This report describes a new technique for treatment of the chronically flexed fingers which applies in particular to fingers previously operated on several times and now presenting cutaneous, tendinous and joint problems. It consists in releasing the entire flexor apparatus through a full length lateral digital incision and a sub periosteal dissection. The volar plates of PIP and DIP are released as a whole with the flexor apparatus. The extended finger and flexor apparatus then are allowed to heal in a new relationship. Straightening of the finger is always possible. The range of motion is ma

INTRODUCTION: Metacarpophalangeal (MCP) joint dislocation and its pathoanatomy was first discussed in detail in an article written by Kaplan et al . ,in 1957. They had identified certain features of complex, that is irreducible MCP joint dislocation through closed method to differentiate from a simple MCP joint dislocation, that is MCP joint dislocation that can be reduced by closed method. The complex MCP joint dislocations (mostly involving the index finger) are called Kaplan dislocations. Here, we describe two cases of MCP joint dislocations to highlight the differences between complex and s

History A 34-year-old man without underlying medical conditions came to the emergency department for evaluation of persistent pain over the volar portion of his right fifth finger after a fall during a football match 3 days before. At physical examination, the injured finger was swollen and purple. Passive and active flexion of the proximal and distal interphalangeal joints were compromised, without interphalangeal instability. Radiography was performed in the emergency department, and the patient was released with a diagnosis of a fifth digit sprain. After the senior radiologist (V.M.C.) revi

The choice of prosthetic or autologous reconstruction for proximal interphalangeal (PIP) joint arthroplasty in degenerative osteoarthritis represents a challenge for hand surgeons, especially in consideration of complications and patient's quality of life. We report the case of a 49-year-old woman who developed diffuse arthritis of the finger joints, especially at the PIP joint of the third right finger. Radiographs showed destruction of the PIP joint, large osteophytes, marked narrowing of joint space, severe sclerosis, and deformation of bone contour. Through a volar approach, we removed the

The locked metacarpophalangeal joint of a finger is an unusual condition frequently confused with a "trigger finger." The causes can be classified into degenerative and spontaneous groups. Ten cases are presented. The cause of locking of one patient in the degenerative group was gouty arthritis, a cause not previously reported in the literature. Although surgery is usually necessary, joint mobility was restored in 30% of the cases by distending the joint capsule with an intraarticular injection of a local anesthetic followed by gentle manipulation. In addition to routine x-ray films, arthrotom

A dorsal longitudinal incision provides excellent exposure for reducing complex dislocations of the metacarpophalangeal joints of the thumb and index finger. Within the past four years, we have successfully treated five patients with irreducible dislocations of the metacarpophalangeal joint of the thumb and four with dislocations of the index finger by open reduction through a dorsal incision. This approach exposes the joint and the major structure blocking reduction, the volar plate. The digital nerves, which are tented volarly over the metacarpal head, are not prone to injury and osteochondr

We propose a novel wearable robotic glove or exo-glove design scalable to the variation of the hand kinematics. While most of the traditional robot hand is driven by rotating the joint directly with a rigid body, our exo-glove deforms a robotic finger's skin and, thus, the hand skeleton joints. Multiple tendons woven on the exo-glove's surface can make multi-DOF finger joint motions. We allocated tendons to mimic a hand's intrinsic and extrinsic muscles. Thus, a robotic hand actuated with the exo-glove can perform natural finger motions, including abduction/adduction and flexion/extension of f

We report the first case of pseudogout attack in the distal interphalangeal (DIP) joints during etidronate disodium therapy. A 64-year-old woman had intermittent administration of etidronate disodium (Didronel; Sumitomo, Osaka, Japan) alone because of osteoporosis. Each cycle consisted of a daily dose of 200 mg for 2 weeks, repeating every 12 weeks. Two weeks after completing the third cycle, severe pain and swelling occurred in the DIP joints of the right middle, ring, and left ring finger; and skin ulcer formation was observed on the dorsal side of the DIP joints of the right middle and ring

