Commentary: a patient-centered theory of satisfaction.

The Primary Provider Theory holds that patient satisfaction occurs at the nexus of provider power and patient expectations. More specifically, patient satisfaction is principally the function of an underlying network of interrelated satisfaction constructs--satisfaction with the primary provider, waiting for the provider, and satisfaction with the provider's assistant(s). Hierarchically linked to patient-centered expectations of provider value, the Theory specifies that primary providers offer the greatest clinical utility to patients. The Theory is operationalized by patient-centered measures

Implementation of an Innovative Pediatric Hospital Medicine Education Series.

OBJECTIVE: Limitations on resident duty hours require formal education programs to be high-yield and impactful. Hospital medicine (HM) topics provide the foundation for inpatient pediatric knowledge pertinent to pediatric residents and medical students. Our primary objective was to describe the creation of an innovative pediatric HM curriculum designed to increase learners’ medical knowledge and their confidence in communicating with patients and families about these topics; our secondary objective was to evaluate the level of innovation of the conference sessions perceived by the learners. The drug abusing adolescent: clinical issues.

Clinical intervention with adolescent substance abusers is a complex issue. Disputes remain regarding the precise nature of the disorder. Treatment of the adolescent is complicated by the normal development tasks of this age, particularly denial of possible consequences and adoption of attendant risk-taking behaviors. Treatment begins with a complete drug history and must not neglect the adolescent's family and other support system. The mental health professional, especially the psychiatrist, can make a unique contribution in this area, despite the "bad press" he has so often received by the r

From hospital to home following a lower limb amputation: A focus group study of healthcare professionals' views and experiences with transitioning.

INTRODUCTION: Patients undergoing major lower limb amputation often have multiple comorbidities, requiring care from numerous healthcare professionals (HCPs). Furthermore, they may experience physical, medical and practical challenges post-amputation. Patients with complex needs are particularly vulnerable during care transitions and may benefit from integrated care models. AIM: This study aimed to explore HCPs' views and experiences regarding the transition process after implementing an integrated care model for patients with lower limb amputation. METHODS: We conducted two focus groups wit

[Cost and Revenue Relationship in Orthopaedic and Trauma Surgery Patients in Relation to Body Mass Index].

BACKGROUND: Growing numbers of patients in orthopaedic and trauma surgery are obese. The risks involved are e.g. surgical complications, higher costs for longer hospital stays or special operating tables. It is a moot point whether revenues in the German DRG system cover the individual costs in relation to patients' body mass index (BMI) and in which area of hospital care potentially higher costs occur. MATERIAL AND METHODS: Data related to BMI, individual costs and revenues were extracted from the hospital information system for 13,833 patients of a large hospital who were operated in 2007 t

Toward instituting a chaperone policy in outpatient pediatric clinics.

OBJECTIVES: We sought to evaluate child, parent and medical provider preferences for chaperones for outpatient encounters and to evaluate the acceptability and frequency of utilization following institution of a chaperone policy. Secondarily, we sought to understand what medical history and examinations teens consider "sensitive." DESIGN: We conducted an observational study 1 month before and 1 month after institution of outpatient clinic chaperone policy. Post clinic questionnaires were used for patients > or =12 years old, parents, and clinicians. A research assistant observed chaperone use

Concurrent and predictive validity of a self-reported measure of medication adherence.

Adherence to the medical regimen continues to rank as a major clinical problem in the management of patients with essential hypertension, as in other conditions treated with drugs and life-style modification. This article reviews the psychometric properties and tests the concurrent and predictive validity of a structured four-item self-reported adherence measure (alpha reliability = 0.61), which can be easily integrated into the medical visit. Items in the scale address barriers to medication-taking and permit the health care provider to reinforce positive adherence behaviors. Data on patient

Operationalizing the new DEA exception: A novel process for dispensing of methadone for opioid use disorder at discharge from acute care settings.

PURPOSE: To describe one strategy for dispensing of methadone at emergency department (ED) and hospital discharge implemented within 2 urban academic medical centers. SUMMARY: Expanding access to medications for opioid use disorder (OUD) is a national priority. ED visits and hospitalizations offer an opportunity to initiate or continue these lifesaving medications, including methadone and buprenorphine. However, federal regulations governing methadone treatment and significant gaps in treatment availability have made continuing methadone upon ED or hospital discharge challenging. To address t

Trends in Substance Use Disorder-related Admissions at a Safety-net Hospital, 2008 - 2020.

