We present a case of forearm soft tissue mass caused by an accessory muscle, distal and deep to flexor carpi ulnaris muscle. Imaging studies, mainly magnetic resonance and ultrasound, allow a specific diagnosis, and obviate unnecessary surgery. In this case, the symptoms associated with ulnar compression led to surgery and confirmed the preoperative diagnosis.

Various attempts at reconstruction of pharyngoesophageal defects after ablative surgery have been made to restore the function of the pharyngoesophagus. A tubed free radial forearm flap was used to reconstruct the pharyngoesophagus in 23 patients after resection of neoplasms from May 1989 to October 1995. Nineteen were males and four were females, the average patient age was 62.2 years. The follow-up ranged from 10 to 64 months (mean: 18 months). Oral intake within 3 weeks was possible in 18 patients (78 percent) The immediate postoperative complications were hematoma (n = 1), bleeding (n = 2)…

OBJECTIVE: Photoplethysmography utilizes a green-light-emitting diode to transmit light into a tissue. Reflected light from hemoglobin in dermal capillary red blood cells is received by a photo detector and is analyzed as light intensity along a frequency spectrum. This method of analysis allows for the removal of "noise" above (stray light and alternating current [AC]) and below (room vibrations and respiratory motion) the peak signal (1 to 2 Hz) and results in a means to distinguish between perfused and nonperfused tissues.
METHODS: Twenty-two of 30 consecutive radial forearm free flap (RFF…

The distally based forearm island flap is vascularized by the perforators of the distal radial artery. The skin flap is along the axis of the radial artery, and the pivot point of its subcutaneous pedicle is about 2 to 4 cm above the radial styloid process. We have treated 12 patients with 12 flaps for soft-tissue defects of the hand. Of these recipient sites, seven were in dorsal hands, two were in thumbs, two were in forearms, and one was in the palmar area. The donor-tissue variants included eight skin flaps, two adipofascial flaps, and two sensate flaps. The sizes of the flaps ranged from …

The effects of forearm rotation and metacarpophalangeal (MP) flexion on carpal tunnel pressure were investigated in 17 healthy adults who had no evidence of carpal tunnel syndrome (CTS). Pressure was continuously recorded with a saline-filled catheter inserted into the carpal tunnel and connected to a pressure transducer while test subjects slowly rotated the forearm from full pronation to full supination. Forearm rotation was repeated with MP flexion of 0 degrees, 45 degrees, and 90 degrees. Both forearm rotation and MP flexion, and their interaction term, significantly affected carpal tunnel…

We treated two children with the unusual complication of ulnar nerve palsy after closed both-bone forearm fractures. Both patients developed an ulnar claw-hand deformity within 7 weeks of injury that resolved spontaneously by 20 weeks postinjury with nonoperative treatment. No patient showed any signs or symptoms of an ischemic compartment syndrome. Both nerve injuries were identified immediately at the time of fracture by a careful neurologic examination. This avoids confusion with a postreduction nerve entrapment injury or ischemic injury after a localized compartment syndrome, which may hav…

Reciprocal inhibition between forearm extensor and flexor muscles was tested by means of an H-reflex technique in patients with spasmodic torticollis and normal controls. In both, patients and controls three different phases of reciprocal inhibition could be demonstrated with maximal inhibition at conditioning test intervals of 0 ms, 15 ms and 100 ms, respectively. However, the quantitative amount of this inhibition was different for the patients and the controls. Significantly less inhibition was found for the second and the third phase of reciprocal inhibition in the patient group. Discrimin…

One of several factors suspected in the development of lateral epicondylitis, often referred to as tennis elbow, is the impact-induced vibration of the racket-and-arm system at ball contact. Using two miniature accelerometers at the wrist and the elbow of 24 tennis players, the effects of 23 different tennis racket constructions were evaluated in a simulated backhand stroke situation. The influences of body weight, skill level, and tennis racket construction onto the magnitude of vibrations at wrist and elbow were investigated. Amplitudes, integrals, and fourier components were used to charact

Eighteen patients with Gustilo and Anderson Grade III A (7), III B (8) or III C (3) open diaphyseal forearm fractures were treated with a protocol consisting of extensive primary debridement, immediate open reduction, dynamic compression plate fixation, and vascular repair when indicated. This was followed by routine redebridement at 24 to 48-h intervals until wound status allowed completion of soft tissue reconstruction. Bone grafting was performed at 8 to 10 weeks following obtainment of a closed soft tissue envelope for injuries with extensive comminution or bone loss (5 patients, 7 fractur…

Forearm diaphyseal fractures in children are commonly treated by conservative management. In about 5% of cases, however, unstable fractures occur and require surgical intervention. We prefer the intramedullary pinning procedure, which has already been published in a similar version in 1913 by Schöne and in 1947 by Bsteh. 41 patients were followed up between 1979 and 1988. One instance with a severe infection of a second-degree open fracture required two reoperations and was completely restored with a very good result. An overall assessment of our collective yielded 31 patients with findings ra…

The technique and results of a series of 38 Baldwin's operations (excision of the ulnar neck) are described. This is a simple and effective procedure to restore forearm rotation following malunited fractures of the distal forearm.

The radial forearm flap, although widely used, has been criticized for the poor quality of its donor site. To investigate the causes of morbidity, 100 radial artery free-flap donor sites have been reviewed. Sixty-seven patients required skin grafting (group 1), and the remaining 33 patients were closed directly (group 2). Seventeen patients in the series had compound osteocutaneous flaps (group 3). Wound healing proved to be a significant problem in groups 1 and 3, and fracture of the radius occurred in 4 of the 17 patients in group 3 and was the most significant cause of morbidity. The radial…

The radial forearm island flap may be used for resurfacing areas of skin in the upper limb. It allows a safe one stage reconstruction of skin loss providing thin, pliable and innervated skin cover. We present five cases which demonstrate the versatility of the flap.

Internal fixation with AO compression plates was carried out in 8 per cent of all children with displaced diaphyseal fractures of the forearm. Primary treatment for 23 of these 43 fractures consisted of primary fixation. All fractures healed in anatomical alignment. Deep infection was observed in 1 child. All fractures united but 1 refractured . Of 29 children, 27 were reexamined after a mean observation time of 5 years; 22 had obtained normal function of the arm, and nobody had restriction of rotation of the forearm exceeding 20 degrees. The mean overgrowth of the bones was 2.4 mm. Fifty-six …

A 57-year-old patient suffering from late-onset McArdle's disease developed myoglobinaemia, massive myoglobinuria and marked serum creatine kinase elevation subsequent to a routinely performed forearm ischaemic work test. Twenty hours after the test, enhancement of 99mTc methylene-diphosphonate activity was demonstrated exclusively in the tested forearm. It is concluded that the forearm ischaemic work test is potentially hazardous to McArdle patients, as it might induce myoglobinuria sufficient to result in acute myoglobinuric renal failure.
