OBJECTIVE: To investigate the impact of mineral metabolism, renal function, months on dialysis and months since transplant for predicting intact parathyroid hormone (iPTH) levels in a cohort of patients who had undergone their first renal graft with estimated glomerular filtration rates (eGFRs) of 30-60 and >60 ml/min/1.73 m2.
MATERIAL AND METHODS: One hundred and twenty-eight patients (mean age 56.0 +/- 14.6 years) with an eGFR of >30 ml/min/1.73 m2 were included. The median time since transplant was 88.6 months (range 2.8-403.2 months). Blood samples were collected for measurement of iPTH,

BACKGROUND: With the use of routine calcium evaluation, the incidence of primary hyperparathyroidism (pHPT) has considerably increased. The aim of our study was the assessment of the morbidity and the outcome of surgically treated patients with pHPT.
METHODS: The charts of 279 patients (204 female, 75 male, median age: 58 years) who underwent surgery for pHPT between 1989 and 1999 were retrospectively reviewed. A follow-up was carried out on 235 patients after a mean period of 3.6 years (0.2-10 years).
RESULTS: In 148 patients a unilateral, and in 130 patients a bilateral approach was taken.…

Primary, secondary, and tertiary hyperparathyroidism have evolved since their original description. What was once a debilitating disease has now become one with few symptoms on initial presentation. Complications from these disorders have decreased significantly because of earlier detection. Improved management of patients with chronic renal disease has also limited complications among those with secondary and tertiary hyperparathyroidism. Appropriate management is essential even in the early phase of the disorder to minimize the morbidities that may result if left untreated.

Multiple endocrine neoplasia (MEN) syndromes comprise the group of heritable endocrinopathies, MEN 1, MEN 2A, and MEN 2B. Primary hyperparathyroidism caused by multiglandular involvement is usually the initial manifestation in MEN 1, occurring in more than 90% of patients. In patients with MEN 2A, hyperparathyroidism develops less commonly and is usually milder than in MEN 1. Advances in genetics and molecular biology aid in confirming the diagnosis and screening relatives who are carriers or at risk for the disease. Surgery plays an important role in the management of hyperparathyroidism in b…

As molecular biology and genetic mapping receive wider application to human disease, genetic alterations have been identified with increased frequency in some patients with primary hyperparathyroidism(HPT). These alterations have been found in molecules related to cellular signaling and growth (RET proto-oncogene)and in tumor suppressors that control cell cycle progression and gene transcription (cyclin D1 and the MEN1 gene product. Although primary HPT can usually be treated surgically without knowledge of which specific genetic alteration has occurred, this information may assist clinicians …

Surgery was performed in 161 patients with primary hyperparathyroidism (PHPT). Eight of 161 patients underwent repeated surgeries. In 6 patients persistent PHPT was associated with hyperplasia of parathyroid glands (PTG), in 1 -- with adenoma of PTG missed during the first surgery. In 4 of them hyperplastic PTG are located in anterior mediastinum, in 1 -- in posterior. In 2 patients with verified hyperplasia of PTG repeated surgeries were limited in the region of the neck. During repeated surgery in one patient recurrence of PTG cancer was diagnosed. Specific group consisted of 8 patients with…

A thorough diagnostic examination is essential before providing endodontic treatment. The sequence of diagnostic procedures must begin with a well-organized review of the medical history. In the early screening process, a health history that reveals a systemic disorder must be investigated further because it may have a significant impact on the dental diagnosis and ultimate endodontic treatment. There are a number of systemic diseases that can cause bone lesions throughout the body. Chronic renal failure is one disorder that may stimulate a secondary hyperparathyroidism that can cause a variet…

Primary hyperparathyroidism (pHPT) is cited as one of diseases which cause secondary osteoporosis and predominantly affects cortical bone. This disease occurs frequently during the postmenopausal period. Parathyroidectomy (PTX) is the only option available for the radical cure of pHPT and brings about marked increase in bone mass even in postmenopausal women. On the other hand, this disease is now recognized as a disorder with few overt manifestations, presenting as asymptomatic hypercalcemia. Even in these cases, a considerable increase in bone mass can be obtained by PTX. Bisphosphonate as w…

OBJECTIVE: Primary hyperparathyroidism (1HPT) is now being diagnosed with increasing frequency. Simultaneously there has been an apparent change in the presentation of the disease and indications for surgery. The aim of this study was to examine the clinical presentation, indications for surgery, and outcomes of neck explorations for primary hyperparathyroidism.
METHODS: This study was carried out over a 12 year period, January 1990 to April 2002 at Jordan University Hospital, Amman, Jordan. Information on the indications, procedure performed, pathology and complications of all neck explorati…

Most patients with renal failure maintained on chronic dialysis have elevated parathyroid hormone (PTH) levels and PTH-mediated bone disease (secondary hyperparathyroidism [sHPT]). Elevated PTH production in this setting represents a progressive, exaggerated physiologic response to hypocalcemia by the parathyroid glands, and generalized growth of the parathyroids is an adaptive response to chronic stimulation. Effective medical strategies to reduce PTH secretion and PTH-mediated bone turnover in sHPT (eg, controlling hyperphosphatemia, normalizing serum calcium, and administering vitamin D ana…

It has been assumed that uremic parathyroid hyperplasia predominantly involves generalized polyclonal cellular proliferation, because of multigland involvement. However, the majority of uremic hyperplasias resected from patients with secondary hyperparathyroidism are monoclonal. The specific acquired genetic abnormalities would have a great role in providing this selective advantage of the parathyroid hyperplasia.

We reported that selective PEIT in parathyroid glands guided by color doppler flow mapping is effective for severe secondary hyperparathyroidism (2HPT) in chronic dialysis patients. According to the recent Survey by the Japanese Working Group on PEIT of Parathyroid, more than 600 patients have been treated by PEIT in Japan by September, 1998. Based on the results of this survey and based on pathophysiological consideration, Working Group proposed a guideline of selective PEIT of parathyroid. I give a brief summary here. But PEIT has a risk of side effects such as recurrent nerve palsy. Recentl…

Secondary hyperparathyroidism with marked parathyroid hyperplasia is the major type of renal osteodystrophy. In addition to classic stimuli for parathyroid hormone (PTH) such as decreased concentrations of ionized calcium and 1,25-dihydroxyvitamin D(3) (1,25(OH)(2)D(3), calcitriol), several mechanisms have been suggested. Those include decreased density of calcitriol and calcium-sensing receptors, as well as the direct action of phosphate. Skeletal resistance to PTH was initially recognized as a blunted calcaemic action of PTH, which has been considered another stimulus for PTH secretion. Once…

Normocalcemic primary hyperparathyroidism is an uncommon condition usually found among patients with urinary stones. To the best of our knowledge, this is the first case of a patient with normocalcemic primary hyperparathyroidism presenting with bilateral femoral neck fracture. A 45-year-old man had pain both hip joints and limping in for several months. Vertical fractures of bilateral femoral necks were found by radiographs. Laboratory findings showed a normal value of total serum calcium but elevated serum ionized calcium. Serum parathyroid hormone and alkaline phosphatase levels were elevat…
