[Chronic heart failure with impaired left ventricular function (systolic heart failure)].

Chronic heart failure is a common public health problem. The disease has a poor prognosis with high mortality rate and the incidence increases continuously. Prognosis of chronic systolic heart failure can be improved by several different medications as well as by special cardiac interventions based on the newly-published European and American guidelines. In case of severe systolic dysfunction, hospitalization and mortality can be reduced using angiotensin converting enzyme inhibitors, angiotensin receptor blocking drugs, beta-receptor blocking agents and aldosterone antagonists, as evidenced i…

Heart transplantation in biventricular congenital heart disease: indications, techniques, and outcomes.

Heart transplantation is an accepted therapeutic modality for end-stage congenital heart disease for both biventricular and univentricular anomalies. Many transplant centers have pushed the limits of transplantation to include patients with high pulmonary vascular resistance, high panel reactive antibodies, positive cross-matches, and ABO-incompatibility. Excellent results have been possible, particularly with the development of improved diagnostic and therapeutic algorithms to prevent and treat rejection, infection, and post-transplant lymphoproliferative disease. Late graft failure and chron…

[Congenital heart disease in adults. Part I. Non-cyanotic heart diseases].

Over the last decades major advances have been made in the diagnosis and treatment of congenital heart disease. Therefore, many children with simple and complex congenital heart defects can now survive to adulthood and represent an expanding population. Most of them are not "cured" and need specialized follow-up. This two-part review presents the more common acyanotic and cyanotic congenital heart diseases that adult cardiologists are likely to face in their daily practice and discusses recent treatment advances.

Heart transplantation for end-stage heart failure caused by iron overload.

Few reports exist concerning heart transplantation in recipients with end-stage myocardiopathy-associated heart failure caused by iron overload occurring with beta-thalassaemia, Diamond-Blackfan syndrome or haemochromatosis. Seven potential transplant candidates (six male, one female, mean age 26 years) with such heart failure, following desferrioxamine application subcutaneously over a number of years, and intravenously during their hospitalization before transplantation, were retrospectively analysed. Five were New York Heart Association (NYHA) class IV, three experienced one or more resusci…

[Extracorporeal heart pump--a bridge to heart transplantation. First Norwegian experiences].

By February 1997, the number of heart transplantations at the National Hospital had reached 265. In our department, the first bridge to transplantation in using Thoratec artificial heart was performed in 1995. A 44 year-old patient with life-threatening heart failure was placed for one week on cardiopulmonary assist with an intra-aortic balloon pump and a veno-arterial cardiopulmonary bypass. Due to unceasing heart failure a Thoratec left ventricular assist device was implanted as a bridge to transplant. The patient was successfully transplanted two days after implantation of Thoratec. After e…

[Heart transplantation as a life saving procedure after conventional heart surgery].

The first cardiac transplantation in Norway was performed in November 1983. At the end of 1995 the number of heart transplantations at our institution had reached 238. Five of these patients (2.1%) were transplanted in the early postoperative period of failed conventional cardiac surgery (respectively 20, 26, 54, 84, and 234 hours post surgery). The primary procedure consisted of mitral valve replacement plus left ventricular aneurysm resection, coronary artery bypass grafting, Konz-Konno procedure, aortic valve replacement, and redo coronary artery bypass grafting in patients aged 47, 54, 49,…

Concomitant donor heart coronary artery bypass grafting during orthotopic heart transplantation.

A 54-year-old diabetic woman with severe cardiomyopathy was placed on our heart transplant candidate list. The patient's condition rapidly worsened and a potential donor-a 45-year-old man whose blood was compatible with that of our patient-was located. Because of the donor's age, coronary arteriography was done, and stenosis in the midleft anterior descending coronary artery was identified. Since the patient's status was critical, the donor heart was accepted despite the presence of stenosis. We used the recipient's internal mammary artery to bypass the stenosis in the left anterior descending…

Intensive care management of children following heart and heart-lung transplantation.

We report the intensive care management of 23 children (age 3-15 years) following orthotopic heart (HT) and combined heart and lung transplantation (HLT) performed at our 2 institutes between February 1985 and August 1989. Cyclosporin A, azathioprine and steroids were given as routine immunosuppression, whilst anti-thymocyte globulin (ATG) was used for the first 3 post-operative days. Mean ventilation time was 24.6 h (range 4-74 h). Cardiovascular support comprised isoprenaline infusions in all patients (mean period 65.7 h) whilst dopamine and other inotropic agents were used less frequently. …

Endothelial cell proliferation in the rat heart following local heart irradiation.

