The value of Pediatric Early Warning Score combined with SBAR in neonatal pneumonia nursing: A retrospective cohort study.

To observe the effect of the Pediatric Early Warning System (PEWS) score combined with the situation-background-assessment-recommendation (SBAR) shift communication system in neonates with severe pneumonia in the pediatric intensive care unit. A total of 230 neonates admitted to the pediatric intensive care unit of our hospital from January 2018 to January 2021 were enrolled in this study. Participants were divided into an experimental group (110 patients, PEWS score combined with SBAR shift communication system) and a control group (120 patients, routine diagnosis and treatment and shift chan

Examination of maternal near-miss experiences in the hospital setting among Black women in the United States.

INTRODUCTION: The high rate of maternal mortality among Black women in the United States continues to gain attention; yet research has not yet fully illuminated the precursors to these events, most impactful among them being "maternal near misses." A maternal near miss occurs when a woman nearly dies but survives a complication that occurred during pregnancy, childbirth, or within 42 days of termination of pregnancy. Researchers have focused on reviewing patient medical records after sentinel maternal events to help determine major contributors to them; however, qualitative studies with near-m

Legionellosis.

Legionella is a common cause of community- and hospital-acquired pneumonia. New information on the pathogenesis of infection and the host immune response is reviewed. Specialized laboratory tests, especially culture, are necessary for diagnosis since the clinical presentation is nonspecific. New antimicrobial agents and innovative approaches to disinfection of water distribution systems are presented.

Chemical weapon functional exercise--Cincinnati: observations and lessons learned from a "typical medium-sized" city's response to simulated terrorism utilizing Weapons of Mass Destruction.

In the wake of the September 11, 2001, attacks and the subsequent anthrax scare, there is growing concern about the United States' vulnerability to terrorist use of Weapons of Mass Destruction (WMD). As part of ongoing preparation for this terrible reality, many jurisdictions have been conducting simulated terrorist incidents to provide training for the public safety community, hospitals, and public health departments. As an example of this national effort to improve domestic preparedness for such events, a large scale, multi-jurisdictional chemical weapons drill was conducted in Cincinnati, O

Measles in India.

Measles is an important cause of morbidity and mortality among the children of India. In spite of this it is not considered a serious problem by the health authorities. Practically no cases are reported, and only those children with significant complications are hospitalized. Thus hospital records do not reflect the magnitude of the problem. In recent years attention has been focused on the evaluation of the morbidity and mortality due to measles in India. In the absence of reliable estimates, it is presumed that almost everyone in India suffers from measles before the age of 10 years. Hospita

Warmth When It Matters Most: Addressing the Overnight Needs of Homeless Emergency Room Patients.

Amid a growing housing and homelessness crisis in Hamilton, Ontario, an innovative four-way alliance responded with urgency and compassion to address a critical service gap: the overnight discharge of homeless patients from hospital emergency departments. Spearheaded by the Greater Hamilton Health Network Ontario Health Team, this cross-sector initiative united healthcare, social services, education and community partners to create a warming program at The Hub - a trauma-informed, low-barrier warming space operating nightly throughout the winter. Within one week, a tri-partner funding model wa

Citywide metagenomics reveals microbial community and resistome dynamics in urban wastewater.

Urban wastewater systems connect hospitals, residential communities, transport hubs, and wastewater treatment plants, creating opportunities for the dissemination of microorganisms and antibiotic resistance genes (ARGs). Here, we conduct a three-month, citywide metagenomic survey of wastewater in Xiamen, China, comprising 252 samples from seven hospital sites (n = 16), 27 residential sites (n = 55), 16 wastewater treatment plant sites (n = 159), and individual international flights (n = 22). Genome-resolved analyses reveal source-specific microbial community structures, resistome profiles, lin

Serious hazards of transfusion: evaluating the dangers of a wrong patient autologous salvaged blood in cardiac surgery.

BACKGROUND: The past half century has seen the near eradication of transfusion-associated hazards. Intraoperative cell salvage while widely used still poses significant risks and hazards due to human error. We report on a case in which blood collected from a patient with lung cancer was mistakenly administered to a patient undergoing cardiac surgery who should have received his own collected blood. The initial investigation found that the cause of the patient harm was violations of procedures by hospital personnel. A detailed investigation revealed that not only violations were the cause, but

The CI plus model: a qualitative study of innovative community isolation and enhanced resilience in urban Thailand during the COVID-19 crisis.

BACKGROUND: The COVID-19 pandemic exposed critical vulnerabilities in urban health systems, particularly in densely populated, low-resource communities. During the overwhelming third wave in Bangkok, Thailand, the Enhanced Community Isolation (CI Plus) Model emerged as an innovative response in the Yothi Medical Innovation District Community (YMIC). This qualitative study examined the mechanisms that enabled this vulnerable urban community to successfully implement a 170-bed isolation center and build resilience through community-based crisis response, providing insights for future pandemic pr

[Community-acquired methicillin-resistant Staphylococcus aureus infections in a hospital for acute diseases].

