COVID-19 Impact on Cerebrospinal Fluid Diversion: A Single Institution Experience.

INTRODUCTION: Several studies have demonstrated that the absolute numbers of select surgical interventions for myocardial infarction, stroke, and appendicitis decreased during the COVID-19 pandemic, possibly due to overall decreased hospital presentation. We sought to identify if this pattern was also true for children with hydrocephalus and cerebrospinal fluid (CSF) diversion procedures. We hypothesized that there would be a detectable decrease in CSF diversion procedures performed during the COVID-19 pandemic, as compared to a pre-COVID-19 baseline. METHODS: A chart review of all patients t

Readmission after neurosurgical intervention in epilepsy: A nationwide cohort analysis.

OBJECTIVE: Hospital readmissions result in increased health care costs and are associated with worse outcomes after neurosurgical intervention. Understanding factors associated with readmissions will inform future studies aimed at improving quality of care in those with epilepsy. METHODS: Patients of all ages with epilepsy who underwent a neurosurgical intervention were identified in the 2014 Nationwide Readmissions Database, a nationally representative dataset containing data from roughly 17 million US hospital discharges. Diagnosis of epilepsy was based on International Classification of Di

Contemporary Analysis of Calculous Nephrectomy Utilization and Outcomes in the United States.

Introduction: Calculous nephrectomy was a mainstay of treatment of complex upper tract stone disease up until the 1970s, but data on its contemporary utilization in the current era of rising rates of stone disease are lacking. We characterized the nationwide utilization and outcomes for calculous nephrectomy in the United States. Patients and Methods: The National/Nationwide Inpatient Sample databases for 2001 to 2014 were queried for adults with a principal diagnosis of upper urinary tract calculi (UUTCs), who underwent nephrectomy as well as other inpatient surgeries for UUTCs. Per-populatio

Worldwide diffusion of robotic approach in general surgery.

The robotic system has overcome some laparoscopic shortcomings, such as stereoscopic 3D-magnified vision, a stable camera and flexible and tremor filtering manipulation. Therefore, robotic surgery has made many steps forward gaining more diffusion in the field of general surgery and this trend was associated with a decrease in laparoscopic surgery. Although the safety and effectiveness of robotic surgical systems in most procedures has been confirmed, some difficult procedures have not been widely performed but rather limited to a few highly specialized centers. This study describes the worldw

Excitation-light blocking filter for photoimmunotherapy tailored to the ORBEYE 4K 3D exoscope.

BACKGROUND: Photoimmunotherapy, which involves combining targeted antibodies with light-activatable dyes such as IRDye700DX (IR700), is an emerging cancer treatment modality. Cetuximab-based photoimmunotherapy is approved for the treatment of unresectable head and neck cancer and has been covered by the national health insurance in Japan since 2021. Use of conventional ceiling surgical lighting during photoimmunotherapy is problematic due to concerns about potential cytotoxic effects. METHODS: To ensure safe surgical application, we developed a custom short-pass filter for the ORBEYE surgical

Trends in and Factors Associated With Surgical Management for Closed Clavicle Fractures.

INTRODUCTION: Clavicle fractures are common and have historically been managed nonsurgically. Over time, there are increasing surgical considerations for such injuries. Nonetheless, trends over the past decade are not well characterized. METHODS: Adult patients presenting for first diagnosis of clavicle fractures were identified from the 2010 to 2020 PearlDiver M151 database. Patients were defined to have undergone surgical management if they received clavicular open reduction and internal fixation within 90 days after fracture diagnosis. Patient age, sex, Elixhauser Comorbidity Index (ECI),

Value of Inpatient Neuromodulation: A National Analysis of Paddle Spinal Cord Stimulation Outcomes.

BACKGROUND AND OBJECTIVES: Spinal cord stimulation (SCS) effectively manages chronic pain and degenerative spine conditions. Paddle SCS often necessitates inpatient care because of surgical complexity, yet the impact of procedural setting on outcomes, costs, and disparities remains unclear. This study evaluates total costs, 90-day readmission and complication rates, and socioeconomic disparities in paddle SCS delivery. METHODS: Using Merative MarketScan data (2007-2023), we retrospectively identified adults undergoing initial paddle lead SCS implantation. Outcomes were total costs, 90-day rea

The Epidemiology of Radial Tunnel Syndrome and Its Overlap With Lateral Epicondylitis.

PURPOSE: Radial tunnel syndrome (RTS) is characterized by nerve compression affecting the posterior interosseous nerve branch in the forearm, and its symptoms often overlap with those of lateral epicondylitis (LE). The purpose of this study was to examine the epidemiology of RTS, frequency of injections and surgical release, and overlap of RTS with LE. METHODS: We queried the PearlDiver database to identify RTS in patients older than 18 years. Demographic data, diagnostic or therapeutic injection within 30 days of diagnosis, surgical release within 1 year of diagnosis, and 90-day postoperativ

Emergency department utilization and predictors of mortality for inpatient inguinal hernia repairs.

BACKGROUND: Although inguinal hernias are common surgical diagnoses, minimally symptomatic patients are often not scheduled for repairs and are asked to seek medical attention if they develop symptoms. We investigated factors associated with emergency department (ED) utilization for inguinal hernia repairs and determined whether ED utilization affected mortality for this otherwise electively treated condition. METHODS: We performed a retrospective analysis of the 2009-2013 Nationwide Inpatient Sample to identify patients who presented through the ED and were then admitted for unilateral ingui

Laparoscopic Technique in the Management of High Anorectal Malformations: A Propensity Score-Matched Outcome Study Using a Large Inpatient Database.

