Assessment of Kidney Function in Survivors Following Fontan Palliation.

OBJECTIVE: The Fontan operation is a palliative procedure for congenital single ventricle heart disease. Long-term kidney function in this cohort is not well-known. Our aim was to evaluate renal function in long-term survivors post-Fontan palliation, and we hypothesize that this cohort will have a higher prevalence of chronic kidney disease (CKD) compared to controls. DESIGN: We performed a retrospective cohort study of 68 subjects evaluated through the Single Ventricle Survivorship Program at the Children's Hospital of Philadelphia between July 2010 and December 2014 compared to 70 healthy c

Case of double-outlet right ventricle after repair with pulmonary arteriovenous malformations using cardiac CT.

We present a case where cardiac CT clearly defines the complex anatomy of a 22 year old man with double outlet right ventricle who is status bilateral bidirectional Glenn and Fontan conduit. This case also shows the association of pulmonary arteriovenous malformations with Fontan and Glenn procedures.

Extracardiac predictors of functional capacity in a group of Fontan patients.

BACKGROUND: The Fontan procedure, a palliative surgical approach for single-ventricle physiology, has significantly improved long-term survival. However, it is frequently associated with reduced exercise capacity and multiorgan dysfunction. In this context, peripheral muscle function and body composition may influence functional outcomes. OBJECTIVE: The aim of the study was to evaluate the relationships among body composition, handgrip strength, and functional capacity in Fontan patients, as well as to investigate the impact of Fontan-related variables on these parameters. METHODS: This sing…

The Use of Pulmonary Vasodilators in Pediatric Patients with Single-Ventricle Palliation: A Ten-Year Experience in a Tertiary Care Center.

In single-ventricle circulation, pulmonary vascular resistances (PVR) play a crucial role at various stages of surgical palliation. Increased PVR detected at cardiac catheterization represents a contraindication to Fontan completion and may lead to an early circuit failure in the postoperative period. Pulmonary vasodilator therapy (PVT) may lower PVR and enhance pre- and post-surgical outcomes in Fontan patients. This study reports the experience with the use of PVT in a tertiary center, focusing on its role in lowering PVR before Fontan procedure and assessing its impact on postoperative outc…

Atrial flow regulator for failing Fontan circulation: an initial European experience.

Transcatheter creation or enlargement of an atrial septal defect has been used to promote adequate blood flow and mixing in some forms of congenital heart defects or as a relief valve in right or left atrial hypertension, resulting in better cardiac output and/or systemic saturation. We report a case of a 4-year-old male affected by complex congenital heart disease who was admitted for management of severe cyanosis following a staged pericardial fenestrated Fontan procedure. Transoesophageal echocardiogram showed a wide fenestration of 9 mm in size with a severely dilated pericardial Fontan sy…

Total cavopulmonary anastomosis (Fontan) in children with Down's syndrome.

BACKGROUND: There is a paucity of information to guide the management of the child with Down's syndrome and congenital heart disease in whom biventricular repair is precluded. METHODS: Through the cardiology and cardiovascular surgery databases of The Hospital for Sick Children and Toronto Congenital Cardiac Centre for Adults, we identified patients with trisomy 21 and ventricular hypoplasia who had undergone a Fontan procedure (or modification). RESULTS: Of 533 patients who had undergone a Fontan operation between 1976 and 1997, 4 had trisomy 21. All 4 patients had unbalanced complete atrio…

Glenn shunt: long-term results and current role in congenital heart operations.

Fifty cyanotic patients (aged 2 days to 22 years) underwent Glenn shunts for tricuspid atresia and other cyanotic heart defects. Thirteen of … us collaterals and decreased flow to the opposite lung necessitated Blalock-Taussig shunts in 6 and Fontan procedures in 10. All survived the Fontan procedures with minimal morbidity. These data support the concept that Glenn shunts do not necessarily result in pulmonary abnormalities and may be indicated as a staged procedure in a few selected patients prior to a …

Prevalence of Subclinical Enteric Alpha-1-Antitrypsin Loss in Children with Univentricular Circulation Following Total Cavopulmonary Connection.