Community-acquired methicillin-resistant Staphylococcus aureus infections in a hospital for acute diseases. Methicillin-resistant Staphylococcus aureus (MRSA) is one of the most prevalent pathogens associated with nosocomial infections. However, most recently, MRSA has arisen as an emerging community pathogen, causing serious infections, mainly among young patients. We herein describe 33 cases of infections caused by community-acquired MRSA (C-MRSA), diagnosed between May 2005 and June 2006, at "Eva Perón" Hospital. The isolations were retrospectively studied. Methicillin resistance was confir

Mortality and functional morbidity after use of PALS/APLS by community physicians.

OBJECTIVES: To test the hypothesis that pediatric shock is a common cause of death and functional morbidity and that pediatric advanced life support (PALS)/advanced pediatric life support (APLS) resuscitation in the community hospital setting improves child health outcomes. METHODS: This study included all children consecutively transported to 5 regional, tertiary care children's hospitals over 4 years, and is a prospective cohort study comparing outcomes in children who did or did not receive PALS/APLS resuscitation in the community hospital. RESULTS: Shock occurred in 37% of the patients t

Epidemiology of early-onset bloodstream infection and implications for treatment.

UNLABELLED: HEALTH CARE-ASSOCIATED INFECTIONS: For over 35 years, infections have been divided into hospital acquired or community acquired. In 2002, in a study of bloodstream infections (BSIs), Friedman et al first suggested creating a new classification: health care-associated BSIs. Kollef et al furthered the concept of health care-associated infection in a 2005 population-based study of culture-positive pneumonia cases. Although the site of infection differed, Kollef et al's results supported Friedman et al's original concept. Then in 2006, Kollef et al reported a population-based study foc

Postoperative care in conflict-affected northwest Syria: a mixed-methods study assessing the perspectives of patients, health workers, and non-governmental organizations.

INTRODUCTION: Thirteen years of conflict have profoundly disrupted Syria's health system. In northwest Syria, constraints such as deliberate attacks on healthcare, chronic underfunding, and a fragmented health system have created a fragile surgical ecosystem in which health workers operate under extreme resource constraints, patients struggle to access care, and postoperative complications are increasingly likely. In this study, we aim to assess the burden of postoperative complications by examining patient-reported outcomes and barriers to care while integrating this data with insights from h

U.S. Primary Care Practice Capabilities Linked to Language Services for Patients with Limited English Proficiency.

BACKGROUND: Patients with limited English proficiency (LEP) experience more challenges with clinician communication than English-proficient patients. U.S. federal policies require the provision of language services, but little is known about language service availability in adult primary care practices nationally. OBJECTIVE: To identify primary care physician practice capabilities associated with the routine availability of language services for patients with LEP. DESIGN: Nationally representative physician practice survey data from the National Survey of Healthcare Organizations and Systems

How Does Integrated Care Do With, Not for, Neighbourhoods & Communities?

A notable development in recent years has been the emphasis on structuring integrated care around 'neighbourhoods' or 'places' and engaging 'communities' in the planning, design and delivery of support. Across the world, related principles are being used to inform national and regional policies and to develop local practice innovations. Despite such interest, community and neighbourhood working can feel a radical endeavour which clashes with established structures, relationships, investments and skills. A group of international experts from policy, practice, communities and research came toget

Successful treatment with inhaled corticosteroid/long-acting β2-agonist in a case of allergic bronchopulmonary mycosis caused by Schizophyllumcommune.

Allergic bronchopulmonary mycosis (ABPM) is a chronic immune-mediated pulmonary disease, which is caused by fungal infection of the airways. Aspergillus species are the main causative fungi and standard treatment typically comprises systemic corticosteroid therapy with or without adjunct antifungal agents. We describe our experience with a case of ABPM caused by Schizophyllum commune (S. commune), with satisfactory response to treatment with a combination of an inhaled corticosteroid and a long-acting β 2-agonist. The patient was a 61-year-old man who was referred to our hospital with dry coug

Associations between hospital characteristics and the delivery of non-cancer palliative care: a nationwide retrospective study in Taiwan.

BACKGROUND: As the population ages, the need for palliative care among patients with non-cancer conditions has increased. Although the benefits of palliative care for non-cancer conditions are well established, integration of such care remains limited across many health systems. Taiwan's universal health system offers a unique opportunity to examine how hospital characteristics influence the delivery of non-cancer palliative care. METHODS: We conducted a retrospective nationwide analysis of all hospitals providing palliative care through either dedicated palliative care units (PCUs) or pallia
