[HELLP syndrome in the "Daniel A Carrion" National Hospital: report of cases].

HELLP syndrome is an unusual complication of severe preeclampsia- eclampsia and is characterized by hemolysis (microangiopathic), elevated liver enzymes and thrombocytopenia. It is present in 2 to 19.3% of patients with toxemia and in 0.85% of all pregnancies. We present 2 cases of Hellp syndrome post-partum (in less than 31% of patients, according to literature) that fulfill with University of Tennessee diagnostic criteria, with clinical and laboratory features as described by literature. The second case presented as a complication, disseminated intravascular coagulation (DIC), which complica…

[Liver rupture in pregnancy: a life-threatening complication of the HELLP syndrome].

A 31-year-old primigravida with pre-existent hypertension was admitted at 28.5 weeks gestation with foetal growth retardation. On the third day after admission she developed a 'haemolysis, elevated liver enzymes and low platelets' (HELLP) syndrome with severe pain in the epigastrium. Intrauterine death had occurred and abruptio placentae was suspected. Induction of labour was started with intravenous prostaglandins. The next day the haemoglobin concentration had decreased despite transfusions of red blood cell concentrates. At ultrasound examination, free fluid was detected in the abdomen and

[Clinical and chemical laboratory course of HELLP syndrome--a retrospective analysis].

The HELLP-Syndrom (hemolysis, elevated liver enzymes, low platelet count) is considered as a severe complication of eclampsia with unpredictable development of pregnancy including high maternal and fetal risk. The result of retrospective analysis of all deliveries of the years 1986-1991 at the UFK Marburg were 28 cases of proved HELLP-Syndrom. Medical history, correlation of clinical and laboratory findings as well as the development of the disease and the neonatal dates were evaluated by computerized documentation. The incidence of HELLP-Syndrom was 28 of 8111 deliveries at all (0,34%). 82% o

[Perinatal mortality and preeclampsia/eclampsia: influence of HELLP syndrome on the primigravida].

Perinatal losses have been studied in 37 pre-eclamptic/eclamptic patients: 13 of them exhibited a HELLP syndrome. Only 23 newborns survived (62.5%); after deduction of 4 pregnancies of less than 28 weeks, the perinatal mortality was 27%. Comparison with the local perinatal mortality showed a 6-fold higher risk for the fetus when pregnancy was complicated by pre-eclampsia/eclampsia, regardless of a superimposed HELLP syndrome. For reasons beyond our understanding the fetal losses with HELLP syndrome are much higher in primigravidae.

[HELLP syndrome. 7-year experience at the National Institute of Perinatology].

We studied 36 patients with Hellp syndrome and her newborn during seven years in the National Institute of Perinatology (INPer). The objective of this study is show the epidemiology and clinic features, unite diagnostic criteria and classification, furthermore establish morbidity and mortality maternal and perinatal. The 44.6% (16/36) was type I, and 55.6% (20) was type II. The average of pregnancies was 3. The 38.9% (14/36) received regular prenatal control. Both types were more common in patients with preterm pregnancies, between 28-36 weeks. The patients with severe preeclampsia and eclamps…

Magnetic resonance evaluation of brainstem dysfunction in eclampsia and the HELLP syndrome.

An unusual case of a 40-year-old woman with eclampsia and the HELLP syndrome associated with brainstem dysfunction is presented. The patient made a progressive neurological recovery. A follow-up magnetic resonance image (MRI) demonstrated complete resolution of the multiple foci. The MRI of the brainstem was found to correlate far more closely with neurological findings than with computed tomography (CT scan).

Temporising management of severe pre-eclampsia with and without the HELLP syndrome.

OBJECTIVE: To test the null hypothesis that the course and outcome of pregnancy in patients with severe pre-eclampsia receiving temporising haemodynamic treatment does not depend on the presence or absence of the syndrome of haemolysis, elevated liver enzymes, and a low platelet count (HELLP). DESIGN: A case-controlled study. SETTING: High risk obstetric unit, University Hospital Rotterdam-Dijkzigt, Rotterdam. SUBJECTS: One hundred and twenty-eight consecutive pre-eclamptic patients with HELLP, gestational age less than 34 weeks, matched for maternal and gestational age with 128 pre-eclampt

[Liver complications of HELLP syndrome--an indication for emergency liver transplantation?].

Three patients were treated suffering from a severe HELLP-syndrome. The problems as far as the diagnostic and therapy are concerned are discussed. All three patients had to be operated on in an emergency state. The operations were performed due to acute and severe abdominal bleeding. Two of the patients needed liver transplantation as an emergency procedure, one of them died after hepatectomy waiting for a graft. Another one died seven weeks after successful transplantation due to prolonged sepsis. The third patient is more than one year after transplantation alive and doing well.

[Acute kidney failure associated with the HELLP syndrome. A case report].

A patient with 36 weeks' gestation with severe preeclampsia and HELLP syndrome associated to acute renal failure, was studied. Symptomatology, abnormal laboratories results and treatment, are emphasized. The haptoglobin level has been reported like sensitive marker of moderate hemolysis; it could permit an early diagnosis of HELLP syndrome. On the basis at physiopathology mechanism that were described, HELLP syndrome is susceptible of prevention in patients with high risk.

