BACKGROUND: Equine gastric ulcer syndrome (EGUS) is common in adult horses, particularly those involved in performance disciplines. We describe the case of a 78-year-old woman admitted to our department for suspected silent myocardial ischaemia with the evidence of T wave inversion in anterior lead. All the instrumental exams excluded inducible myocardial ischaemia. A gastroscopy showed a moderate hiatal hernia. We postulate that electrocardiogram modification could be attributed to hiatal hernia.

RATIONALE: Pyloric gland adenoma (PGA) is often associated with pyloric gland metaplasia. It has high malignant potential but a low clinical diagnosis rate. Therefore, we reported a case of PGA and reviewed the literature to summarize the clinicopathological features of pyloric adenoma.

Antral web is a rare cause of gastric-outlet obstruction. We report a case of gastric antral web with intermittent vomiting in a 10-year-old girl who received medical treatment for 6 months. The literature on this anomaly is reviewed with respect to differential diagnosis on the basis of upper gastrointestinal radiographic series, gastroscopy, and treatment planning.

During gastroscopy on a 75-year-old man because of bleeding from the upper gastro-intestinal tract, a white wallpaper-like covering was noted along the whole length of the oesophagus. Cytology of smears demonstrated typical cellular structure of Herpes, characterized by multinuclear cells with pale, flattened opaque glass-like nuclei. Subsequently the patient died of other organic complications.

A prospective study was undertaken to investigate the frequency of bacteremia after endoscopy of the gastrointestinal tract. In one out of 100 patients undergoing gastroscopy and in one out of 25 patients undergoing coloscopy, positive blood cultures could be demonstrated. In both cases anaerobe lactobacilli were recovered. No bacteremia was demonstrated in 25 patients undergoing endoscopic retrograde cholangiopancreatography (ERCP).

We present a case of a 62 year old woman with history of liver cirrhosis secondary to autoimmune hepatitis, with portal hypertension and coagulopathy. Gastroscopy findings were a polypoid and polylobed lesions in the gastric antrum. These were removed and the pathological study described hyperplastic polyps with edema, vascular congestion and hyperplasia of smooth muscle, corresponding to "portal hypertensive polyps" (PHP).

It is reported on the endoscopic proof of a double pylorus in a 57-year-old patient. The anamnesis of ulcer of many years and a florid prepyloric gastric ulcer refer to the ulcerous origin of this anomaly. It is referred to the diagnostic importance of the gastroscopy of the acquired or congenital duplication of the pylorus channel which is difficult to be represented radiologically with regard to the avoidance of possible diagnostic errors (e.g. carcinoma of the antrum).

Three patients are presented with double pylori due to antrobulbary fistulas after penetration of ulcers through the prepyloric mucosa or the pyloric ring. Two cases were diagnosed radiographically; in one of them the diagnosis was confirmed by gastroscopy and verified by subsequent operation. The third case was revealed at autopsy. The etiology and pathogenesis of fistulas within the gastrointestinal tract are discussed with regard to the current literature.