Protein-Losing Enteropathy post Fontan palliation is associated with significant morbidity and mortality. To date, very little research has … elevated stool alpha-1-antitrypsin. Subclinical protein loss is rare in asymptomatic children after Fontan procedure with only 5% of patients having elevated stool alpha-1-antitrypsin but no other symptoms. These findings may relate to our small cohort size and the time to testing post cardiac surgery. Future longitudinal follow-up studies should assess the abilit …

The Lifesaving Impact of Transcatheter Interventions in the Early Post-Fontan Palliation Period.

Despite advancements in postoperative outcomes after Fontan surgery, there remains a risk of suboptimal outcomes and significant morbidity in the early postoperative period. Anatomical obstructions in the Fontan pathway can lead to prolonged pleural effusion or ascites, cyanosis, and low cardiac output syndrome (LCOS). Transcatheter interventions offer an alternative to early re-surgery for treating these complications. Over a 13-year period, early catheter angiography, performed within 30 days post-index procedure, was administered to 41 patients, identifying anatomical issues that necessitat

Functionally Single-Ventricle Complications After Fontan Palliation-A Narrative Review.

Functionally single-ventricle (FSV) defects are complex congenital heart anomalies that require Fontan palliation, a surgical procedure redirecting systemic venous blood directly to the pulmonary arteries, bypassing the heart. Despite improvements in surgical techniques and perioperative care leading to enhanced survival rates, patients remain vulnerable to significant long-term complications, due to the unique Fontan circulation physiology. This circulation relies on low pulmonary vascular resistance and preserved single-ventricle function but predisposes patients to venous congestion and red

Overgrowth of the Amygdala in Children with Single Ventricle Congenital Heart Disease.

Aim: Early life stress is associated with overgrowth of the amygdala, which plays a key role in the processing and memory of emotional responses. Herein, we aimed to explore the amygdala volume in children with single-ventricle congenital heart disease who experience repeated admissions during the neonatal period and infancy. Methods: We compared the amygdala volume measured using brain magnetic resonance imaging (MRI) between 40 patients after completion of the Fontan procedure and 40 age- and sex-matched control subjects Results: Age at the MRI study were 9.2 (8.5-11.1) and 10.2 (9.2-10.3) y…

Surgical and Catheter-Based Reinterventions Are Common in Long-Term Survivors of the Fontan Operation.

BACKGROUND: There are limited follow-up studies examining surgical and catheter-based reinterventions in long-term survivors of the Fontan operation. METHODS AND RESULTS: All 773 patients who underwent Fontan at our institution between 1992 and 2009 were retrospectively reviewed. Current information regarding post-Fontan intervention was available for 70%. By 20 years after Fontan, 65% of patients had experienced either surgical or transcatheter intervention. The median time to first reintervention was 9.8 years. Freedom from reoperation was 69% at 15 years and 63% at 20 years. The most commo

Importance of acquired systemic-to-pulmonary collaterals in the Fontan operation.

BACKGROUND: Children with chronic cyanotic heart disease often develop systemic-to-pulmonary collateral arteries that can be deleterious at the time of a Fontan procedure due to excessive pulmonary blood flow. We therefore occlude all significant collaterals during cardiac catheterization. METHODS: From June 1993 to May 1998, 93 children aged 1.5 to 15.8 years (median 2.5 years) underwent a fenestrated lateral tunnel Fontan procedure. Eighty-nine (96%) had a previous bidirectional Glenn anastomosis, including 31 (33%) with a Norwood procedure. RESULTS: Preoperatively, 33 children (35%) requi…

Earlier Fontan Candidacy Determination: A Focus on Anatomic and Physiologic Variables.

BACKGROUND: Timing and patient selection for Fontan palliation in single-ventricle congenital heart disease remain subjects of debate. An early Fontan procedure can limit long-term morbidity after total cavopulmonary connection, yet patient-specific anatomic and physiologic variables may limit candidacy. Weight thresholds are often applied to anticipate physiologic tolerance and acceptable achievable conduit size. The purpose of this study was to evaluate outcomes within a single, high-volume center by using anatomic and physiologic variables rather than a weight threshold for Fontan candidacy…
