BACKGROUND: Corrosive gastric injuries are not uncommon in developing countries because acids, which are more frequently associated with gas … prohibits major resection or where the stricture extends to the antrum the best treatment is a loop gastroenterostomy. Type III, IV, V strictures require individualized treatment. Delayed gastric outlet obstruction affects the treatment plan of combined gastric and esophageal injuries.

INTRODUCTION: The efficacy of Billroth II with Braun (BIIB) anastomosis in laparoscopic distal gastrectomy remains uncertain. We aimed to perform a systematic review and meta-analysis comparing BIIB with Roux en Y (RY) for distal laparoscopic gastrectomy.
METHODS: We systematically searched PubMed, Embase, and Cochrane for studies comparing BIIB versus RY in adult patients undergoing distal gastrectomy. We computed risk ratios (RRs) for binary outcomes and mean differences (MDs) for continuous outcomes, with 95% confidence intervals (CIs). Heterogeneity was assessed with I2 statistics. Statis

INTRODUCTION: Laparoscopic distal gastrectomy (LDG) is the standard treatment for gastric cancer, during which the laparoscopic Billroth I anastomosis technique, also known as the delta-shaped anastomosis (DA), is widely performed. This study aimed to evaluate the differences in short- and long-term symptoms between DA and traditional circular anastomosis (CA) techniques.
METHODS: We retrospectively compared DA and CA procedures in patients undergoing LDG and subsequent reconstruction using the Billroth I method at the Kanagawa Cancer Center from 2017 to 2022.
RESULTS: Other than blood loss,

AIM: Currently, there is no optimal digestive tract reconstruction technique well recognized by scholars after distal gastrectomy. A new reconstruction method, which was modified from the classic Roux-en-Y procedure, the continuous jejunal pouch and residual stomach anastomosis combined with jejunal lateral anastomosis (Contin-L), was established. In order to fully clarify the superiority of this procedure, this study has conducted a systematic analysis and prepared a summary of the clinical data of patients who underwent distal gastrectomy for gastric cancer.
METHODS: From June 2013 to March

BACKGROUND: There is no consensus regarding which reconstruction methods are superior after laparoscopic distal gastrectomy (LDG). This study compared four reconstruction methods after LDG for gastric cancer.
METHODS: Literature in EMBASE, PubMed, and the Cochrane Library was screened to compare Billroth I (B-I), Billroth II (B-II), Roux-en-Y (RY), and uncut Roux-en-Y (URY) anastomoses after LDG for gastric cancer. A Bayesian network meta-analysis (NMA) was conducted to compare these methods.
RESULTS: Eighteen studies involving 4347 patients were eligible for our NMA. The operative time in R

Treatment of complicated giant duodenal ulcers is one of the most difficult problems in abdominal surgery. Simultaneous course of diabetes mellitus and giant duodenal ulcer has clinical features and requires correction of the treatment. It is presented case report that may be useful regarding accumulation of knowledge about comorbid course of these pathologies. The patient 58-year old was urgently delivered with signs of gastrointestinal bleeding, ulcerative anamnesis for 15 years, diabetes mellitus type 2 for 8 years. Gastroduodenoscopy revealed chronic giant (2.5×3.5 cm) duodenal ulcer compl

In Japan, the Billroth I and Billroth II operations have been used for reconstruction after a distal gastrectomy for gastric cancer. However, a Roux-en-Y reconstruction is increasingly performed to prevent duodenogastric reflux. We herein discuss the indications for Roux-en-Y in gastric surgery and review the literature to determine its advantages and disadvantages. Indications for Roux-en-Y reconstruction after a distal gastrectomy are: (a) When the primary lesion has directly invaded the duodenum or head of the pancreas, the Billroth I operation is likely to result in local recurrence near t

UNLABELLED: Duodenal localization of Crohn's disease is rare, accounting for only 0.5-4% of all cases. Most common complaints are gastric outlet obstruction and weight loss. Histologic findings of endoscopic biopsy are frequently not definitive, making differentiation from other, benign structures complicated. There are also no standard guidelines regarding indications for surgical management.
PATIENTS AND METHODS: We reviewed the cases of three surgically managed patients with duodenal Crohn's disease at the 1st Department of Surgery, Semmelweis University of Medicine, Budapest, during a 5-y

A 19-year-old man who developed extensive oesophageal lye (Alkali) stricture and received long-term enteral nutrition (eight months) with a jejunostomy tube developed macrocytic anaemia (Hb: 41 g/L) with leucopenia (white blood cell [WBC]: 3.0 x 10(9)/L). The patient's serum vitamin B12, folate, iron and liver function tests were normal. Bone marrow examination revealed gross erythroid hyperplasia and cytoplasmic vacuolization of erythroid and myeloid elements. Further investigations revealed low serum copper (0.3 micromol/L) and ceruloplasmin concentrations (<30 mg/L) with marginally low norm

Our study presents a number of 5219 gastroduodenal ulcers that have been operated during 30 years (1970-1999), from which 703 (13%) hemorrhagic ulcers. The incidence of ulcer bleedings pointed out the necessity to select patients in groups: with gastric (222-31.5% cases), duodenal (341-8.5%) and postbulbar (140-19.9%) bleedings. The medical treatment (H2 blockators, proton pump inhibitors), endoscopic procedures and substitute infusions that can solve almost all ulcer bleedings. In the first period: (1970-1985) 682 patients with bleedings have been admitted, immediate operations have been perf

A 63-year-old woman with Wernicke-Korsakoff syndrome, despite the absence of alcoholism and malnutrition, is reported. She had undergone gastrojejunostomy for ileus 30 years ago, and this operation was thought to be an important risk factor for her vitamin B1 deficiency. Brain MRI showed symmetrical high intensity areas on T2-weighted images in the periaqueductal region and bilateral dorsomedial nuclei of the thalamus. On single photon emission computed tomography (SPECT) using 99mTc-hexamethylpropyleneamine oxime, bilateral frontal perfusion was reduced, which was attributed to thalamo-cortic

The effects of gastroenteroanastomosis, antral exclusion and antral resection on the enterochromaffin (argentaffin) cell system in the rat alimentary tract were studied 6 weeks after the operation. When measured as cell density per mm3 mucosa, the number of enterochromaffin cells was decreased in most gastrointestinal regions following the three surgical procedures. In the antral exclusion group this decrease was most apparent in the caecum and large intestine, and in the animals with antral resection it was most pronounced in the proximal and middle regions of the small intestine. When expres

Truncal vagotomy was employed in 120 cases of simple or complicated duodenal ulcer. Pyloroplasty was also performed in 98 cases, antroduodenostomy in 5, Billroth I or II in 12, and gastrojejunostomy in 5. No operative deaths were noted, even though the series included emergency situations and patients in poor condition. The incidence of complications and immediate sequelae was insignificant. Follow-up for 2-6 yr showed complete success in over 91%, with partial improvement in 3.8%. Poor results included 3 cases of recurrence, 1 of late scar stenosis on the pyloroplasty site, and 1 of intermitt

A case of massive haemorrhage into the gastrointestinal tract from a pancreatic pseudocyst is presented. The pseudocyst was a complication of acute pancreatitis, and control of the haemorrhage by operation was difficult because of the friability of the cyst wall. The surgical procedure called duodenal diverticulization is described and was found to be a useful adjunct to suture ligation for bleeding which was almost uncontrollable and when death of the patient seemed inevitable.
