The Role of Geriatric Syndromes in Kidney Transplant Decision-Making: A Domain-Based Analysis of Comprehensive Geriatric Assessment in Older Adults.

BACKGROUND: As the population ages, kidney transplantation (KT) is increasingly considered for older adults with advanced chronic kidney disease (CKD). However, frailty, cognitive impairment, and malnutrition complicate transplant decision-making. A Comprehensive Geriatric Assessment (CGA) offers a multidimensional approach, but the relative contribution of individual CGA domains to transplant eligibility and receipt remains unclear. METHODS: We conducted a retrospective observational study of 164 older adults with advanced CKD seeking KT and underwent CGA by a geriatrician at an urban academ

Exploring depression, comorbidities and quality of life in geriatric patients: a study utilizing the geriatric depression scale and WHOQOL-OLD questionnaire.

BACKGROUND: The increasing prevalence of depression among older adults is a growing concern. Chronic health conditions, cognitive impairments, and hospitalizations amplify emotional distress and depression levels in this population. Assessing the quality of life is crucial for the well-being of older adults. AIMS: Our study aimed to examine how comorbidities affect depression and quality of life in geriatric patients in both outpatient and hospital settings. METHODS: 100 patients (50 from internal medicine outpatient clinic and 50 from internal medicine ward) were included in the study accor

[Geriatric multimorbidity in claims data: part 3: prevalence and predictive power of geriatric conditions in an age-specific systematic sample].

BACKGROUND: In various contexts, the identification of insurants with geriatric conditions (GC) can offer new approaches for specific medical services. GC can be determined from diagnoses data of insurants retrieved from different care sectors, and supplemented with other relevant claims data, e.g., long-term care levels and pharmaceutical data. METHODS: Part 3 of this study is based on a systematic sample of 957,447 AOK insurants (age ≥ 60 years). Prevalence of 15 GC was investigated using anonymous claims data of diagnoses from physicians in the ambulant care setting and diagnoses from hosp

Finger-Stick Hemoglobin Test in Geriatric Patients.

BACKGROUND: Finger-stick hemoglobin tests, also known as point-of-care testing (POCT) for hematology, offer a convenient and minimally invasive method for obtaining blood samples and conducting various hematological analyses. Given the paucity of studies in the literature assessing fingertip hemoglobin levels in geriatric patients and the simplicity of the method, the objective of this study was to measure fingertip blood hemoglobin levels in the geriatric population. METHODS: The study was conducted on geriatric patients who presented to the emergency department of a tertiary teaching and re

Criteria for the qualification of geriatric patients to the trauma center.

BACKGROUND: The growth of the geriatric population, resulting in an exponential increase in the number of injured older people, determines the necessity of ensuring adequate access to specialized units such as trauma centers (TCs). The specificity of pathophysiological processes that progress with age, worsening the body's response to trauma, makes it difficult to develop an optimal triage protocol for geriatric patients, reducing high undertriage, i.e., underestimation of injuries and referral to lower-reference units. AIM: To verify the TC qualification criteria for 65+ patients and analyze

Voiding Dysfunction and Parkinson Disease in Geriatric Patients.

Urinary symptoms suggestive of overactive bladder are common nonmotor symptoms associated with Parkinson disease (PD) that increase burden of living with PD as well as the risk of falls and institutionalization. Emerging evidence suggests lifestyle and behavioral therapies including pelvic floor muscle exercise-based treatments are reasonable initial strategies. When considering medication-based therapies clinicians should assess for common comorbid conditions such as falls, constipation, sleep disturbance, and orthostatic hypotension, which may influence the potential for adverse drug events.

Outcomes of the Unvaccinated Geriatric Population with Coronavirus Disease.

Coronavirus disease (COVID-19), an acute infectious disease caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Older patients, especially those aged > 65 years, are very frail, primarily owing to comorbidities and physiological changes. This retrospective, observational, single-center cohort study included all unvaccinated patients admitted in the geriatric ward with a COVID-19 diagnosis confirmed by reverse transcription polymerase chain reaction during the first year of the pandemic. The study population was divided into survivors and deceased patients. We retrieved

An Unusual Case of Abdominal Pain in a Geriatric Man.