One hundred and twenty six patients (148 hands) with Dupuytren's disease of the fifth finger were reviewed with an average follow-up of 5 years. Twenty-nine dermo-fasciectomies were performed on the palmar area of the fifth finger. The improvement in the MP joint was complete in all cases. A classification in to 4 grades according to the degree of pre- and postoperative retraction is proposed. The average improvement at the PIP joint was 50 degrees. 26 recurrences were observed. The recurrence rate was similar with or without a skin graft but the functional result was better with a digital gra

Dorsal fracture-dislocations of the proximal interphalangeal joint are challenging injuries to treat and are associated with many complications. The determination of stability is crucial to appropriate management. Stable injuries can usually be treated nonsurgically, whereas unstable injuries typically require surgical stabilization. Many surgical techniques have been used, including extension block pinning, volar plate arthroplasty, open reduction and internal fixation, external fixation, and hemihamate autografting. Because stiffness and flexion contracture are frequent complications, every

Fracture and luxation of the long fingers are particularly frequent in hand trauma. Untreated or poorly treated, they may leave highly disabling sequelae. The aim of treatment is two-fold: to repair the anatomical lesion and to obtain movement as rapidly as possible, the only means of avoiding joint stiffening. It is important to underline that the metacarpo-phalangeal joints usually stiffen in extension and that the proximal interphalangeal joints on the contrary stiffen in flexion. Thus, in case of immobilisation of the hand, an attempt should be made to obtain an "intrinsec plus" immobilisa

Irreducible palmar dislocation of the proximal interphalangeal joint in which the central slip remains intact is a rare injury. A noose formed by the extensor mechanism traps the condyle of the proximal phalanx. The collateral ligament is avulsed and the volar plate is disrupted. Closed reduction was unsuccessful in this case. Open reduction achieved a good result.

A dislocation of the interphalangeal joint of a thumb was treated without problems. The simultaneous dislocation of the lunate was not diagnosed or treated until the plaster cast was removed. Even in cases with an obvious lesion like a finger joint dislocation it is necessary to investigate all other fingers and the wrist to exclude other lesions and to avoid delay in treatment.

Finger fractures and dislocations are commonly seen in the primary care setting. Patients typically present with a deformity, swelling, and bruising with loss of function. Anteroposterior, lateral, and oblique radiography should be performed to identify fractures and distinguish uncomplicated injuries from those requiring referral. Uncomplicated distal phalanx fractures, caused by a crush injury to the end of the finger, require splinting of the distal interphalangeal joint for four to six weeks. Uncomplicated dorsal avulsion fractures (mallet finger) of the distal interphalangeal joint, cause

Pachydermodactyly is a rare and benign form of acquired digital fibromatosis characterized by the expansion of soft tissue around proximal phalanges and interphalangeal joints. The etiology remains unknown, although it is suggested that repetitive mechanic trauma like the interlacing or rubbing of the fingers can lead to skin thickening. We report a case of a young man with skin thickening around interphalangeal joints and compulsive finger manipulation habit. The histopathology disclosed hyperkeratosis, discrete papillomatosis, mild increase of fibroblasts and dermal mucinosis. The lesions re

A tendon subluxation test was performed on the unaffected side in 13 patients with traumatic extensor tendon dislocation of the middle finger and on both middle fingers in 800 healthy controls to identify extensor mechanism laxity at the metacarpophalangeal joint. Ten of the 13 patients with dislocation had laxity of middle finger extensor tendon, compared with 174 of the 800 (22%) controls. The difference in these rates of extensor tendon laxity is significant (P<0.0001), and suggests that extensor mechanism laxity at the metacarpophalangeal joint may predispose to traumatic extensor tendon d

PURPOSE: This study aimed to present the clinical outcomes of the dorsal plating technique for palmar fracture dislocations of the proximal interphalangeal (PIP) joint. This plating technique minimizes dissection and interference with the finger extensor mechanism.
METHODS: We treated seven patients (with a mean age of 39.1 years) with dorsal hook plates for palmar fracture dislocations of the PIP joint between April 2018 and August 2022. The little finger was affected in five patients, and ring finger was affected in two. The mean time to surgery was 5.6 days, and the postoperative follow-up
