima is an allocative efficiency modelling tool that has been used since 2010 in over 50 settings to generate evidence for country-level HIV and TB resource allocation decisions. This evaluation assessed the utilisation of modelling to inform financing priorities from the perspective of country stakeholders and their international partners. METHODS: In October to December 2021, the World Bank and Burnet Institute led

Oral health payment systems around the world: structures, challenges, and trends, a scoping review.

BACKGROUND: The prevalence of dental and oral diseases is increasing globally, yet the utilization of dental services has not shown a corresponding rise. Cost remains a major barrier, as dental care ranks among the four most expensive health services globally. These costs are shaped by each country's health payment system, which plays a critical role in determining access to healthcare. This scoping review aims to map the existing evidence on oral health payment systems and examine how these systems influence access, utilization, equity, and financial protection in different settings. METHODS

Delivering physical rehabilitation services during the COVID-19 pandemic in Iran: Common challenges and potential solutions for future pandemics.

The COVID-19 pandemic affected people with disabilities in different aspects, including their access to physical rehabilitation (PR). Despite a significant global surge in the need for PR, it has not been prioritized and is under-resourced, even in the non-COVID era, in many countries. This study aimed to explore the challenges of delivering PR services during the COVID-19 pandemic in Iran and potential solutions to these challenges. This qualitative study was done using a thematic approach from November 2023 to March 2024 in Iran. 45 PR professionals and faculty members from three fields-phys

Navigating cost and care: A qualitative study on oncologists' perspectives on financial toxicity in India.

BACKGROUND: This study aimed to explore oncologists' perspectives on how financial toxicity (FT) shapes clinical decision-making and to identify potential solutions to reduce its impact. METHODS: We conducted a qualitative study using semi-structured interviews with nineteen oncologists working across public, private, and not-for-profit hospitals in nine Indian states. The interviews were audio-recorded, transcribed, and analyzed till thematic analysis until data saturation was achieved. Policy analysis of the Global Financing Facility in Uganda.

BACKGROUND: In 2015, Uganda joined the Global Financing Facility (GFF), a Global Health Initiative for Reproductive, Maternal, Newborn, Child, and Adolescent Health (RMNCAH). Similar initiatives have been found to be powerful entities influencing national policy and priorities in Uganda, but few independent studies have assessed the GFF. OBJECTIVE: To understand the policy process and contextual factors in Uganda that influenced the content of the GFF policy documents (Investment Case and Project Appraisal). METHODS: We conducted a qualitative policy analysis. The data collection included a

Who Pays When Temperatures Soar?

The health effects of extreme heat have gained attention rapidly, as rising global temperatures cause more frequent and severe heat waves. More attention is needed, however, to the myriad pathways by which the financial and physical tolls of extreme heat are ultimately borne by the most disadvantaged members of society. These tolls include direct impacts on health and indirect impacts on income as costs incurred by industry, government, and other entities are passed on to individuals as increased prices or decreased earnings. Strained household finances in turn affect individuals' ability to p

Continuous ambulatory peritoneal dialysis (CAPD): experience with the first 100 patients in a Hong Kong centre.

We treated 100 Chinese patients age 16 to 83 years by CAPD, using three 2-litre exchanges per day. The treatment was self-financed in 69 patients, by charitable organisations in 25 patients, and by government funds in 6 patients. Satisfactory biochemistry was maintained and there was no gross hyperlipidaemia, renal osteodystrophy, or loss of ultrafiltration capacity of the peritoneum. Rehabilitation was good and 62% of patients returned to full-time employment. The average duration of hospitalization was 11.3 days per patient year. Peritonitis usually due to Staphylococcus pyogenes occurred at

Medical and psychosocial aspects of the habilitation of thalidomide children.

The habilitation of 34 thalidomide children, begun in 1963 at the Rehabilitation Institute of Montreal, involved the contribution of many medical and paramedical disciplines. These individual contributions are described.Habilitation of congenitally deformed children must be initiated at an early age. The early prescription of a prosthesis is desirable. Their care involves a broad range of services that must be continued over many years. Parents require support before they can accept the misfortune that has befallen them and their deformed child. Despite the necessity of frequent hospitalizatio

Rare diseases in Tanzania: a National Call for Action to address policy and urgent needs of individuals with rare diseases.

A rare disease is generally defined as a condition which affects about 1 among 2000 people and currently, there are approximately 5000-8000 rare diseases (RDs) affecting over 400 million people world-wide. Although RDs may arise from different causes such as infections and environmental factors, about 80% are caused by genetic abnormalities. In Tanzania, there are no reports of the types of RDs, their incidence, distribution and numbers of individuals affected. In addition, there have been no strategies to map RDs in the country and develop a definition that fits the local context. Public awar

Improving access to child health services at the community level in Zambia: a country case study on progress in child survival, 2000-2013.

Reductions in under-five mortality in Africa have not been sufficient to meet the Millennium Development Goal #4 (MDG#4) of reducing under-five mortality by two-thirds by 2015. Nevertheless, 12 African countries have met MDG#4. We undertook a four country study to examine barriers and facilitators of child survival prior to 2015, seeking to better understand variability in success across countries. The current analysis presents indicator, national document, and qualitative data from key informants and community women describing the factors that have enabled Zambia to successfully reduce under-

Eradication, control or neither? Hookworm vs malaria strategies and Rockefeller Public Health in Mexico.

Malaria's epidemiological importance in Mexico greatly exceeded that of hookworm, but the Rockefeller Foundation (RF) paid far more attention to hookworm. Although the RF collaborated with malaria campaigns around the world, malaria was only incidental to the RF's activities in Mexico. The hookworm campaign, on the other hand, involved the RF at every stage, from conceptualization and design to financing, hiring, and day-to-day administration. This paper seeks to understand why the RF's involvement in Mexico differed for the two diseases and what the organizational, political, and health impli

Resilience as recovery: stakeholder perspectives on Uganda's health system response to infectious disease shocks.

BACKGROUND: The conceptualization, definition, and measurement of health system resilience remain contested, hindering policy integration. Country-specific empirical evidence from sub-Saharan Africa, where infectious disease shocks recur, is limited. This study explored how national-level Ugandan stakeholders conceptualize health system resilience to infectious disease shocks and its attributes. METHODS: We conducted a descriptive exploratory qualitative study using semi-structured key informant interviews with 15 purposively selected national-level stakeholders from the Ministry of Health, a
