Food allergy and gastrointestinal diseases.

Several gastrointestinal diseases and symptoms have been attributed to food intolerance, but in only a few cases has the participation of immune mechanisms been confirmed. Acute or chronic gastroenteritis is commonly due to allergic reactions to food proteins, inflammatory diseases and infections, or the presence of immunological defects may induce secondarily a food allergic disorder. This review will focus on the immunopathogenetical importance of the mucosal and systemic defenses, and the absorption of food proteins by the gut.

Ageing-driven gastrointestinal variability in Parkinson's disease: implications for oral levodopa pharmacokinetics and formulation design.

Parkinson's disease is a distinctly age-associated neurodegenerative disorder in which oral levodopa remains the therapeutic foundation, particularly in older adults. Yet with advancing age, the reliability of oral therapy progressively declines not simply due to inadequate dosing, but because ageing reshapes the gastrointestinal environment on which drug absorption depends. This review integrates evidence spanning neuromuscular decline, epithelial barrier fragility, altered luminal chemistry, immune dysregulation, microbiome remodelling, and enteric neurodegeneration to explain how the ageing

Clarithromycin extended-release in community-acquired respiratory tract infections.

Clarithromycin extended-release (ER) is a second-generation macrolide with bactericidal activity against a broad group of pathogens. It allows for convenient once-daily dosing (2 x 500 mg/day), and has less severe adverse effects than the immediate-release (IR) formulation of the drug, which may result in improved patient compliance. Clarithromycin ER has been approved for the treatment of community-acquired pneumonia, acute maxillary sinusitis and acute bacterial exacerbation of chronic bronchitis. In comparison to the IR formulation, clarithromycin ER demonstrates prolonged absorption from t

Intestinal absorption of oestrone, oestrone glucuronide and oestrone sulphate in the rat in situ--I. Importance of hydrolytic enzymes on conjugate absorption.

The biliary excretion of steroid after administration of [3H]oestrone ([3H]E1), [3H]oestrone glucuronide ([3H]E1G) and [3H]oestrone sulphate ([3H]E1S) into the hepatic portal vein of anaesthetized rats was very rapid with more than 70% of E1S and greater than 80% of E1 and E1G excreted in the first 30 min. There was a lag period in the biliary excretion of E1S, this was less apparent with E1 and absent with E1G. Biliary excretion accurately reflects the amount of steroid in the portal circulation and was therefore used as an assessment of absorption from the gastrointestinal (GI) tract. Absorp

Reduced venesection requirement as a clue to occult colorectal cancer in hereditary haemochromatosis.

Colorectal cancer (CRC) is a leading cause of cancer-related mortality worldwide. We report a case of CRC in a man in his late 70s with hereditary haemochromatosis (HH) who historically required venesection two to three times per year to maintain target ferritin levels. Despite the absence of gastrointestinal symptoms or anaemia, his requirement for maintenance venesection unexpectedly ceased and ferritin levels remained low without treatment. Two faecal immunochemical tests were negative. Colonoscopy subsequently revealed an adenocarcinoma in the caecum. Chronic occult blood loss from the tum

Egg Chalaza Improves Calcium Utilization in Growing Rats: Evidence from Intestinal Aspects.

Chalaza (CLZ), a byproduct generated during egg processing, has shown potential to promote calcium absorption, but its underlying mechanisms remain unclear. In a low-calcium growing rat model, coadministration of CLZ with CaCO3 dose-dependently increased apparent calcium absorption and retention, enhanced bone calcium deposition, and improved bone microarchitecture and mechanical properties compared with CaCO3 alone. These effects were accompanied by improved duodenal morphology, upregulation of intestinal calcium transport-related proteins (TRPV6, PMCA1b, and S100G/CaBP-D9k), and associated g

Vitamin D: recent advances, associated factors, and its role in combating non-communicable diseases.

The field of nutrigenomics has produced numerous studies indicating the impact of vitamin D on various disease conditions. Trace elements of this vitamin in the body play a significant role in the regulation of body metabolism. This immunomodulatory vitamin plays a role in management of both communicable (viz. respiratory illness like COVID-19 and Respiratory tract infections) and non-communicable diseases e.g., cancer, osteomalacia, diabetes, and cardiovascular diseases. Deficient levels, i.e., vitamin D deficiency in body can lead to the onset of chronic non-communicable illnesses. Vitamin D

Oral fructose tolerance, gastric emptying and absorption: a compartmental model.

Gastric emptying and intestinal absorption regulate transfer of ingested fructose from stomach to plasma. Using conscious rhesus monkeys we have developed a compartmental model which describes this system. Fructose tolerance tests were performed in groups of monkeys by intragastrically administering 2 g/kg of 10.5% D-fructose solution; fructose concentration in arterial plasma, [fructose], and intragastric volume were measured at intervals afterward. One group was pretreated with atropine; stomachs imptied with a time constant, tau, of 67 min. Another group received fructose solution with tris

Intestinal calcium transport in mole-rats (Cryptomys damarensis and Heterocephalus glaber) is independent of both genomic and non-genomic vitamin D mediation.

The role of vitamin D in mineral uptake in the gastrointestinal tract (GIT) of mole-rats (Heterocephalus glaber and Cryptomys damarensis; family Bathyergidae), animals with a naturally impoverished vitamin D status, was investigated. We measured relative rates of passage of radioactive markers, mode of calcium (Ca) uptake, paracellular movement and the opening of voltage-sensitive Ca channels (VSCCs) along the GIT with and without oral vitamin D3 supplementation. The ratio of relative absorption of labelled 45Ca to [14C]polyethylene glycol ([14C]PEG) indicated that within 24 h more than 88% of

The battle for iron in enteric infections.

