Peripheral Nerve Schwannoma With Intense 68 Ga PSMA Uptake.

Prostate-specific membrane antigen (PSMA) is a specific marker of the prostate cell lineage. 68 Ga-PSMA PET/CT is a sensitive and valuable imaging modality used for staging at diagnosis and restaging in cases of recurrence during the follow-up of prostate carcinoma patients. However, increased PSMA uptake can be observed in various malignancies, such as cholangiocarcinoma, hepatocellular carcinoma, adenoid cystic carcinoma, renal cell carcinoma, and in several benign conditions, such as angiolipoma. Here, we present a case of a newly diagnosed prostate carcinoma patient with a schwannoma exhib

Bisphosphonate Bone Imaging Agent [68Ga]Ga-P15-041 for Evaluating Skeletal Metastases in Prostate Cancer: A Comparison With [68Ga]Ga-PSMA-11 PET/CT.

BACKGROUND: Accurate diagnosis of bone metastases in prostate cancer is essential for staging, prognosis, and treatment. Although PSMA PET/CT is highly effective, complementary imaging is needed to clarify indeterminate lesions. The novel bisphosphonate-based agent [68Ga]Ga-P15-041 shows superior diagnostic accuracy over conventional SPECT imaging, indicating its potential as an auxiliary diagnostic tool. This study explores its role in detecting and assessing prostate cancer bone metastases. PATIENTS AND METHODS: This prospective study enrolled 35 patients with prostate cancer and skeletal m

[18F]FDG and [68Ga]Ga-PSMA dual-tracer total-body PET/CT and PET/MR in patients with prostate cancer.

Prostate cancer (PCa) is a heterogeneous disease and prevalent malignancy in men, necessitating accurate imaging techniques to facilitate effective diagnosis and management. Recent evidence has demonstrated that [18F]fluorodeoxyglucose ([18F]FDG) and prostate-specific membrane antigen (PSMA)-targeted PET tracers positron emission tomography/computed tomography (PET/CT) imaging can provide complementary biological information, thereby improving diagnostic accuracy and the assessment of lesion heterogeneity in patients with PCa. However, optimal protocols for PSMA/FDG dual-tracer PET/CT and PET/

Volumetric Changes and Acute Toxicity With 68Ga Prostate-Specific Membrane Antigen Versus 18F-Fluciclovine Positron Emission Tomography/Computer Tomography Guided Postprostatectomy Radiation: Final Analysis of a Randomized Trial.

PURPOSE: We evaluated changes in radiation therapy target volume and acute toxicity using 68Ga-prostate specific membrane antigen (PSMA) versus 18F-fluciclovine positron emission tomography (PET)/computed tomography in postprostatectomy patients with biochemical recurrence. We hypothesized that both fluciclovine and PSMA-guided radiation therapy would (1) significantly change pre-PET radiation therapy volumes and (2) show similar toxicity. METHODS AND MATERIALS: We performed an institutional review board-approved, randomized trial comparing fluciclovine (Arm 1) and PSMA (Arm 2)-guided postpro

A pictorial view on false positive findings of 68Ga-PSMA-11 PET/CT and their prognostic value in patients with prostate carcinoma after radical prostatectomy and undetectable PSA values.

OBJECTIVE: Recently, gallium-68-prostate-specific membrane antigen-11 (68Ga-PSMA-11) positron emission tomography/computed tomography (PET/CT) has become a key imaging method in prostate carcinoma staging and biochemical progression, with varying sensitivities in different studies (from 40% to 80%). After four years of experience with 68Ga-PSMA-11 PET/CT, we found that it is possible to detect lesions with increased PSMA expression in patients with undetectable prostate specific antigen (PSA) levels after radical prostatectomy. The key questions we wanted to answer were as follows: if those le

Schwannoma Showing Avid Uptake on 68Ga-PSMA-HBED-CC PET/CT.

