Infliximab treatment of pediatric refractory Crohn's disease: a case report.

Infliximab is a monoclonal antibody that targets TNF-alpha and has been shown to be effective for the management of steroid-dependent or refractory Crohn's disease. It is an effective therapy in adult patients, but experience in children is limited. We report a case of Crohn's disease which was refractory to the conventional treatment. A 14-year-old boy was admitted to the hospital with arthralgia and oral and perianal lesions. On physical examination his body weight was below the 3rd percentile, and height was between the 3rd-10th percentiles. He had elevated erythrocyte sedimentation rate an…

Successful treatment of long-standing neuro-Behçet's disease with infliximab.

Behçet's disease is a chronic relapsing form of vasculitis affecting multiple organs. Central nervous system involvement is a common presentation. Tumor necrosis factor-alpha (TNF-alpha) is considered to play a major role in the disease pathogenesis. We describe a patient with a long history of neuro-Behçet's disease who showed a remarkable response to infliximab therapy. Given the variable and limited success of other interventions, the use of anti-TNF-alpha therapy seems to be effective for patients with refractory disease.

Drug-induced agranulocytosis during treatment with infliximab in enteropathic spondyloarthropathy.

Agranulocytosis is a disorder characterized by a severe decrease in the number of granulocytes in blood, that frequently occurs as an adverse reaction to some drugs. By now, there are no reports in literature of agranulocytosis caused by tumur necrosis factor-alpha blockers. We describe the case of a 20-year-old Caucasian male affected by enteropathic (Crohn's disease) spondyloarthropathy HLA B27 negative, successfully treated with infliximab. After the second infliximab infusion, he was found to have a severe transient neutropenia (0.5 x 10(9)/L). Routine serum chemistry and full blood cell c…

Therapy of progressive rheumatoid arthritis-associated corneal ulceration with infliximab.

PURPOSE: To review the relative efficacy of the TNFalpha antagonist infliximab in treating 3 patients with rheumatoid arthritis-associated peripheral ulcerative keratitis. METHODS: Review of the clinical course of 3 nonconsecutive patients with progressive rheumatoid arthritis-associated peripheral ulcerative keratitis who were initially treated with conventional immunosuppressant therapy and subsequently treated with infliximab. RESULTS: All 3 patients experienced progressive corneal ulceration on oral prednisone and weekly oral or intramuscular methotrexate. They all demonstrated a marked …

Familial Mediterranean fever responds well to infliximab: single case experience.

The most common arthritic involvement in familial Mediterranean fever (FMF) is acute recurrent monoarthritis; however, sometimes spondyloarthropathy-like findings or typical ankylosing spondylitis may also ensue. Reported here is our favorable experience with infliximab in an FMF patient who had been resistant to colchicine and disease-modifying antirheumatic drugs (sulfasalazine and methotrexate) treatments. A 72-week follow-up of the patient yielded complete remission of the febrile abdominal episodes, and spondylitis responded well. The patient's bilateral aseptic necrosis of the femoral he…

Crohn's disease: future anti-tumor necrosis factor therapies beyond infliximab.

The dramatic benefit experienced by many Crohn's disease patients treated with the anti-tumor necrosis factor-alpha (TNF-alpha) antibody infliximab underscores the centrality of this cytokine in the pathogenesis of Crohn's disease and the potential use of the therapeutic strategy of blocking TNF-alpha in this disease. In the hopes of emulating and improving on the success of infliximab, numerous strategies are being devised and studied to inhibit the actions of TNF-alpha. This article focuses on those agents, other than infliximab, which target TNF-alpha to treat inflammatory bowel disease as …

Infliximab treatment for steroid-refractory acute graft-versus-host disease.

BACKGROUND AND OBJECTIVES: Tumor necrosis factor a is one of the principal cytokines involved in the pathogenesis of acute graft-versus-host-disease (GVHD). Infliximab is an antibody to this cytokine. DESIGN AND METHODS: We performed a retrospective analysis to evaluate the activity of infliximab in 32 patients with severe steroid-refractory acute GVHD. The patients received a median of 3 weekly courses of infliximab. The main organs involved in the patients were skin (n=2) liver (n=1), bowel (n=19), liver and bowel at the same stage (n=10). RESULTS: Nineteen out 32 patients (59%) responded …

Effectiveness of biosimilar infliximab CT-P13 compared to originator infliximab in biological-naïve patients with rheumatoid arthritis and axial spondyloarthritis: data from the Portuguese Register.

OBJECTIVES: To compare the effectiveness of the infliximab biosimilar CT-P13 with originator infliximab over 24 months of follow-up in biological-naïve patients with rheumatoid arthritis (RA) and axial spondyloarthritis (axSpA). METHODS: Biological-naïve patients from the Rheumatic Diseases Portuguese Register (Reuma.pt), with a clinical diagnosis of RA or axSpA, who were starting either the infliximab biosimilar CT-P13 or the originator infliximab after 2014 (date of market entry of CT-P13 in Portugal), were included. Patients on biosimilar and originator were compared regarding different re…

An open-label pilot study of the effectiveness of adalimumab in patients with rheumatoid arthritis and previous infliximab treatment: relationship to reasons for failure and anti-infliximab antibody status.

This prospective open-label pilot study evaluated the effectiveness and safety of adalimumab and the relationship to antibodies against infliximab (IFX) in adult patients with active rheumatoid arthritis (RA) who had been treated previously with IFX and experienced treatment failure owing to lack or loss of response or intolerance. Patients self-administered adalimumab 40 mg subcutaneously every other week for 16 weeks, followed by maintenance therapy for up to Week 56. Measures of effectiveness included American College of Rheumatology (ACR) and European League Against Rheumatism (EULAR) resp…

Potential infliximab-induced Kounis syndrome in a patient with metastatic melanoma.

BACKGROUND: Kounis syndrome is a rare type of allergic or hypersensitivity-induced acute coronary syndrome. The release of numerous inflammatory mediators induces vasospasms and potential thrombosis leading to myocardial infarction. CASE HISTORY: We present the case of a 65-year-old woman with metastatic melanoma who experienced chest tightness and dyspnea after her 3rd infliximab infusion and who had transitory ST elevations. Suspecting allergic reactions and acute coronary syndrome she was simultaneously treated for both conditions with complete resolution of symptoms and electrocardiograph…

A Challenging Case of Advanced Cardiac Sarcoidosis Refractory to Infliximab Therapy.

Cardiac sarcoidosis can be life-threatening, and lack of response to immunosuppressive therapy remains a significant challenge in active disease. We present a case of refractory cardiac sarcoidosis despite third-line immunosuppression (infliximab). Ongoing multimodality assessment is essential for managing refractory cardiac sarcoidosis.
