Implants have been developed in different sizes and shapes to mimic the pectoralis major muscle and its nipple-areolar complex below it. These soft, solid silicone implants have the approximate dimensions in height, length, and thickness as the muscle the implant is to mimic. Some improvements in the treatment of breast hypertrophy have also allowed a better result in the surgery of gynecomastia. The challenge of obtaining a smooth and flat result has been helped by the technique of liposuction with circum-areolar excision.

Two men at the age of 48 and 54 yr developed gynecomastia and lost their potency after the use of estrogen containing hair lotions. During exposure to the lotion the levels of 17-beta estradiol were increased, whereas the levels of testosterone and gonadotropins were depressed. Thus, a previous application of such hair lotions should be considered in the differential diagnosis of gynecomastia.

The authors describe the case of a 36-year-old man with gynecomastia who was previously treated with liposuction of the breast for cosmetic purposes. Histologic examination of a subsequent excisional biopsy revealed nests of displaced epithelial cells in adipose tissue. Epithelial cell displacement is a well-known risk of core needle biopsies and fine-needle aspirations of breast lesions. However, to the authors' knowledge, epithelial displacement in gynecomastia after liposuction, mimicking invasive ductal carcinoma, has not previously been reported.

We describe a 16-year-old boy with Leydig cell tumor who initially presented bilateral gynecomastia with increased estradiol concentrations, decreased testosterone concentrations and normal gonadotropin levels. Testicular ultrasonography showed a tumor in the left testicle, and orchidectomy was performed. Histopathological analysis revealed a Leydig cell tumor. Two months after surgery, the gynecomastia diminished gradually and estrogen levels returned to normal. No recurrences have occurred during a 2-year follow-up.
