Pathways between under/unemployment and health among racialized immigrant women in Toronto.

OBJECTIVE: We sought to document pathways between under/unemployment and health among racialized immigrant women in Toronto while exploring the ways in which gender, class, migration and racialization, as interlocking systems of social relations, structure these relationships. DESIGN: We conducted 30 interviews with racialized immigrant women who were struggling to get stable employment that matched their education and/or experience. Participants were recruited through flyers, partner agencies and peer researcher networks. Most interviews (21) were conducted in a language other than English.

Systematic study and 30-year projections of global and multi-regional burden of multiple sclerosis, 1990-2021.

Multiple sclerosis (MS) is a chronic neurodegenerative disease with significant temporal and regional heterogeneity. While earlier studies described the burden before 2019, recent shifts influenced by socioeconomic development, healthcare access, and environmental exposures remain unclear. Using the Global Burden of Disease database, we analyzed recent MS trends, assessed interactions of gender, age, and sociodemographic index (SDI), and projected future dynamics. Based on the Global Burden of Disease database from 1990 to 2021, this study collected epidemiological data of 2795 patients with M

Ethnicity, socio-economic position and gender--do they affect reported health-care seeking behaviour?

While the pursuit of equity of access to health care is a central objective of many health care systems, there is evidence that patients of ethnic minority descent, in lower socio-economic position (SEP) or of female gender are less likely than Whites, more affluent groups or men, respectively, to access secondary and tertiary medical care. However, it is unclear at which point in the chain of events leading from perception of need through attendance at primary/emergency care, to referral and receipt of secondary care, this inequality occurs. This study examined the influence of ethnicity, soc

Public health nurse advocacy as essential for addressing the health impacts of climate change.

BACKGROUND: Climate change is transforming the environmental, social and structural determinants of health, intensifying inequities across populations and geographies. Although nurses are at the forefront of addressing climate-related health impacts, nurses' influence on climate policy remains significantly underleveraged. Strengthening the leadership and advocacy of nurses is essential for meeting the escalating population health challenges posed by climate change. ANALYSIS: There is limited organized advocacy for climate action by public health nurses. At the 7th Global Network of Public He

Comorbid Cardiovascular-Liver Disease Disparities: Current Understanding and Future Directions.

PURPOSE OF REVIEW: Metabolic dysfunction-associated steatotic liver disease (MASLD) is a prevalent chronic liver disease and a multisystem cardiometabolic disorder. Cardiovascular disease (CVD) is a major cause of morbidity and mortality in this population. This review examines disparities in the recognition and management of coexisting cardiovascular and metabolic liver disease. RECENT FINDINGS: Shared metabolic and inflammatory pathways link liver disease with obesity, type 2 diabetes, hypertension, and CVD. Differences in measured disease prevalence across racial and ethnic groups must be

Revisiting the three delays model: Persistent drivers of obstetric fistula in sub-Saharan Africa during the era of maternal health interventions.

Obstetric fistula remains one of the most preventable yet persistent forms of maternal morbidity in sub-Saharan Africa (SSA), despite major investments in maternal health services. Traditionally associated with prolonged obstructed labor and delayed access to emergency obstetric care, fistula continues to expose failures across the continuum of care. This narrative review critically reassesses the contemporary relevance of the Three Delays Model as an explanatory framework for persistent obstetric fistula in SSA. A structured search of PubMed, Scopus, Web of Science, and Google Scholar identif

FairGuard: Blockchain-Enforced Continuous Fairness Governance for Demographically Equitable LLM-Based Emergency Triage Decision Support.

Demographic bias in AI-driven clinical decision support systems (CDSS) represents one of the most consequential and least-addressed risks in healthcare AI. Large Language Models (LLMs) applied to emergency triage can silently perpetuate or amplify existing health disparities across thousands of patient decisions. This paper introduces FairGuard, a continuous bias detection and governance framework that embeds demographic fairness auditing directly into the LLM inference pipeline using four integrated mechanisms: (1) an equity-enforcing consent gate that applies demographic-blind access control

Exploring the role of social media and online communities in the infertility journey: a scoping review.

OBJECTIVE: Infertility is a global health challenge with profound psychosocial consequences. Couples navigate this journey within social media and online communities. These digital spaces offer opportunities for support, coping and information exchange, but also expose users to comparison culture, misinformation, and inequities. This scoping review aims to synthesize the available evidence on how social-media and online communities shape the psychosocial experience of infertility, identifying both supportive and harmful dimensions. METHODS: Following Joanna-Briggs Institute methodology PRISMA

Sleep inequities at the intersections of racialized, gender, and sexual identity in the U.S.: An intersectional Multilevel Analysis of Individual Heterogeneity and Discriminatory Accuracy (MAIHDA).

OBJECTIVES: Sleep inequities are marked and persistent in the U.S., yet there remains limited research examining the patterning of adverse sleep outcomes across intersecting dimensions of social identity. This study's objective was to estimate the prevalence of short and long sleep duration across population subgroups jointly defined by racialized identity, sex, and sexual identity, and quantify inequities. METHODS: Data came from the 2022 Behavioral Risk Factor Surveillance System (N = 235,326). We implemented Multilevel Analysis of Individual Heterogeneity and Discriminatory Accuracy (MAIHD

Performance of a Brazilian health care network in leprosy care: Early diagnosis, timely treatment, and disability outcomes.

