Contraception for midlife women: a review.

Family planning represents a key component of reproductive health care. Accordingly, the provision of contraception must span the reproductive age spectrum, including perimenopause. The risk of pregnancy is decreased, but not trivial, among women over 40 years of age. Evidence-based guidelines for contraceptive use can assist clinicians in counseling their patients in this population. Intrauterine contraception is one of the most effective methods and is safe to use in midlife women with few exceptions. Progestin-only contraception is another safe option for most midlife women because it is no

Human immunodeficiency virus (HIV) related cryptococcal meningitis in rural central Thailand--treatment difficulties and prevention strategies.

In 2000, cryptococcal meningitis was a common HIV related opportunistic infection in central Thailand requiring inpatient management but few patients suffering from it could afford a full course of treatment once infection had occurred. Since then, the production of generic fluconazole and highly active antiretroviral therapy (HAART) in Thailand (and national guidelines regarding their use) is reducing the incidence of this condition and such a strategy of prevention appears much more effective than treating established disease. There remains a continuing need for health education in the regio

[Do German nutrition interventions for children and adolescents meet quality criteria for program planning and evaluation? - Results of a survey of institutions at state and regional levels].

PURPOSE: The international literature describes comprehensive nutrition interventions for children and adolescents, and gives recommendations for successful program planning. For Germany, only few studies on these issues are available, and there is a particular scarcity of data on implementation, acceptance and effectiveness of nutrition interventions. Thus the aim of this study was to record nutrition interventions at state and regional levels in Germany, and to assess their project design and evaluation quality. On the basis of these data, conclusions about implementation, acceptance and eff

A framework for scabies control.

Scabies is a neglected tropical disease (NTD) that causes a significant health burden, particularly in disadvantaged communities and where there is overcrowding. There is emerging evidence that ivermectin-based mass drug administration (MDA) can reduce the prevalence of scabies in some settings, but evidence remains limited, and there are no formal guidelines to inform control efforts. An informal World Health Organization (WHO) consultation was organized to find agreement on strategies for global control. The consultation resulted in a framework for scabies control and recommendations for map

Creating Change at Scale: Quality Improvement Strategies used by the California Maternal Quality Care Collaborative.

Creating change at scale within a short time frame poses multiple challenges. Using the experience of the California Maternal Quality Care Collaborative, the authors illustrate how state perinatal quality collaboratives have been able to achieve this goal using a series of key steps: engage as many disciplines and partner organizations as possible; mobilize low-burden data to create a rapid-cycle data center to support the quality improvement efforts; provide up-to-date guidance for implementation using safety bundles and tool kits; and make available coaching and peer learning to support impl

A Scoping Review on Category A Agents as Bioweapons.

Category A agents are biological pathogens that could pose a threat to health and human safety if used as bioweapons. The exploration and possibility of these threats must be comprehensively reviewed to create a preparedness plan to recognize outbreaks, to educate the public, and to offer vaccinations and/or treatment options, if available. A scoping review using PRISMA guidelines was performed to categorize current information on Category A biological agents as well as understand their potential for future threats. The results used 34 articles and found that while botulin neurotoxins were the

Combining diagnosis and treatment using ASBRU.

Traditionally, diagnosis and treatment have been seen as two distinct tasks. Consequently, most approaches to computer supported health care focus on one of the two-mostly on diagnosis or rather on the interpretation of measurements which is much better understood and formalised. However, in practice diagnosis and treatment overlap and influence each other in many ways. Combinations range from repeatedly going through the diagnosis-treatment loop over a period of time to permanent monitoring of the patients' health condition as it is done in intensive care units. In this article we describe ho

Evidence-Based Guidelines for Keratorefractive Lenticule Extraction Surgery.

TOPIC: Development of evidence-based guidelines for keratorefractive lenticule extraction (KLEx). CLINICAL RELEVANCE: Keratorefractive lenticule extraction refers to various corneal refractive procedures involving removal of refractive lenticules of intrastromal corneal tissue, typically through a small incision, eliminating creation of a corneal flap. This technique has gained popularity rapidly; however, no clinical practice guidelines exist. METHODS: These evidence-based guidelines were developed following the World Health Organization guidebook using the Appraisal of Guidelines for Resea
