New approaches to endoscopic therapy for hemostasis of upper GI bleed.

Hematemesis and/or melena are the most common presentations of upper gastrointestinal bleeding. Endoscopic hemostasis after diagnosis still remains a challenge to the gastroenterologist. Most variceal and peptic ulcer bleeding episodes stop spontaneously, and then prevention of rebleeding might be indicated by endoscopic techniques. Four not so well known endoscopic hemostasis techniques are discussed and comparative studies are reviewed.

Understanding the learning curve of intestinal ultrasound in inflammatory bowel disease: A comparative study between novice, regular and expert.

BACKGROUND: Intestinal ultrasound (IUS) has emerged as a valuable tool for monitoring inflammatory bowel disease (IBD), but its reliability depends on the technical skills of the operator. AIMS: This study aimed to assess the IUS learning curve of a novice briefly trained to get him familiarized with the ultrasound machine and normal/abnormal IUS images, and a gastroenterologist with formal general ultrasound training and regular IUS experience (> 200 exams), using an expert radiologist as the gold standard. METHODS: The ULTRA-IBD study was a single-center, prospective study involving 50 con

An Approach to Globus Sensation: Clinical Pearls for Clinicians.

Globus sensation is a common presenting symptom encountered by primary care physicians, otolaryngologists, and gastroenterologists. Although common, it can have a significant impact on quality of life and is often difficult to treat, given lack of a standardized approach to diagnosis and management. In this concise review, we outline the various causes that may contribute to globus sensation, encompassing sinonasal processes such as post-nasal drip, esophageal conditions including laryngopharyngeal and gastroesophageal reflux, and psychological factors as well as structural abnormalities, amon…

Surgical pathology of Hirschsprung disease (HSCR).

The practice, art and science of surgical pathology for disease entities evolves continuously. Standards for diagnosis and management of Hirschsprung disease (HSCR), variants and related dysmotility disorders are no exception. Morphologic parameters that withstand the test of time still fulfill aims for 'personalized' and precision medicine. The expert management of these patients relies on critical points of interaction between pathologists, gastroenterologists and pediatric surgeons for: (1) diagnosis or exclusion within the HSCR spectrum, (2) intraoperative determination of the extent of ag…

What is the best primary therapy for achalasia: medical or surgical treatment? Who owns achalasia?

This article reviews the medical and surgical modalities for the treatment of esophageal achalasia. It compares and critically analyzes the short- and long-term outcomes of pneumatic dilatation and laparoscopic Heller myotomy. Today, laparoscopic Heller myotomy is considered in most centers to be the procedure of choice for the treatment of achalasia. Nevertheless, the success of either procedure is based on the careful selection of the patients and on the surgeon's and gastroenterologist's level of expertise.

The History of Minimally Invasive Surgery.

The foundation of minimally invasive surgery (MIS) lies in the evolution of surgical endoscopy. Endoscopic and MIS techniques often replace open procedures and are associated with decreased morbidity and faster patient recovery. As surgeons have rapidly adopted laparoscopic and robotic platforms, surgical endoscopy remains mainly limited to specialists practicing MIS and gastroenterologists. This review explores the origins of MIS, its current uses across different subspecialties, and its impact on the future of surgical practice.

An Innovative and Alternative Technique in Total Colonic Hirschsprung Disease (TCHD) Treatment, Two Case Reports.

With reported occurrences ranging from 1% to 20%, total colonic Hirschsprung disease (TCHD) accounts for about 8% of Hirschsprung disease cases. The selection and sequencing of the surgical approach, access to parenteral nutrition, and collaborative care between surgeons, gastroenterologists, and pathologists are all critical factors in achieving optimal long-term outcomes. Currently, there is no consensus on the best way to treat TCHD patients. Despite the accompanying morbidity and mortality, standardization of care is required.

Endoscopic findings of Bouveret syndrome.

Bouveret syndrome is a rare complication of gallstone disease. We present a 76-year old male patient in whom the diagnosis was complicated by the reporting of coffee-ground emesis suggesting upper gastrointestinal bleeding. Considering the diagnosis of Bouveret syndrome is commonly made through cross-sectional imaging, these patients rarely undergo diagnostic endoscopy. However, this case presented with typical findings during emergency esophagogastroduodenoscopy (EGD) underscoring its educational value for gastroenterologists and endoscopists.

[Esophago-gastric complications after ablation in atrial fibrillation].

Atrial-esophageal fistula (AOF) after ablation in atrial fibrillation has a fatal outcome. Fully documented in 2004, AOF has never completely disappeared despite technological evolvements. Survival depends on early recognition and treatment. Gastroscopy allows early detection of esophageal lesions. We summarize here AOF mechanisms, evoking symptoms and treatment ; contribution of gastroscopy in early detection ; and finally symptoms from vagal and phrenic nerve injuries. This information is useful for emergency doctors, gastroenterologists / endoscopists, and general practitioners.

Splenic hematoma. A rare complication of colonoscopy.

An extremely rare yet potentially fatal complication of colonoscopy is reported. A 52-year-old female developed a splenic subcapsular hematoma following routine colonoscopy. Conservative treatment was successful. In the English literature, only 14 similar cases have been reported. Treatment of a splenic flexure lesion, previous surgery with splenocolic adhesions, and inflammatory bowel disease increase the risk of such a complication. Increased awareness by surgeons and gastroenterologists should lead to prompt treatment and favorable outcome.

Radiologists should read CT colonography.

This article defines the necessary skill set and knowledge base required for accurate computed tomography colonography (CTC) interpretation. The components of the interpretative process as well as the various strategies currently employed are discussed. The role of extracolonic evaluation as an integral part of this examination is also covered. Within this context, the question of whether a radiologist or gastroenterologist is better suited to interpret this examination is explored.
