Pathological and Comprehensive Genetic Investigation of Autopsy Cases of Idiopathic Bradyarrhythmia.

BACKGROUND: Idiopathic bradyarrhythmia is considered to be due to pathological degeneration of the cardiac conduction system (CCS) during aging. There appears to have been no comprehensive genetic investigations in patients with idiopathic bradyarrhythmia. METHODS AND RESULTS: Ten autopsy cases with advanced bradyarrhythmia (6 men and 4 women; age: 70-94 years, 81.5±6.9 years; 5 cases each of sinus node dysfunction [SND] and complete atrioventricular block [CAVB]) were genetically investigated by using whole-exome sequencing. Morphometric analysis of the CCS was performed with sex-, age- and

Recurrent pacemaker-mediated arrhythmia with a right bundle branch block pattern in a patient with a cardiac resynchronization therapy device.

Under certain conditions, cardiovascular implantable electronic devices can be directly involved in initiating and sustaining pacemaker-mediated arrhythmias (PMA), of which repetitive reentrant ventriculoatrial synchrony (RRVAS) is the most well-known and common type. RRVAS, also known as pacemaker-mediated tachycardia, was commonly secondary to atrioventricular (AV) dissociation and subsequent ventriculoatrial (VA) conduction. RRVAS in a biventricular system is rare due to its less predisposition to the appearance of AV dissociation and subsequent VA conduction, but urgent in its adverse impa

Effects of a novel vesamicol receptor ligand, m-(iodobenzyl)trozamicol, on the canine isolated, blood-perfused atrioventricular node preparation.

m-(Iodobenzyl)trozamicol (MIBT) is a recently discovered vesamicol analogue. It has been shown that radiolabelled [125I]MIBT can be used as a marker of cholinergic innervation in the heart as well as in the brain. The purpose of this study was to analyze the direct effects of MIBT on the atrioventricular and intraventricular conduction in addition to the coronary blood flow using the canine isolated, blood-perfused atrioventricular node preparation. Intracoronary administration of MIBT suppressed the atrioventricular and intraventricular conduction, while it increased the coronary blood flow.

Clinicopathologic assessment of arrhythmias in a case of scleroderma heart disease with sudden death.

Electrophysiologic and histopathologic correlation has been carried out in a patient with scleroderma heart disease, affected by syncopal seizures, who died of recorded ventricular fibrillation. The electrophysiological investigation disclosed dysfunction of sinoatrial conduction, revealed by sinoatrial blocks and by an abnormal return cycle pattern after premature atrial beats. Atrial effective and functional refractory periods were increased and an unusual 'pseudo-Wenckebach' phenomenon between artificial stimulus and atrium was observed during atrial pacing. Intra-AV nodal conduction time w

Cooling for Hypoxic Ischemic Encephalopathy: From Evidence to Practice.

The cardiac conduction system (CCS) develops asymmetrically along the body axes. In heterotaxy syndrome - resulting from aberrant left-right axis formation - atrial and atrioventricular conduction defects can cause life-threatening arrhythmias. However, the developmental mechanisms regulating the atrioventricular conduction system (AVCS) disposition and integrity remain unclear. To investigate the etiology of AVCS malformations in laterality defects, we analyzed CCS development and function in mouse mutants for Cryptic and Lefty1, which are key regulators of Pitx2 in the left-right axis format

Analysis of peripheral nerve and autonomic nervous system function and the stage of microangiopathy in patients with secondary Raynaud's phenomenon in the course of connective tissue diseases.

BACKGROUND: The pathogenesis of secondary Raynaud's phenomenon (SRP) associated with connective tissue diseases (CTD) is not entirely understood. Nervous system dysfunction and microangiopathy are considered to be causes of this pathology. OBJECTIVES: Peripheral and autonomic nervous system function, the stage of microangiopathy, and the relationships between these in patients with SRP were analyzed. MATERIAL AND METHODS: In the study, 20 patients with CTD-related SRP and 30 healthy controls were subject to capillaroscopy, standard conduction velocity tests and conduction velocity distributi

Outcome measures for heart involvement in systemic sclerosis.

Primary myocardial involvement is common in SSc. Increasing evidence strongly suggests that this involvement is related to repeat focal ischaemic injury causing irreversible myocardial fibrosis. Clinically evident cardiac involvement is recognized to be a poor prognostic factor, thus pre-clinical identification is highly encouraged. Echocardiography, if possible coupled to pulsed tissue Doppler, is the cornerstone of heart assessment even if radionuclide ventriculography remains the gold standard for evaluation of ventricular function. Myocardial perfusion may be assessed by single photon emis

Ebstein's anomaly: a complex congenital heart defect.

Ebstein's anomaly is a complex, congenital heart defect characterized by a malformation of the tricuspid valve and right side of the heart. A variety of cardiac abnormalities are associated with Ebstein's anomaly, including atrial septal defect, conduction system abnormalities, patent foramen ovale, pulmonary stenosis or atresia, and ventricular septal defect. The clinical course of a patient with Ebstein's anomaly depends on the severity of the abnormalities present. Surgical repair of Ebstein's anomaly involves repair or replacement of the tricuspid valve and repair of any associated cardiac

Morphometric analysis of cardiac conduction fibers in horses and dogs, a comparative histological and immunohistochemical study with findings in human hearts.

The principal function of the ventricular conduction system is rapid electrical activation of the ventricles. The aim of this study is to conduct a morphometric study to pinpoint the morphological parameters that define cardiac conduction cells, allowing us to distinguish them from other cells. Five male horse hearts and five male dog hearts were used in the study. The hearts were fixed in a 5% formaldehyde solution. Histological sections of 5 μm thickness were acquired and stained with hematoxylin-eosin and Masson's trichrome and cardiac conduction cells and their junctions were identified by

The infarct Q wave: foretaste and aftertaste; presentiment and recall.

Since acute myocardial infarction involves a progressive disturbance of intramyocardial propagation the process might be regarded as a conduction disturbance: incomplete in the evolving phase; complete with Q wave inscription. Premature atrial excitation (PAE) can induce fascicular or bundle branch conduction delay, or complete functional block. Considered here are the concepts of foretaste: in which PAE reveals the infarct Q before it has appeared in regular sinus beats, and aftertaste: in which, following the healing of the myocardial infarction, the Q wave is recalled by PAE after it has di

[Pathophysiologic mechanisms of atrial rhythm disorders. II. Atrial flutter].

Atrial flutter is a common cardiac dysrhythmia which responds for half of the supraventricular tachycardias with exception of atrial fibrillation. Activation and entrainment mapping studies in man and in animals confirmed the reentry mechanism of atrial flutter and demonstrated that the reentry circuit is located in the right atrium. The most important anatomical structures which enable the occurrence of atrial flutter are crista terminalis, Eustachian valve/ridge and tricuspid annulus. In typical atrial flutter the impulse rotates "counterclockwise". According to the present knowledge the ree

Spectrum of supraventricular tachycardias.

The term "supraventricular tachycardia" includes a wide variety of arrhythmias with very different clinical and prognostic significance. Each supraventricular tachycardia has unique characteristics and each patient, even when similar arrhythmias are present, remains a person with specific manifestations. In 1988, physicians should not be satisfied with the diagnosis of supraventricular tachycardia. Knowledge acquired in recent years makes it possible to diagnose correctly the type of supraventricular tachycardia using quite simple means. The spectrum of clinical presentation and significance o
