The fate of stage I Masquelet cement spacers in long bone reconstruction for acute fractures.

INTRODUCTION: The Masquelet technique is widely used for reconstruction of segmental long bone defects following acute fractures; however, n … = 0.423). Among patients with at least 6 months of follow-up, rates of fracture-related infection, hardware removal, wound dehiscence, amputation, and mortality were similar between groups. On multivariable analysis, no demographic or clinical variables were independently associated with Stage II completion. CONCLUSION: Nearly one-third of patients undergoing th …

Proximal forearm antegrade elastic stable intramedullary nailing for pediatric distal radius metaphyseal-diaphyseal junction fractures: A modified technique.

BACKGROUND: Distal radius metaphyseal-diaphyseal junction (MDJ) fractures in children are technically demanding, and the optimal fixation strategy remains controversial. The clinical performance of proximal-forearm antegrade elastic stable intramedullary nailing (A-ESIN) was compared with conventional retrograde crossed Kirschner-wire fixation (R-KW). METHODS: This retrospective study included 79 children treated for distal radius MDJ fractures at [blinded for review] between January 2018 and April 2024, with a minimum follow-up of 12 months. Thirty-eight underwent A-ESIN and 41 received R-KW

Vertebral body expansion and percutaneous short-segment posterior fixation of thoracic and lumbar fractures requiring corpectomy: a retrospective single-center study.

BACKGROUND: Thoracic and lumbar fractures with a Load Sharing Classification (LSC) score ≥ 7 are considered at high risk of failure after isolated short posterior fixation, which historically led to systematic anterior reconstruction by corpectomy with substantial morbidity. Minimally invasive vertebral body expansion combined with short posterior fixation may restore anterior column support while limiting surgical aggressiveness. However, data remain limited for fractures with high LSC scores. This study addressed the following questions: (1) Does the combined strategy reduce the failure rate

Sagittal and Coronal Plate Position Is Not Associated With Flexor Tendon Irritation, Rupture, or Need for Plate Removal After Volar Locking Plate Fixation of Distal Radius Fractures.

BACKGROUND: Whether sagittal and coronal plate placement influences the risk of flexor tendon complications after volar plating of distal radius fractures is debated. In this study, we tested the null hypothesis that sagittal and coronal plate position is not associated with flexor tendon irritation, rupture, or plate removal. METHODS: We performed a retrospective review of 144 patients treated for distal radius fractures with a volar locking plate by 4 fellowship-trained hand surgeons. Patient, surgical, and radiographic factors were recorded, including measures of sagittal (Soong grade) and

Repair of a floating sternum with autologous rib grafts and polylactide bioabsorbable struts in an 18-year-old male.

Failed regeneration of costal cartilage after open repair of pectus chest wall deformities can result in a floating sternum. A floating sternum can be repaired by insertion of a rib graft between the rib and sternum, and stabilization with a metal strut. The metal implant is usually removed with a second operation. We report use of bioabsorbable struts to stabilize rib grafts during repair of a floating sternum in an 18-year-old male with a failed open repair of pectus carinatum. He had an uncomplicated peri-operative course. One year later, the sternum had a normal appearance and was sturdy.

Fixation of Displaced Medial Epicondyle Fractures in Adolescents.

BACKGROUND: Open reduction and internal fixation of displaced medial epicondyle fracture dislocations in adolescents is recommended for incarceration of the epicondyle in the joint and for athletes who need a stable elbow for their sport. A screw placed across the epicondyle into the medial column avoiding the olecranon fossa is a common fixation method. One author has recommended adding a metal washer to the screw fixation because of the perceived risk of epicondyle fragmentation or penetration when using a screw alone. The purpose of this study was to determine whether the use of a screw and

Septic arthritis due to Nocardia: Case report and literature review.

Nocardia is an uncommon cause of septic arthritis. We found only 37 cases reported in the literature thus far. Amongst these, only five involved prosthetic joints. Three cases were caused by N. nova and one each by N. farcinica and asteroides. Septic arthritis due to Nocardia has a favorable outcome with a combination of surgical debridement and prolonged antimicrobial therapy of three to six months. For prosthetic joint infections, removal of hardware seems to carry a better prognosis. Trimethoprim-sulfamethoxazole continues to remain the drug of choice.