BACKGROUND: Immunoglobulin A vasculitis, historically known as Henoch-Schönlein purpura, is a rare form of autoimmune-induced vasculitis most common in children. This disease is characterized by a purpuric rash, arthritis, digestive tract complication, and renal inflammation (Hopkins). CASE REPORT: We present the case of a 78-year-old man in the emergency department with findings of weakness, abdominal pain, and bloody diarrhea for 3 days and a new-onset bilateral lower extremity rash. Diagnostic imaging and labs diagnosed this patient with immunoglobulin A vasculitis (IgAV) with associated a

New horizons in undergraduate geriatric medicine education.

Current projections show that between 2000 and 2050, increasing proportions of older individuals will be cared for by a smaller number of healthcare workers, which will exacerbate the existing challenges faced by those who support this patient demographic. This review of a collection of Age and Ageing papers on the topic in the past 10 years explores (1) what best practice geriatrics education is and (2) how careers in geriatrics could be made more appealing to improve recruitment and retention. Based on these deeper understandings, we consider, as clinician educators, how to close the gap bot…

Rib Fractures and Frailty in Geriatric Trauma Patients.

As rib fractures are a common injury in the geriatric trauma population and can result in increased morbidity and mortality, we sought to understand predicting outcomes in this population. We hypothesized that frail geriatric rib fracture patients would have worse outcomes than their non-frail counterparts. This single-center retrospective study includes patients from July 2019 to June 2022 who were ≥65 years-old, had ≥ 2 rib fractures, and a documented Clinical Frailty Scale score. Univariate analysis was conducted comparing frail vs non-frail, and ≤3 rib fractures vs >3 rib fractures. Multiv

Spine Surgery in a Geriatric Population. Is it Really Different?

OBJECTIVES: Spinal degenerative disorders are a major cause of morbidity in the elderly resulting in high dependency. Most of them have a trend to be managed conservatively considering age, comorbidities, and apprehensions of surgical complications. Surgical intervention at early stage with appropriate indications can have better outcomes rather than conservative management in fit patients. The objective of the study is to evaluate the functional outcome in geriatric patients > 60 years who have undergone various spinal procedures for degenerative spine. METHODS: The study is retrospective, w

Oral health status in a Lebanese geriatric population.

BACKGROUND: The sustained growth of older people and the epidemiological shift from communicable to chronic health problems have attracted increased attention to geriatric wellbeing, including oral health. AIMS: To evaluate the oral health of urban community-dwelling older people and identify their priority oral health needs. METHODS: We recruited 352 adults aged ≥ 65 years in 2015 from social organizations and primary healthcare centres in Beirut and its suburbs. Older people underwent dental examination including the decayed, missing and filled teeth (DMFT) score, gingival health [plaque i

A medicolegal evaluation of geriatric cases with traumatic injuries.

BACKGROUND: The morbidity and mortality of traumatic injuries are higher in individuals aged >65 years, which are a population group showing an increase worldwide, compared to other age groups. The aim of this study was to compare the medicolegal aspects, such as abuse of the elderly with the morbidity and mortality rates of injuries resulting from trauma by examining the age, gender, trauma history, type of traumatic injury, clinical progress following injury, elderly abuse, and the care environment of the person in cases aged 65 years and older. METHODS: In this cross-sectional and descript

Specific Challenges in Geriatric Cirrhosis and Hepatic Encephalopathy.

As the world's population ages, diseases predominantly found in the elderly now overlap with diseases that were thought to be the purview of younger patients. This includes chronic liver disease, which affects more than 2 billion people worldwide. Owing to the obesity epidemic (and associated metabolic diseases), nonalcoholic fatty liver disease has become the most common cause of chronic liver disease and cirrhosis. A major complication of cirrhosis is hepatic encephalopathy (HE), which becomes challenging to diagnose in elderly patients. HE is usually included in the differential diagnosis o