OBJECTIVES: Safety-net hospitals disproportionately care for people with substance use disorders (SUDs), yet little is known about trends in hospital admissions related to specific substances. This study uses electronic health record data to describe trends in substance-specific admissions at a Midwest urban safety-net hospital. METHODS: We included all admissions from 2008 through 2020 and defined them as non-SUD (N = 154,477) or SUD-related (N = 63,667). We described patient characteristics and trends in substance-specific admissions. We estimated the association of SUD diagnoses with disch

The DRIP Criteria: Reducing the Frequency of Peripheral Intravenous Catheter Insertion in Hospitalized Patients.

BACKGROUND: Patient harm attributed to invasive devices is a global concern. Around 18% to 54% of all catheter-related hospital-acquired bloodstream infections (HABSIs) are attributable to peripheral intravenous cannulas (PIVCs). Between 4% and 28% of PIVCs placed in hospitalized patients and up to 50% of emergency department (ED) PIVCs are not used. Avoiding insertion of the "just in case" PIVC where safe to do so has potential to reduce patient risk. TOOL DEVELOPMENT: The DRIP mnemonic (Deterioration, Rehydration, Intravenous medications, Procedure) was designed around four simple questions

Perspectives of healthcare workers on the integration of overdose detection technologies in acute care settings.

BACKGROUND: People who use drugs (PWUD) face disproportionately high rates of hospitalizations and patient-initiated discharge (leaving against medical advice), explained by a combination of stigma, withdrawal, judgment, blame, and improper pain management. In addition, evidence has shown that despite abstinence-based policies within healthcare settings, PWUD continue to use their substances in healthcare environments often hidden away from hospital staff, resulting in fatalities. Various novel overdose detection technologies (ODTs) have been developed with early adoption in a few settings to

Reflections from a Medical Malpractice Defense Attorney: Insights on Avoiding Claims and Lawsuits.

Good decision-making, solid notes that show care and a reasonable thought process, and skillful interpersonal dealings with patients and loved ones are keys to avoiding claims and lawsuits related to care in the emergency department (ED). This means taking measures to control the brisk assembly line of work in the ED so that important steps such as assessment, testing, treatment, and communication promptly occur without any dilution in quality. It is crucial that the physician in the ED displays excellent bedside manner, including dealing with an increasing challenging patient population and t

Telephonic Outreach to Engage Patients with Substance Use Disorder Post-Hospitalization During the COVID-19 Pandemic.

During the initial COVID-19 surge, one public hospital in NYC updated their post-discharge outreach approach for patients with substance use disorder, as part of the CATCH (Consult for Addiction Treatment and Care in Hospitals) program. Beginning April 1, 2020, three peers and two addiction counselors attempted telephonic outreach to patients who received a CATCH consultation during hospitalization from program launch (October 7, 2019) through March 31, 2020 (n = 329). Outreach calls could include counseling, in-depth peer support, and referrals to substance use services (SUS)-a significant ex

[Borderline Personality Disorder in the Somatic Hospital Setting].

Borderline Personality Disorder in the Somatic Hospital Setting Abstract. The Borderline Personality Disorder is a common mental illness characterized by emotional instability, interactional difficulties, an unstable sense of self and impulsive behaviour which is often self-destructive. Since these problems frequently occur in the doctor-patient relationship, patients might be regarded as "difficult" or even manipulative. This may lead to stigmatization and medical undertreatment when due to common mental and somatic comorbidities the condition requires thorough high-standard medical care. Whe

What fuels suboptimal care of peripheral intravenous catheter-related infections in hospitals? A qualitative study of decision-making among Spanish nurses.

BACKGROUND: Peripheral intravenous catheters (PIVC) are commonly used in hospital worldwide. However, PIVC are not exempt from complications. Catheter-related bloodstream infections (CRBSI) increase morbidity and mortality rates, and costs for the healthcare organization. PIVC care is shaped by the complex mix of professional and organizational culture, such as knowledge gaps, low perception of impact of PIVCs on patient safety, or lack of hospital guidelines. AIM: To explore determinants of decision-making about the prevention of PIVC-BSI among nurses in Spanish hospitals. METHODS: We condu

Predictors of Opioid and Alcohol Pharmacotherapy Initiation at Hospital Discharge Among Patients Seen by an Inpatient Addiction Consult Service.

BACKGROUND: Medications for opioid use disorder (MOUD) and alcohol use disorder (MAUD) are effective and under-prescribed. Hospital-based addiction consult services can engage out-of-treatment adults in addictions care. Understanding which patients are most likely to initiate MOUD and MAUD can inform interventions and deepen understanding of hospitals' role in addressing substance use disorders (SUD). OBJECTIVE: Determine patient- and consult-service level characteristics associated with MOUD/MAUD initiation during hospitalization. METHODS: We analyzed data from a study of the Improving Addi
