Idiopathic focal myositis is a rare clinical entity and is mostly localized in the neck and thigh muscles presenting as a pseudotumor. We describe a 49-year-old patient with inflammation restricted to the small muscles of one forefoot. Systemic disease was not present and progression to polymyositis did not occur within 2 years of follow-up. The myositis improved promptly after initiation of immunosuppressive therapy with corticosteroids and azathioprine. This unusual location of focal myositis has not been previously reported.

UNLABELLED: In 35 patients with diabetic gangrene, amputation above the ankle (n = 18) or below the ankle (n = 17) was planned by nonspecialised general surgical departments or practicing surgeons. Prior to the operation, these patients presented to a specialised diabetic foot clinic; their clinical courses were analysed retrospectively.
RESULTS: in 14 cases, peripheral arterial occlusive disease (PAOD) stage IV was the underlying cause, which was treated by bypass-surgery in 10 cases and by angioplasty in 1 case. In 2 cases, bypass-operation was impossible due to anatomic conditions, and in …

The methods and results of treatment of 13 cleft foot in 8 patients are presented. An approximating dexon-polyester suture has been used in Blauth I deformity. Joplin procedure (resection of the intermediate cuneiform) has been done in grade III deformities. If hallux valgus coexisted an arthrodesis of interphalangeal joint has been performed. Family history of hand and foot deformity in one case might prove genetic theory of autosomal dominant inheritance. At follow-up examination mean 11 years after surgery satisfactory cosmetics, efficient gait and standard shoes fitting were found.

Athlete's foot, or tinea pedis, is a fungal infection that commonly affects active individuals. Intense itching is the primary symptom. Rash distribution, degree of redness, type of scaling, and presence of vesicles can distinguish athlete's foot from inflammatory conditions, but there is much overlap in presentation. Antifungal creams, used consistently for a period of weeks, are generally effective; in certain cases, keratolytics or oral antifungals are necessary. Educating patients about medication use, appropriate footwear, and avoiding excessive heat and moisture can help keep the conditi…

The assessment of congenital foot deformities is only relevant in the context of German legislation (SchwbG) defining disability and fitness for military service. All assessments of congenital foot deformities should be based on the twin concepts of function and capacity to withstand stress. However, assessments with regard to the degree of disability and fitness for military service will occasionally differ. This article discusses the various congenital foot deformities individually, allocating them to the respective degrees of disability [as laid down in German law]. The classification may s…

Diabetic neuropathy may cause an alteration of the function of the muscles of the sole of the foot. This is at the origin of the chronic dislocation of the articular heads, mainly of the little metatarsal bones (neuro diabetes arthropathy), with formation of areas of pathological pressure. The tissues below being chronically under pressure are affected by trophic lesions called "plantar perforating disease". Recovery may be obtained by not exposing the injured area of the foot to pressure and through careful local therapy. This doesn't prevent disease from appearing again, even though some spe…

Foot-progression angle was determined using the Shutrack carbon paper system for 52 feet treated by clubfoot release without wide subtalar release and 43 age-matched controls. Mean foot progression angle was -5 degrees for the clubfeet. This represented 13 degrees of inturning from the normal mean of 8 degrees. Forty-eight percent of the clubfeet had inturning greater than 2 SD from the normal mean. The causes of inturning included adductus (p = 0.005) and, in some cases, internal tibial torsion or internal femoral torsion. Varus of the foot was usually associated with inturning, but inturning…

Standardisation is vital if the clinical images are to be used for monitoring any changes in the patient's condition. The two images can be cut and mounted in one 35 mm mount for comparison.

Congenital triangular deformity of the foot bones may occur in phalanges and metatarsals. As it resembles the Greek letter delta it is called "delta phalanx" and "delta metatarsal". We report on 19 delta formations in ten patients with foot polydactyly. Our long-term follow-up of these patients indicates four stages in the ossification process: no ossification of the epiphysis; appearance of single or multiple ossification centers; unification of the ossification centers with nonosseous tissue between the diaphysis and the epiphysis; and, finally, complete ossification. Pathogenetically, the d…

Numerous studies have shown that bacteremias occur in patients during and immediately after some types of surgeries, placing the patient at risk of serious infection. This study was designed to ascertain whether a bacteremia will exist following routine podiatric surgery. Blood cultures were taken perioperatively from 42 subjects, and were incubated both aerobically and anaerobically, and swabs of the incision were made during the surgery. No bacteremias occurred during this study. This suggests that podiatric surgery, when properly performed, does not present a risk of bacteremia to the patie

This is a study of the history of club-foot from ancient times up to the present. It embraces not only the clinical aspects of the deformity but also its social implications, particularly of its early history when the attitude of society towards the lame and crippled in general and to foot deformities in particular is discussed. Relevant references from Greek mythology and also from the Old Testament are quoted. A close study is made of the old Chinese custom of foot-binding which produced club-foot like deformities which became a symbol of social standing. Inevitably, like most other medical …
