Health Services Professionals Endorse Omitting Some Research Information Under Limited Circumstances: Survey.

In an era of polarization, misinformation, and declining public trust in experts, effective public communication about health research is essential for disseminating evidence and maintaining the credibility of the research enterprise. Yet when communicating research findings to general audiences, experts face dilemmas about whether to omit information to avoid public confusion, to prevent the stigmatization of marginalized constituencies, or to keep information from being weaponized against vulnerable groups. Drawing on a national survey of health services professionals conducted in April and …

Rebuilding health and human services: a call to strategic planning and action.

The capacity of the U.S. Department of Health and Human Services (HHS) has been substantially reduced since early 2025. This has included staffing cuts on a scale not seen in decades, delays in implementing billions of dollars in grants, and other disruptions to normal operations, all of which have received considerable public coverage and attention. To date, far less public attention has been given to rebuilding efforts. This commentary presents recommendations for rebuilding HHS' lost capacity, following a series of roundtable discussions convened by a network of former senior HHS appointees

Workers' Illness, Injury, and Exit From the Home Health Care Services Industry.

BACKGROUND: High workforce turnover is among the main challenges facing home health care services industries (HHCS) in many countries. Illness and injury are often mentioned among the main reasons for leaving HHCS. We examine the impact of illness and injury on Canadian workers' exit from HHCS and discuss the implications. METHODS: We use linked Canadian longitudinal administrative employment and health microdata from 2009 to 2022, also linked to 2016 Census microdata. Our longitudinal analysis uses the event study framework. We identify HHCS workers' health events using three sources: Record

Adolescent utilization of school based mental health services in the United States.

BACKGROUND: Over 14 million adolescents have a diagnosable mental, behavioral, or emotional disorder yet only 20% receive adequate mental health services. There is a critical need to identify accessible and effective pathways to treatment. School based mental health services (SBMHS) are an optimal setting for timely identification, effective management, and convenient delivery of evidence-based mental health care. METHODS: Using data from the 2019 National Survey on Drug Use and Health, we employed structural equation modeling to examine the relationships between utilization of SBMHS and scho…

A prognostic model for predicting functional impairment in youth mental health services.

BACKGROUND: Functional impairment is a major concern among those presenting to youth mental health services and can have a profound impact on long-term outcomes. Early recognition and prevention for those at risk of functional impairment is essential to guide effective youth mental health care. Yet, identifying those at risk is challenging and impacts the appropriate allocation of indicated prevention and early intervention strategies. METHODS: We developed a prognostic model to predict a young person's social and occupational functional impairment trajectory over 3 months. The sample include…

Optimising the delivery of sexual health services in Australia: a qualitative study.

BACKGROUND: With a rise in sexually transmitted infections in Australia, it is important to evaluate and optimise current sexual health services to reduce barriers to access and increase testing. We aimed to describe the range of sexual health services in Australia, focusing on strategies to improve testing among priority populations, and any future strategies participants believed would improve service delivery. METHODS: We conducted 20 semi-structured interviews with key informants working in Australian sexual health services from July to December 2020, and collected data on service charact…

Medicaid Expansion And Health Services Use For Adults Experiencing Homelessness In Arkansas.

More than 500,000 people in the US experience homelessness at any given time, many of whom now qualify for Medicaid in states that expanded coverage under the Affordable Care Act (ACA). In this article we use a novel data set from Arkansas to provide the first estimates of the association between gaining coverage through the ACA's Medicaid expansion and health services use for a population experiencing homelessness. We find that Medicaid expansion was associated with large initial increases in inpatient hospitalizations and emergency department visits-which declined steadily over time-among ad…

Providing reproductive health services for women who inject drugs: a pilot program.

BACKGROUND: Needle syringe programs (NSPs), a proven harm reduction strategy for people who inject drugs, frequently offer limited healthcare services for their clients. Women who inject drugs face multiple barriers to accessing reproductive health care in traditional settings: personal histories of trauma, judgmental treatment from providers, and competing demands on their time. Our aim was to implement patient-centered reproductive healthcare services at a Seattle NSP. METHODS: We interviewed clients and staff of an NSP in Seattle and staff of other community-based organizations serving wom

Access to Health Care Services among Young People Exchanging Sex in Detroit.

Within the related epidemics of sex exchange, drug use, and poverty, access to health care is shaped by intersecting identities, policy, and infrastructure. This study uses a unique survey sample of young adults in Detroit, who are exchanging sex on the street, in strip clubs, and at after-hours parties and other social clubs. Factors predicting access to free or affordable health care services, such as venue, patterns of sexual exchange influence, drug use and access to transportation, were examined using multivariable logistic regression and qualitative comparative analysis. The most signifi

The impact of Recovery Colleges on mental health staff, services and society.

AIMS: Recovery Colleges are opening internationally. The evaluation focus has been on outcomes for Recovery College students who use mental health services. However, benefits may also arise for: staff who attend or co-deliver courses; the mental health and social care service hosting the Recovery College; and wider society. A theory-based change model characterising how Recovery Colleges impact at these higher levels is needed for formal evaluation of their impact, and to inform future Recovery College development. The aim of this study was to develop a stratified theory identifying candidate …

Stigma at every turn: Health services experiences among people who inject drugs.

BACKGROUND: People who inject drugs (PWID) encounter varying forms of stigma in health services contexts, which can contribute to adverse outcomes. We explored the lived experience of stigma among PWID to elucidate pathways by which stigma influences health care access and utilization. METHODS: We conducted 46 qualitative interviews with PWID in California's Central Valley between March and December 2015, as part of a multi-phase, multi-method study examining implementation of a new pharmacy syringe access law. A "risk environment" framework guided our data collection and we used a deductive/…

[Economic Effect of Home Health Care Services for Community-dwelling Vulnerable Populations].

PURPOSE: In this study the costs and benefits of a home health care program were examined to evaluate the economic feasibility of the program. The costs and benefits of the program were analyzed using performance data and health data. The benefits were classified as the effects of pressure ulcer care, skin wound care and catheters management. The program effect was evaluated on the change of progress using transition probability. Benefits were divided into d

[Hospital readmission rate and associated factors among health services enrollees in Colombia].

Hospital readmissions are common and expensive, and there is little information on the problem in Colombia. The objective was to determine the frequency of 30-day all-cause hospital readmissions and associated factors. This was a retrospective analytical cohort study of 64,969 hospitalizations from January 2008 to January 2009 in 47 Colombian cities. 6,573 hospital readmissions, prevalence: 10.1% (men 10.9%, women 9.5%), 44.7% > 65 years of age. Hospital readmissions was associated with higher mortality (5.8% vs. 1.8%). There was an increase in the Hospital readmissions rate in patients with d