Radiation-induced myocardial degeneration in the rat is preceded by changes in capillary structure and function, which may be a major factor in the pathogenesis of radiation-induced heart disease. In order to investigate the mechanism of capillary damage we studied endothelial cell proliferation in untreated control rats and in rats at different times following local heart irradiation with 20 Gy using [3H]thymidine autoradiography. Since the latency times of myocardial degeneration as well as capillary damage are about twice as long in Sprague-Dawley rats as in Wistar rats, endothelial cell pr…

Central nervous system infections in heart and heart-lung transplant recipients.

Infections, a major cause of morbidity and mortality in immunosuppressed heart and heart-lung transplant recipients, frequently involve the central nervous system and can produce devastating neurologic sequelae. Between 1980 and 1987, a total of 363 heart transplant and 54 heart-lung transplant recipients at the University of Pittsburgh sustained 13 intracranial infections two to 143 weeks after transplantation. Computed tomography demonstrated well-defined Nocardia and Aspergillus abscesses in four patients. Cerebrospinal fluid was normal in all cases studied, including in those patients in w…

Early and late postoperative complete heart block in pediatric patients submitted to open-heart surgery for congenital heart disease.

The incidence of complete heart block (CHB) following open-heart surgery for congenital heart disease is about 1%. Most of postoperative CHBs are the consequence of procedures involving the closure of ventricular septal defect; they usually occur immediately after surgery or early in the postoperative period; in few cases they also may occur many months or years after surgery. Early postoperative CHB can be transient or permanent. Permanent pacing is generally not recommended in the former. On the contrary, if CHB persists after at least two weeks of temporary pacing, permanent pacing is neede…

Cyanotic Heart Disease.

Cyanotic congenital heart defects are malformations of the heart that affect its structure and function. The etiology of most of these defects is multifactorial, but some common causes include maternal conditions, genetic defects, and genetic mutations. Six of the most common cyanotic congenital heart defects include Tetralogy of Fallot, transposition of the great arteries, persistent truncus arteriosus, total anomalous pulmonary venous return, tricuspid atresia, and Ebstein's anomaly. These defects share several common features, however, each is distinguished by key cardiac abnormalities and …

Ischemic heart disease.

The new section on ischemic heart disease (IHD) among the Italian Sports Cardiology Guidelines (COCIS) provides updated recommendations for the evaluation, management and eligibility of athletes with known or suspected IHD. Emphasizing a risk-stratified approach, the guidelines integrate clinical, functional, and imaging assessments to determine the safety of competitive sports participation. Key updates include considerations for athletes with asymptomatic or subclinical disease. This paper discusses COCIS criteria for risk evaluation in three clinical settings: suspected or stable coronary a…

AUDIENCE: The target audience for the key learning objectives of this Left-Ventricular Assist Device (LVAD) simulation are emergency medicine residents. Other team members such as attendings, nurses, pharmacists, and technicians could potentially be integrated. INTRODUCTION: Left ventricular assist devices (LVADs) are common bridge therapy for patients suffering from severe heart failure to cardiac transplant or destination therapy for non-transplant candidates.1 Emergency medicine physicians must be prepared for a variety of device complications that may result in an acute care presentation,…

Heart Failure Pharmacotherapy.

Pharmacotherapy for heart failure (HF) with reduced ejection fraction (HFrEF) is the foundation of management. The basis of pharmacotherapy for HFrEF includes 4 pillars: beta-blockers, renin angiotensin aldosterone system modulators, mineralocorticoid receptor antagonists, and sodium glucose cotransporter 2 inhibitors. These drug classes collectively reduce mortality, hospitalizations, and can improve functional and clinical status. Additional pharmacotherapy with loop diuretics and sometimes addition of thiazide diuretics at the core can reduce HF symptoms. Contemporary data from recent rando…

Carcinoid Heart Disease.

Carcinoid heart disease (CHD) is a rare but potentially life-threatening sequela of advanced neuroendocrine neoplasm with carcinoid syndrome. These tumors can secrete vasoactive substances of which serotonin is the most prevalent. Carcinoid heart disease typically involves the right-sided heart valves and eventually leads to right heart failure. Monitoring N-terminal pro-B-type natriuretic peptide and 5-hydroxyindoleacetic acid at diagnosis and during treatment, as well as cardiac echocardiogram, helps to screen for CHD. Many patients are not screened for this appropriately. Multidisciplinary …