Background: Posterior sagittal anorectoplasty (PSARP) has been the standard for management of children with high anorectal malformations (ARMs). Recently, there has been an increase in the use of laparoscopy in its management. We analyzed the outcomes of laparoscopically assisted anorectal pull-through (LAARP) compared to PSARP using a large inpatient database. Methods: Kids' Inpatient Database was analyzed for ARM (ICD-9-CM 751.2) between 1997 and 2012. Perineal fistulas and low/intermediate ARM were excluded. Propensity score (PS)-matched analyses were performed using 37 variables. Cases wer

Editorial Commentary: Health Care Policies Must Ensure Equitable Access for All Patients.

It is critical to identify disparities in health care, including orthopaedic pathology, to ensure equitable care across all populations. Recent research shows that White, socially nondeprived, male patients with private insurance are more likely to undergo surgical procedures for rotator cuff disease after a minimum of 6 months of nonoperative management, and race and low income are associated with delays in access to health care and worse outcomes. Moreover, many practitioners and researchers do not record or report important socioeconomic factors. Health care policies must ensure equitable a

Sociodemographics of Chiari I Malformation.

Socioeconomic and demographic factors affect the care of patients with Chiari I malformations. This article describes the current cost of surgical treatment of Chiari I malformations and highlights how careful patient selection, treatment selection, postoperative protocols, and varying payment models may serve as cost-reducing measures. In addition, the article highlights racial disparities resulting in delayed diagnosis and greater disease progression in non-white patients, as well as the relative parity in the workup and treatment of Chiari I malformation based on insurance status. These fin

Back surgery: Modern medical pitfall.

Medical iatrogenesis is at an all-time high with increasing deaths, disability, and costs compounded by unnecessary and ineffective surgeries despite the warnings from WHO, the US Public Health Service, and the Institute of Medicine. One area in particular, failed back surgeries, has drawn increasing attention by researchers due to disproved medical theories and surgical treatments. Paradoxically, while spinal manipulative therapy has been shown to achieve better results for this epidemic of low back pain in particular, medical and insurance programs often limit or boycott this inexpensive and

How Long Can You Delay? Curve Progression While Awaiting Vertebral Body Tethering Surgery.

Background: The implications of delaying surgical intervention for patients with adolescent idiopathic scoliosis (AIS) wishing to undergo vertebral body tethering (VBT) have not yet been explored. It is important to understand how these delays can impact surgical planning and patient outcomes. Methods: This was a retrospective review that analyzed all AIS patients treated between 2015 and 2021 at a single tertiary center. Time to surgery from initial surgical consultation and ultimate surgical plan were assessed. Patient characteristics, potential risk factors associated with increased curve p

Impact of Socioeconomic Status, Race and Ethnicity, and Geography on Prenatal Detection of Hypoplastic Left Heart Syndrome and Transposition of the Great Arteries.

BACKGROUND: Prenatal detection (PND) has benefits for infants with hypoplastic left heart syndrome (HLHS) and transposition of the great arteries (TGA), but associations between sociodemographic and geographic factors with PND have not been sufficiently explored. This study evaluated whether socioeconomic quartile (SEQ), public insurance, race and ethnicity, rural residence, and distance of residence (distance and driving time from a cardiac surgical center) are associated with the PND or timing of PND, with a secondary aim to analyze differences between the United States and Canada. [Sentinel node biopsy in breast cancer].

Sentinel node biopsy is a promising surgical technique to avoid unnecessary axillary lymph node dissection for breast cancer patients with histologically negative nodes. Several randomized phase III trials in Western countries are underway to assess sentinel node biopsy in cases of breast cancer in comparison with conventional axillary lymph node dissection. Other investigators have also started observational studies in cases when sentinel lymph nodes are proven histologically negative. In Japan, many issues regarding this minimally invasive surgery remain unresolved: optimal procedure, pathol

Preoperative Corticosteroid Injection and Postoperative Infection Following De Quervain's Release: A National Database Analysis.

BACKGROUND: De Quervain's tenosynovitis is initially treated with analgesics, immobilization, and corticosteroid injections. Patients with symptoms refractory to corticosteroid injection may require surgical release. No prior studies have identified whether injection of corticosteroid for De Quervain's tenosynovitis leads to adverse postoperative outcomes. This study aimed to determine whether corticosteroid injections administered within 3 months prior to De Quervain's release increase the risk of postoperative surgical site infection. METHODS: We used a national administrative database to i

Closed Claims Analysis of Data for Ambulatory Anesthesia.

Ambulatory anesthesia has grown rapidly, encompassing more complex procedures and higher-risk patients. Analysis of closed claims reveals that while major complications and mortality remain rare, moderate injuries such as dental trauma, nerve damage, and communication-related issues are frequent and remained largely stable in frequency over the past decade. Patient comorbidities-including obesity, obstructive sleep apnea, and diabetes-significantly increase perioperative risks. Improving patient safety in this setting depends on careful risk assessment, multimodal analgesia, opioid-sparing tec

Anesthetic mishaps and the cost of monitoring: a proposed standard for monitoring equipment.

Review of insurance data indicates that approximately 1.5 claims are paid per 10,000 anesthetic procedures, a conservative estimate of the incidence of preventable serious injury associated with anesthesia. Insurance data permit estimation of the premium cost for the anesthesiologist and hospital, per operating room per year, of $69,429.00. We propose the use of an enhanced monitoring standard requiring a pulse oximeter, capnograph, spirometer, halometer, automatic sphygmomanometer, breathing circuit oxygen analyzer, stethoscope, electrocardiographic monitor, and temperature monitor.