Is the HELLP syndrome due to inherited factors? Report of two cases.

The etiology and pathogenesis of the HELLP syndrome, a multisystem disease occurring only in pregnancy, are still unclear. Curiously, very few authors have investigated whether inherited factors may be involved. We report two cases of HELLP syndrome in two unrelated women whose fetuses were relatives (first cousins). The first case concerned a woman aged 32 with a normal course pregnancy who was admitted to the hospital for fever, nausea and vomiting, low platelets, hemolysis and increased liver enzymes. Abruptio placentae with fetal death and severe disseminated intravascular coagulation with…

HELLP-syndrome. Difficulties in diagnosis and therapy of a severe form of preeclampsia.

As in cases of HELLP-Syndrome both the mother and the fetus are at greater risk we analyzed retrospectively the clinical records of 14 patients concerning changes of typical laboratory parameters, clinical course including time of correct diagnosis, as well as fetal and maternal complications. The death of one woman represents a maternal mortality rate of 7.1% whereas perinatal mortality ranged 20% due to 3 intrauterine fetal losses. In our study group the mean time interval from onset of clinical symptoms like pain, vomiting etc. until admission to our department was 8.2 days (median: 4 days)

Thrombocytopenia in the HELLP syndrome is not due to platelet-associated IgG (PAIgG).

The presence of membrane-bound and circulating platelet-associated IgG (PAIgG) in nine pregnant women with hemolysis, elevated liver enzymes, and low platelet count (HELLP syndrome) was investigated. The reference group consisted of 21 patients with chronic idiopathic thrombocytopenic purpura (ITP). In the majority of patients with ITP, thrombocytopenia results from the binding of PAIgG to the platelet membrane with subsequent platelet destruction. In none of the patients with HELLP syndrome membrane-bound or circulating PAIgG was found. By contrast, membrane-bound and circulating PAIgG was de…

[Clinical course of premature and newborn infants of mothers with HELLP syndrome].

The HELLP-syndrome is complicated by a maternal mortality of 3.5% and a perinatal mortality between 9.5 and 60%. It is a variant of severe preeclampsia which includes hemolysis, elevated liver enzymes and low platelets. It is described in the literature that neonates of mothers with HELLP-syndrome show characteristic symptoms especially thrombocytopenia, leukocytopenia and prenatal somatic dystrophy. In this retrospective investigation of 36 preterm and term infants of mothers with HELLP-syndrome we found the following results: 1. Thrombocytopenia was seen in 11% and leucocytopenia in 12% of t

The natural history of HELLP syndrome: patterns of disease progression and regression.

Despite much recent interest in HELLP syndrome (hemolysis, elevated liver enzymes, and low platelet count), there is little published information about the natural history of this unique form of severe preeclampsia-eclampsia. The time course and pattern of laboratory abnormalities for 158 patients with HELLP syndrome managed in a single tertiary referral center between 1980 and 1989 were studied retrospectively. Despite considerable patient variation, most gravid women with HELLP syndrome had decreasing platelet counts until 24 to 48 hours after delivery. Conversely, lactate dehydrogenase conc…

[The HELLP syndrome--a rare form of preeclampsia. Anesthesiologic and obstetric aspects].

A 34-year-old quadripara was hospitalized in the 33rd gestational week due to an acute hypertensive crisis, headache, upper abdominal pain, icterus, and proteinuria. Laboratory testing revealed hemolysis, hepatic dysfunction, and thrombopenia. The values returned toward normal after delivery by cesarean section. Diffuse bleeding in the surgical wound and acute renal failure necessitated two relaparotomies, intensive hemotherapy, and hemodialysis. Mother and child were released in good condition. Anesthesiological and obstetrical aspects of the HELLP syndrome are discussed.

Posterior reversible encephalopathy syndrome in patient of severe preeclampsia with Hellp syndrome immediate postpartum.

Posterior reversible encephalopathy syndrome (PRES) is a rare clinico-neuroradiologic condition, not commonly reported in the literature. PRES is an uncommon complication of severe preeclampsia/eclampsia. We report the management of one patient with postpartum preeclampsia as an association of HELLP syndrome presenting with status-epileptics. Early diagnosis along with timely supportive therapy resulted in the successful management of this challenging case. Recent understanding on the pathophysiology of this uncommon condition is discussed. We highlight the importance to obstetricians, intensi…

The serum gamma-glutamyl transpeptidase-to-platelet ratio predicts HELLP syndrome.

BACKGROUND: HELLP (Hemolysis, elevated liver enzymes, and low platelets) syndrome is a dangerous obstetric condition that is in great need of simple and inexpensive non-invasive early predictors, but it has been poorly studied. This study was conducted to investigate the predictive role of serum gamma-glutamyl transpeptidase to platelet ratio (GPR) during pregnancy in HELLP syndrome and its adverse pregnancy outcomes. METHODS: This was a retrospective study in a tertiary hospital. One hundred parturients were allocated into two groups: HELLP group (n = 50) and control group (n = 50). RESULTS…