Leprosy remains a neglected and transmissible disease and continues to challenge the World Health Organization elimination goals, mainly due to delays in diagnosis and treatment. This analytical cross-sectional study assessed indicators related to the performance of the Health Care Network (HCN) of Minas Gerais, Brazil, in leprosy care using secondary data from 2014 to 2023. The outcomes evaluated were early diagnosis, timely treatment initiation, and disability outcomes. Data were obtained from the Notifiable Diseases Information System (SINAN) and the National Register of Health Establishmen

Consent Myths and Norm Violation Responses: Reconceptualizing Rape Myth Theory in Sexual Assault Sentencing in China.

Criminal sentencing in sexual assault cases exhibits systematic disparities based on victim-offender relationships, yet existing theoretical explanations remain inadequate for understanding the underlying cognitive mechanisms. This study reconceptualizes rape myth theory by distinguishing between two distinct cognitive structures: consent myths that operate through everyday cultural reinforcement, and norm violation responses that activate through moral boundary violations. Analyzing 7701 Chinese rape case judgments (2012-2020), we demonstrate that these mechanisms produce opposing sentencing

Hard-to-Reach or Hardly Reached? The "Difficulty" of Engaging Cisgender Black Females in Sexual Health Research.

Rather than placing the onus on stigmatized and disenfranchised communities as hard-to-reach in sexual health research, we challenge researchers to recognize and provide outreach to populations who are hardly reached, such as cisgender Black women. We posit that the disparate human immunodeficiency virus (HIV) and sexually transmitted infection (STI) rates experienced by Black women in the USA are due in part to social and structural inequities and lack of researcher outreach within these communities. Social inequities give rise to racial and gender discrimination, which often results in struc

Ethics and the treatment as prevention strategy among transgender women living with HIV in Argentina.

While numerous ethical concerns have been voiced regarding HIV service scale-up strategies targeting key populations, few studies have examined these from the perspective of affected groups. This study therefore sought to understand transgender women's experiences and perspectives of targeted HIV services scale-up in the context of Argentina's Treatment as Prevention strategy. In 2016, 25 purposively selected transgender women living with HIV were interviewed by a peer research associate. Interviews were audio recorded, transcribed verbatim and analysed using participatory coding techniques. F

Typologies of violence against women in Brazil: A latent class analysis of how violence and HIV intersect.

We address the limited understanding around the overlap between violence and HIV in Brazil. Data was from two clinic-based samples of HIV-po … times (CP: 0.54) by an intimate partner (CP: 0.60). Findings inform interventions to educate around gender equity, violence, and the health effects of violence including HIV, integrate HIV and violence services, and improve the provision of bio-medical HIV prevention among HIV-negative women who experience violence.

Sociodemographic variation in the association between modes of access and patient experience of primary care: a retrospective cross-sectional patient-level analysis of the General Practice Patient Survey in England in 2023.

BACKGROUND: Patient experience is a key indicator of healthcare quality. Access to general practice (GP) has shifted with increasing use of digital tools for contacting practices and more remote consultations. These changes may improve patient experience, but unequally across sociodemographic groups. This study examines how patient experience of GP varies by access mode and sociodemographic characteristics. METHODS: Retrospective cross-sectional analysis of 759 149 responses from the 2023 GP Patient Survey in England. Mixed-effects logistic regression was used to examine associations between

Integrating LGBTQIA + Community Member Perspectives into Medical Education.

ProblemLGBTQIA+ (lesbian, gay, bisexual, transgender, queer, intersex, asexual, and all sexual and gender minorities) people have unique health care needs related to their sexual orientation, gender identity and expression, and sex development. However, medical education has historically excluded LGBTQIA + health-related content in formal curricula. It is common for medical students to interact with diverse patient populations through clinical rotations; however, access to and knowledge about LGBTQIA + patients is inconsistently prioritized in medical schools. This is especially true for LGBTQ

Healthcare worker training to improve quality of care for sexual and gender minority people in sub-Saharan Africa: learning from efforts in Uganda.

INTRODUCTION: Training in care for sexual and gender minority (SGM) populations is critical for ending the HIV epidemic. SGM people, particularly men who have sex with men (MSM) and transgender women, experience disproportionate HIV infection across the globe. The objective of this commentary was to synthesize facilitators of and barriers to SGM health training efforts for healthcare workers in Uganda, in order to help inform potential priorities, strategies and next steps to advance culturally responsive HIV-related care for SGM communities across Uganda and sub-Saharan Africa. DISCUSSION: S

PrEP discontinuation among sex workers: Gender disparities and the impact of housing instability in a prospective cohort study.

OBJECTIVES: PrEP discontinuation remains a major challenge in HIV prevention, yet comparative data on retention among sex workers-particularly across gender identities-are scarce. We evaluated the incidence of PrEP discontinuation and associated risk factors in a diverse cohort of sex workers in Madrid, Spain. METHODS: We conducted a prospective observational study of HIV-negative adults engaged in sex work initiating PrEP at a specialized hospital-based clinic between November 2022 and March 2025. We evaluated the incidence of PrEP discontinuation and associated structural and demographic fa

"It's a Big Ordeal": A Mixed Methods Study of the Experiences of non-HIV STI Testing Among Trans and Gender Diverse People.

OBJECTIVES: Transgender and gender diverse (TGD) persons are disproportionately affected by sexually transmitted infection (STI) inequities. Research predominantly focuses on HIV disparities among transfeminine persons, whereas non-HIV STIs (e.g., chlamydia) and transmasculine and nonbinary persons are overlooked. Thus, we examined barriers and facilitators to uptake of non-HIV STI testing among TGD persons, inclusive of transmasculine, transfeminine, and nonbinary persons. METHODS: This community-based explanatory sequential mixed-methods study utilized secondary quantitative data collected