Iron is an essential element for almost all living organisms, but can be extremely toxic in high concentrations. All organisms must therefore employ homeostatic mechanisms to finely regulate iron uptake, usage and storage in the face of dynamic environmental conditions. The critical step in mammalian systemic iron homeostasis is the fine regulation of dietary iron absorption. However, as the gastrointestinal system is also home to >1014 bacteria, all of which engage in their own programmes of iron homeostasis, the gut represents an anatomical location where the inter-kingdom fight for iron is

Autointoxication revisited.

The pustular dermatitis associated with small bowel bypass surgery and the cutaneous manifestations of inflammatory bowel disease are well known and generally assumed to be due to the absorption of microbial antigens from the bowel. Monomeric serum IgA is assumed to originate in the gastrointestinal tract, and circulating IgA immune complexes, as seen in dermatitis herpetiformis, should make us suspicious of a gastrointestinal tract source. These circulating immune complexes and perhaps polyclonal increases in serum IgA may be the result of minor perturbations of mucosal permeability or the fa

Enhanced oral insulin delivery with charge-reversible lipid nanoparticles.

Oral insulin has therapeutic advantages, as it can mimic the endogenous insulin pathway and relieve patients from daily self-injections. Among the many investigated oral insulin delivery systems, lipid nanoparticle (LNP)-based drug delivery systems are considered promising platforms for improving oral insulin absorption due to their unique in vivo properties and high design flexibility. However, challenges such as toxicity and low oral bioavailability persist. Dioleoylglycerophosphate-diethylenediamine (DOP-DEDA) is a pH-responsive and charge-reversible lipid for cytosolic cargo delivery. In t

Effects of Coffee on Gut Microbiota and Bowel Functions in Health and Diseases: A Literature Review.

Background and objectives: As one of the most popular beverages in the world, coffee has long been known to affect bowel functions such as motility, secretion, and absorption. Recent evidence obtained in human and animal studies suggests that coffee has modulating impacts on gut microbiota. We aim to present an overview of the specific effects of coffee on gut microbiota composition, diversity, and growth. We will also critically review the impacts of coffee on bowel functions in health and diseases and discuss whether gut microbiota play a role in the coffee-associated functional changes in t

Clinical implications of gastric complications on levodopa treatment in Parkinson's disease.

Disorders of the gastrointestinal (GI) tract are common and distressing nonmotor symptoms of Parkinson's disease (PD) that can adversely affect levodopa absorption and lead to OFF periods, also known as motor fluctuations. Gastroparesis, which is primarily defined as delayed gastric emptying (DGE), and Helicobacter pylori infection, which is present with increased frequency in PD, are among the most common and important GI disorders reported in PD that may impair oral levodopa absorption and increase OFF time. Symptoms of gastroparesis include nausea, vomiting, postprandial bloating, fullness,

Enhanced oral absorption of ibandronate via complex formation with bile acid derivative.

Bisphosphonates are recommended for the treatment of postmenopausal osteoporosis, Paget's disease, bone metastasis, and multiple myeloma. However, the efficacy of oral preparations is limited because of their low bioavailabilities and adverse effects from the gastrointestinal tract. This study was conducted to investigate whether N(α)-deoxycholyl-L-lysyl-methylester (DCK), an absorption enhancer derived from deoxycholic acid, can increase the oral bioavailability of ibandronate. We prepared a physical complex of ibandronate with DCK, and evaluated its permeability across a parallel artificial

Interaction of dietary carbohydrate, ascorbic acid and copper with the development of copper deficiency in rats.

The effects of dietary carbohydrate and ascorbic acid on the development of copper deficiency were investigated. Male Sprague-Dawley rats (n = 48) were fed one of eight diets in a 2 X 2 X 2 factorial design for 21 d. These diets varied in copper (1.11 or 8.96 micrograms Cu/g diet), carbohydrate (sucrose or cornstarch, 62.3%) and ascorbic acid (0 or 1%). Compared to controls, copper-deficient rats had lower hematocrit and ceruloplasmin levels, lower levels of copper and iron in several tissues, higher heart weights and lower spleen weights. During copper deficiency, liver iron levels were highe

Nutritional Considerations in Neonates Requiring Gastrointestinal Surgery.

Neonates who require gastrointestinal surgery are a complex group of patients that require special consideration with regard to nutritional supplementation and growth. During the acute postoperative phase, a major stress response causes catabolism with degradation of the body's nutrient stores leading to poor growth. Following surgery, parenteral nutrition is often required to support the surgical neonate; although, enteral nutrition, if feasible, is critical because it helps improve intestinal adaptation. However, the best type, mode, and duration of feeding is not established in the current

[Long-term animal experiment analysis of the use of resorbable staple sutures in partial gastrectomy].

In animal experiments with a 12-month follow-up, the safety and compatibility of absorbable staple lines following gastrointestinal surgery were analysed. In two groups of pigs, partial gastrectomies with gastroduodenostomy (Billroth I, n = 10) or gastrojejunostomy (Roux-en-Y, n = 12) were performed, and Lactomer-a copolymer of glycolic acid and lactic acid-staple lines were used for closure of the lesser curvature of the stomach and the duodenal stump. Application of the staple lines caused no problems and was safe. In 1 of the 22 pigs, which died of an ileus 9 days postoperatively a suture d
