[Chronic leg ulcers in older patients].

Chronic ulcers of the lower extremities are one of the most common medical problems encountered in western societies. The prevalence of leg ulcers is estimated to be 0.5-1.0% of the German population and is clearly associated with age. Therefore, in an aging society chronic leg ulcers are an important health issue with respect to increased morbidity and healthcare costs. The most frequent causes of leg ulcers are chronic venous insufficiency, peripheral arterial occlusive disease and diabetes mellitus. Efficient treatment necessitates an exact diagnosis and a close interdisciplinary collaborat

Biofeedback training to overcome poststroke foot-drop.

The technique has exciting potential for elderly hemiplegic patients, even those who are confined to a nursing home. There are limiting factors, of course; the method is time-consuming and the initial outlay for equipment is high. But the rewards can be well worth the time, effort, and cost involved. The recovery potential of some geriatric patients after a cerebrovascular accident may seem bleak because of multiple disabilities, e.g., paralysis, delayed reflexes, aphasia. Fortunately, these disorders do not necessarily decree failure of biofeedback training. Neither does advanced age. The str

The utility of the Kohlman Evaluation of Living Skills test is associated with substantiated cases of elder self-neglect.

PURPOSE: Self-neglect is the most prevalent finding among cases reported to Adult Protective Services (APS) and is characterized by an inability to meet one's own basic needs. The Kohlman evaluation of living skills (KELS) has been validated in geriatric populations to assess performance with both instrumental and basic activities of daily living and as an assessment tool for the capacity to live independently; therefore, the purpose of this analysis was to compare the scores of the KELS between substantiated cases of self-neglect and matched community-dwelling elders. DATA SOURCES: This is a

Over the Counter Pain Medications Used by Adults: A Need for Pharmacist Intervention.

The safety of pharmacotherapy for geriatric patients is an essential aspect of the demographic perspective in view of the increasing size of this population. Non-opioid analgesics (NOAs) are among the most popular and often overused over-the-counter medications (OTC). The reasons for drug abuse are common in the geriatric population: musculoskeletal disorders, colds, inflammation and pain of various origins. The popularity of self-medication and the ability to easily access OTC drugs outside the pharmacy creates the danger of their misuse and the incidence of adverse drug reactions (ADRs). The

Senior care pharmacy profile: Grace Lawrence. Innovative services for assisted living, hospice, and the community.

Pharmacists can offer a variety of medication and consulting services to elderly people living in the community and in assisted living facilities. Such services may include reviewing drug therapy regimens to ensure that medications are appropriate, effective, and safe for a specific patient; determining whether a sign, symptom, syndrome, or decline in function is medication-related; and identifying medications that may cause or aggravate common geriatric problems. Pharmacists providing "senior care" services talk with patients and caregivers to pinpoint medication issues, work closely with nur

An international survey of undergraduate trainees' interests and expected teaching strategies in geriatric oncology.

BACKGROUND: Age is a critical driver of the worldwide increasing cancer incidence rates, and with the growing number of older adults diagnosed with cancer, there is a rising need for specialized care. However, geriatric oncology (GO) education remains underrepresented in medical curricula. The gap between demand and adequately trained providers requires the development of more robust GO training programs. This study examined the current state of GO education, and the areas of interest within GO and the expected teaching methods for this specialized content. METHODS: A multi-center internation

There's No Place Like Home: Delirium as a Barrier in Geriatric Trauma.

INTRODUCTION: Delirium is associated with a three-fold increase in frequency of 6-mo mortality following intensive care unit admission. Outside of mortality, it has been linked with severe morbidity including long-term cognitive decline, loss of autonomy, and increased risk of institutionalization. There is a paucity of literature regarding delirium and geriatric trauma patients. The aim of our study is to determine predictive factors of delirium in geriatric trauma patients. METHODS: This is a retrospective review of all geriatric (>65 y) trauma patients with a documented frailty score at a

The Arkansas aging initiative: an innovative approach for addressing the health of older rural Arkansans.

The Donald W. Reynolds Institute on Aging at the University of Arkansas for Medical Sciences in Little Rock is addressing one of the most pressing policy issues facing the United States: how to care for the burgeoning number of older adults. In 2001, the Institute created the Arkansas Aging Initiative, which established seven satellite centers on aging across the state using $1.3 to $2 million dollars annually from the state's portion of the Master Tobacco Settlement. These centers on aging assist the state's population of older adults, many of whom reside in rural areas, live in poverty, and

Wuli-Shili-Renli methodology as a service quality management tool among private older adults care institutions in Taizhou, China: an empirical study.

BACKGROUND: Population aging, a global crisis, has made private integrated medical - nursing facilities the primary providers for older adults care. However, the operational flaws of these facilities can potentially be addressed by the China - originated Wuli-Shili-Renli (WSR) methodology, which integrates physical resources, systematic management, and human factors. METHODS: A retrospective empirical before-after study was conducted to evaluate the implementation effect of the WSR framework on service quality management in one representative private integrated medical-nursing care institutio

Management of skin conditions in the older population 2.

This is the second of two papers covering assessment and management of elderly skin. Part 1 of this clinical review focused on the importance of a comprehensive assessment of the skin and a detailed clinical history of the patient (Bianchi and Cameron, 2008). This second paper considers some common dermatological conditions seen in the older adult and explores suitable management strategies.

[Health status of the elderly over 67 years old in a community, and their need of nursing care. A survey with descriptive analysis. Health status in the light of life style].

The purpose of this descriptive study was to explore the problems of community dwelling persons 67 years and older, in meeting their self-care requisites, and the factors in living arrangements associated with these problems. Community nurses in both administration and practice can use the results in planning curative and preventive interventions for all the elderly or for groups of elderly according to living arrangements. These groups were those living alone, living with a spouse or living with other family members. A standardized questionnaire revised from Dr. Agnes Bjørns interview guide u

Frailty and falls in older adults: risk assessment and management for community nurses.

Frailty and falls are highly prevalent among community-dwelling older adults and are linked through a bidirectional relationship that increases the risk of injury, hospitalisation, functional decline and mortality. Community nurses play a central role in early identification and prevention. This article examines the relationship between frailty and falls, and outlines practical approaches to risk assessment and management in community nursing practice. The authors present a clinically focused synthesis of contemporary evidence and national guidance relating to frailty and falls in community-dw

The Impact of Living Arrangements on the Prevalence of Falls after Total Joint Arthroplasty: A Comparison between Institutionalized and General Geriatric Population.

Due to population aging, there is an increasing need for orthopedic surgery, especially total knee arthroplasty (TKA) and total hip arthroplasty (THA). In geriatric patients, postoperative falls are common events which can compromise the success of these expensive procedures. The aim of our study was to assess the influence of living arrangements on the prevalence of postoperative falls following joint replacement. We included 441 patients after TKA or THA, living in nursing homes, alone or with family. The prevalence of falls in the first 2 years (15.2%) was significantly influenced by living