Ga prostate-specific membrane antigen (PSMA) PET/CT is a relatively new and highly sensitive imaging modality used in staging metastatic prostate cancer. We report a case of a 65-year-old man with newly diagnosed prostate carcinoma who had a PSMA PET/CT scan for staging of his disease. A PSMA-avid right pelvic mass was identified anterior to the sacrum. Surgical removal and histopathological examination of this lesion revealed the diagnosis of schwannoma. It is important to be aware that schwannoma may also show avid uptake on PSMA PET/CT scan and may potentially lead to an incorrect diagnosis

Optimization of 68Ga-DOTA radiolabeling conditions for disulfide-directed multicyclic peptides and amphiphilic antimicrobial peptides.

Gallium-68 labeled peptide radiotracers have attracted significant attention in positron emission tomography (PET) imaging of various diseases. While 68Ga-DOTA radiolabeling protocols are well-established, two classes of peptides, disulfide-directed multicyclic peptides (DDMPs) and amphiphilic antimicrobial peptides (AAMPs), presented challenges in achieving satisfactory radiolabeling efficiency using existing methods. This study therefore aimed to establish optimized 68Ga-DOTA radiolabeling protocols specifically tailored to these peptides, in order to achieve high radiochemical purity (RCP)

Development and Preclinical Evaluation of the Novel Peptide-Based Tracer 68Ga-PDT01 for Noninvasive PET Imaging of PD-L1 Expression.

Programmed death-ligand 1 (PD-L1) is a key mediator of tumor immune evasion and plays a critical role in tumor immunodiagnosis and immunotherapy. We developed a PD-L1-targeted peptide PET tracer, 68Ga-PDT01, incorporating a 2-substituted naphthyl group and 5-(trifluoromethyl)nicotinic acid to enhance tumor uptake and retention. 68Ga-PDT01 exhibited moderate binding affinity (KD = 304 ± 26 nM) and specific uptake in PD-L1-positive cells, which was blocked by excess unlabeled peptide. PET imaging demonstrated clear tumor visualization in multiple subcutaneous tumor models with tumor-to-muscle ra

Metachronous Ectopic CRH-Secreting Tumors Detected by 68 Ga-Pentixafor PET/CT.

A 39-year-old woman with ACTH-dependent Cushing syndrome underwent pituitary surgery after bilateral inferior petrosal sinus sampling (BIPSS) suggested a central source; however, symptoms persisted, and pathology revealed an ACTH-negative adenoma, raising suspicion for ectopic corticotropin-releasing hormone (CRH) secretion. Subsequent 68 Ga-DOTATATE PET/CT was negative. CXCR4-targeted 68 Ga-Pentixafor PET/CT demonstrated focal intense uptake in a right neck lesion, confirmed as a CRH-secreting tumor by histopathology. Two years later, recurrence revealed a second anatomically distinct lesion

Secondary Pulmonary Hypertrophic Osteoarthropathy Detected on [ 68 Ga]Ga-FAPI-46 PET/CT.

A 55-year-old man presented with arthralgia, digital clubbing, and pitting edema. A suspicious pulmonary mass showed high tracer uptake on both [ 18 F]FDG and [ 68 Ga]Ga-FAPI-46 PET/CT. Remarkably, FAPI PET revealed a diffuse periosteal tracer uptake of the long bones, highly suggestive for hypertrophic osteoarthropathy. The secondary and paraneoplastic nature of this condition was confirmed after a biopsy of the pulmonary mass. Hypertrophic osteoarthropathy is characterized by the clinical symptoms of arthralgia, digital clubbing, and periostosis, and is associated with malignancy in 90% of c

PSMA PET/CT in the diagnosis of prostate cancer: why and when?

Prostate-specific membrane antigen (PSMA) is expressed in most primitive and metastatic prostate cancer (PCa), and PSMA inhibitors conjugated with the radionuclides Gallium 68 (68Ga) and fluoride 18 (18F) have been evaluated to detect PCa; moreover, tumour uptake, which represents PSMA expression, is highly correlated with the aggressiveness of the primary prostatic tumour. PSMA positron emission tomography/computed tomography (PET/CT) demonstrated to be sensitive for the detection of primary prostatic lesions, regional lymphadenopathy and clinical metastases in case of biochemical recurrence.
