[Peripheral arterial injuries due to fractures and dislocations].

Using 12 typical cases, which are tabulated, the diagnosis and therapy of combined bone and vascular injuries are demonstrated. Particular emphasis is put on a detailed presurgery diagnosis, if possible angiographic diagnosis. Early surgery, subject to a generous indication, with resection of the injured vascular segments and subsegment interposition of an autologous vein transplant is preferred. The correctness of this procedure is confirmed by the corresponding good postsurgery results.

Dislocation of the hip associated with fracture of the femoral head.

With increasing numbers of road traffic accidents, dislocation of the hip associated with fracture of the femoral head is becoming more common. Seven cases are presented. Treatment of this injury is difficult but it is suggested that conservative methods should be considered initially. Long-term follow-up is necessary as many of these patients develop late complications.

Ankle joint dislocation treating dislocated trimalleolar fractures accompanied with the complex posterior malleolus fracture without separation of the tibiofibular syndesmosis.

To explore the therapy of ankle joint dislocation treating dislocated trimalleolar fractures accompanied with the complex posterior malleolus fracture without separation of the tibiofibular syndesmosis and improve surgical results.Patients who had dislocated trimalleolar fractures accompanied with the complex posterior malleolus fracture without separation of the tibiofibular syndesmosis were retrospectively analyzed and 30 patients were enrolled the study. They were all treated by ankle joint dislocation and the surgical results were evaluated by the Baird-Jackson ankle scoring system. Longit

Results of Perilunate Dislocations and Perilunate Fracture Dislocations With a Minimum 15-Year Follow-Up.

PURPOSE: To evaluate the long-term clinical, functional, and radiological outcomes in 30 patients with at least 15 years of follow-up. METHODS: We performed a retrospective study that identified 73 patients. Thirty patients agreed to participate and were included. The mean follow-up was 18 years (range, 15-24 years). There were 14 cases of perilunate dislocation and 16 cases of perilunate fracture-dislocation (including 13 transscaphoid perilunate fracture-dislocations). At the last follow-up, the clinical and functional evaluation was based on the range of motion, grip strength, the Mayo wri

Perilunate dislocations and fracture-dislocations. Closed and early open reduction compared in 28 cases.

27 patients (28 cases), were treated for perilunate dislocations in 20 and fracture-dislocations in 8. Capitate displacement was dorsal in 27 cases and palmar in 1 case, and in 16, the scaphoid was fractured. There was no substantial delay of treatment. In 8 patients treated with closed reduction, follow-up averaged 6 (1-15) years. 19 patients (20 cases) who underwent early open reduction with K-wire stabilization and ligamentous repair, had an average follow-up of 2 (0.5-7) years. The clinical outcome was evaluated using a scoring system based on pain, occupation, ROM and grip strength, while

[Reposition of old dislocations or dislocation fractures of the carpal bones using Wagner's distraction apparatus].

In treatment of old traumatic dislocations and fracture dislocations of the carpal bone complex, a careful and exactly dosed pre-extension of the shrunken capsular ligaments can be obtained by the use of Wagner's distraction apparatus. The pre-extension procedure is followed by open or closed reposition. The surgeon's movements are not hindered. Sterile draping of the operating field can easily be performed in contrast to other methods of reposition like hanging up the forearm with a counter-weight or olecranon-metacarpal extension with counter-traction. Up to 10 months after volar dislocation…

Lack of experience is a significant factor in the missed diagnosis of perilunate fracture dislocation or isolated dislocation.

OBJECTIVE: The aim of this study was to analyze the risk factors contributing to the misdiagnosis of perilunate injuries. METHODS: The study included patients who had perilunate dislocation or fracture dislocation correctly or incorrectly diagnosed on initial examination between 2008 and 2014. Data related to the length of time until correct diagnosis of the perilunate injury; cause of injury; presence of associated fractures, polytrauma or concomitant trauma in the ipsilateral upper extremity; time between injury and first presentation; first treatment applied; presence of ligamentous perilu…

An unusual variety of simultaneous fracture dislocation pattern: medial swivel dislocation of talonavicular joint with displaced fractures of the fourth and fifth metatarsals.

In published studies, a very rare, special type of Chopart dislocation termed a swivel dislocation has been reported. This injury is characterized by dislocation of the talonavicular joint, but the calcaneocuboid joint remains intact. The foot creates a typical rotational movement without inversion or eversion. The axis of rotation is the interosseous talocalcaneal ligament, which remains intact. We report the case of an 18-year-old male who had experienced a medial swivel dislocation of the talonavicular joint associated with displaced fractures of the fourth and fifth metatarsals. The occurr

Surgical management and functional outcomes of perilunate dislocations and fracture dislocations through the dorsal approach.

BACKGROUND: Perilunate dislocations and perilunate fracture dislocations (PLD/PLFDs) are rare injuries of the wrist, with surgical management leading to acceptable functional results. PURPOSE: The purpose of this study was to assess the functional outcomes of the patients of our department who were treated with surgical management of PLDs/PLFDs through dorsal approach, as well as to report any complications on their follow-up. PATIENTS AND METHODS: In this retrospective study, 52 patients with PLD/PLFD, fulfilling the inclusion and exclusion criteria of the study, underwent surgical manageme…

Posterior sternoclavicular joint dislocation with thoracic costovertebral joints fracture-dislocations: A case report.

CASE: A 59-year-old man presented with posterior sternoclavicular joint dislocation concomitant with fracture-dislocations of multiple thoracic costovertebral joints caused by traumatic injury. The posterior sternoclavicular joint dislocation was treated using an ultra-high molecular weight polyethylene fiber cable and the joint was stabilized. The degree of malpositioning of the thoracic costovertebral joints was difficult to reduce. CONCLUSION: The patient achieved an excellent shoulder range of motion at 12 months postoperatively; however, chronic shoulder stiffness and posterior neck disc
