Effect of infant immunisation on childhood mortality in rural Bangladesh: analysis of health and demographic surveillance data.

BACKGROUND: In developing countries, immunisation programmes must compete with other strategies to improve public health and quality of life. Studies of long-term effects of immunisation programmes are rare. We assessed associations between vaccinations and mortality over 15 years after the introduction of routine infant immunisation programmes in Matlab, Bangladesh. METHODS: We analysed data recorded in a comprehensive health and demographic surveillance system from 1986 to 2001. We did univariate analyses and assessed vaccinations as independent factors with other variables in Cox models wi

The effect of air pollution on infant mortality appears specific for respiratory causes in the postneonatal period.

To examine the association between individual lifetime measures of mean exposure to air pollution and postneonatal respiratory deaths, we ha … lutant and mortality from other causes. These results indicate that the effects of air pollution on infant mortality are specific for respiratory causes in the postneonatal period, are independent of socioeconomic factors, and are not mediated by birth weight or gestational age.

Effects of air pollution on postneonatal infant mortality among firstborn infants in Seoul, Korea: case-crossover and time-series analyses.

Infants are known to be susceptible to the adverse health effects of ambient air pollution. The authors examined the relationship between air pollution and postneonatal mortality from all causes among firstborn infants in Seoul, Korea, during 1999-2003, using both case-crossover and time-series analyses. Using a bidirectional control-sampling approach, the authors compared the effects of various types of air pollution on postneonatal mortality. The relative risk of postneonatal mortality from all causes was 1.000 (95% confidence interval [CI] = 0.998-1.002) for particulate matter with a diamet

Trends and disparities in infant mortality from intrauterine hypoxia and birth asphyxia in the United States, 1999-2023.

INTRODUCTION: Despite continued advances in prenatal and neonatal care, birth asphyxia and intrauterine hypoxia remain significant causes of preventable mortality in the early neonatal period. Fetal, maternal, and health-system factors influence outcomes immediately after birth. This study aims to analyze trends in infant mortality related to these conditions and examine differences across demographic groups. METHODS: Data on infant mortality related to intrauterine hypoxia or birth asphyxia were obtained from the Centers for Disease Control and Prevention Wide-ranging Online Data for Epidemi…

Infant Mortality Rates of Non-Hispanic White and African-American Term Births: The Effect of Acknowledged Fathers' Education.

OBJECTIVES: To determine the extent to which acknowledged fathers' education attainment is associated with the infant mortality rate of term births to African-American and non-Hispanic White women. We hypothesized that African American (compared to non-Hispanic White) term births would have a greater percentage of first-year deaths attributable to low paternal education attainment. METHODS: Stratified and multivariable regression analyses were performed on the 2018-2019 National Center for Health Statistics US cohort-linked live birth-infant death data files. Only term (37-42 weeks) singleton…

Mapping infant streptococcal mortality burden in low- and middle-income countries: temporal trends, geospatial patterns, and socioeconomic determinants.

UNLABELLED: Streptococcal mortality burden threatens children's health in low- and middle-income countries (LMICs), yet their temporal trends, geographic disparities, and socioeconomic determinants remain unclear. Using the MICROBE database and World Bank data, we conducted a cross-sectional study to assess infant mortality burden of group A Streptococcus (GAS), group B Streptococcus (GBS), Streptococcus pneumoniae, and other streptococci in LMICs. Joinpoint regression estimated the average annual percent change (AAPC). Frontier analysis assessed the potential reduction in antimicrobial resist…

Systemic racism and Non-Hispanic Black to Non-Hispanic White disparities in infant mortality at the county level.

OBJECTIVE: To use a novel measure of systemic racism to examine its relationship with Non-Hispanic Black (NHB) to Non-Hispanic White (NHW) racial disparities in infant mortality across U.S. counties. STUDY DESIGN: In this cross-sectional study, a composite, multi-dimensional measure of systemic racism at the county level was developed using confirmatory factor analysis based on indicators across five dimensions of systemic racism. Using linear regression analysis, we examined the relationship between the systemic racism factor scores and the NHB to NHW racial disparities in county-level infan…

Sexually transmitted infections during pregnancy and subsequent risk of stillbirth and infant mortality in Kenya: a prospective study.

OBJECTIVES: We evaluated the relationship of sexually transmitted infections (STIs) and genital infections during pregnancy and subsequent risk for infant mortality and stillbirth. METHODS: This was a nested longitudinal analysis using data from a study of peripartum HIV acquisition in Kenya. In the parent study, HIV-uninfected women were enrolled during pregnancy and followed until 9 months postpartum. For this analysis, women who tested positive for HIV at any point, had a non-singleton pregnancy or a spontaneous abortion <20 weeks were excluded. At enrolment, laboratory methods were used t…

Ljungan virus: a commentary on its association with fetal and infant morbidity and mortality in animals and humans.

Epidemiologic and experimental data support the notion that Ljungan virus (LV), endemic in some rodent populations in Sweden, Denmark, and the United States, can cause morbidity and mortality in animals and humans. LV infection can cause type I diabetes mellitus, myocarditis, and encephalitis in bank voles and experimental mice, and lemmings. Mouse dams infected with LV experience high rates of stillbirth that may persist across generations, and their fetuses may develop cranial, brain, and limb malformations. In humans, epidemiologic and serologic data suggest that LV infection correlates wit

Reinterpreting the effects of maternal smoking on infant birthweight and perinatal mortality: a multivariate approach to birthweight standardization.

Infants of women who smoke during pregnancy have lower birthweights and have been observed to have higher rates of perinatal mortality than infants of non-smokers. It is not clear whether this increased risk of mortality is due to an excess of small births among smokers or to an independent effect of smoking. Although infants of smokers have overall higher mortality rates than non-smokers, low birthweight (< 2500 g) infants of smokers have lower mortality rates than low birthweight infants of non-smokers. However, comparison of birthweight-specific mortality between two groups is problematic w
