Long-term characteristics of healthcare-associated infections in a neonatal intensive care unit.

BACKGROUND/PURPOSE: Healthcare-associated infections in neonatal intensive care units (NICUs) are associated with a significant risk of morbidity and mortality. Knowledge regarding pathogens, primary sources of infection and antibiotic resistance in the NICU is essential for developing management strategies. This study aimed to analyze the long-term characteristics of healthcare-associated infections in a tertiary referral center in southern Taiwan. METHODS: Infants < 30 days old, with positive blood, cerebrospinal fluid, urine or tissue fluid cultures during hospitalization in the NICU of Na

Strategies to prevent bacterial and fungal infection in the neonatal intensive care unit.

Hospital-acquired infections are one of the leading causes of preventable morbidity and mortality in neonatal intensive care units (NICUs). Device-related infections, such as catheter-associated blood stream infections (CABSIs) and ventilator-associated pneumonia (VAP), are the most common nosocomial infections. This review examines the pathogenesis of CABSIs and methods, widely accepted and novel, that can be used to help prevent them. Strategies to prevent fungal infections, which are often associated with the presence of a central venous catheter, are also reviewed. Finally, the dilemmas in

Variability on red blood cell transfusion practices among Brazilian neonatal intensive care units.

BACKGROUND: Guidelines for red blood cell (RBC) transfusions exist; however, transfusion practices vary among centers. This study aimed to analyze transfusion practices and the impact of patients and institutional characteristics on the indications of RBC transfusions in preterm infants. STUDY DESIGN AND METHODS: RBC transfusion practices were investigated in a multicenter prospective cohort of preterm infants with a birth weight of less than 1500 g born at eight public university neonatal intensive care units of the Brazilian Network on Neonatal Research. Variables associated with any RBC tr

Cefepime restriction improves gram-negative overall resistance patterns in neonatal intensive care unit.

Antibiotic restriction can be useful in maintaining bacterial susceptibility. The objective of this study was verify if restriction of cefepime, the most frequently used cephalosporin in our neonatal intensive care unit (NICU), would ameliorate broad-spectrum susceptibility of Gram-negative isolates. Nine hundred and ninety-five premature and term newborns were divided into 3 cohorts, according to the prevalence of cefepime use in the unit: Group 1 (n=396) comprised patients admitted from January 2002 to December 2003, period in which cefepime was the most used broad-spectrum antibiotic. Patie

Clinical and microbiological characteristics of hospital infections in the neonatal intensive care unit.

Neonates hospitalized in intensive care units, are exposed to a higher risk of infectious complications. The research involved 52 neonates hospitalized in the Neonatal Intensive Care Unit (NICU), Chair and Clinic of Obstetrics and Perinatology over a span of one year. The incidence of hospital infections as well as etiological factors were analyzed. Clinically manifested hospital infections were diagnosed in 38.5% of babies with very low or extremely low birth weight, in boys twice as often as in girls. Generalised invasive infections prevailed; in most cases they were caused by Gram-negative

Use of antimicrobial agents in United States neonatal and pediatric intensive care patients.

OBJECTIVE: Antimicrobial use contributes to the development of emergence and dissemination of antimicrobial-resistant bacteria among intensive care unit (ICU) patients. There are few published data on antimicrobial use in neonatal (NICU) and pediatric ICU (PICU) patients. METHODS: Personnel at 31 Pediatric Prevention Network hospitals participated in point prevalence surveys on August 4, 1999 (summer) and February 8, 2000 (winter). Data collected for all NICU and PICU inpatients included demographics, antimicrobials and indications for use and therapeutic interventions. RESULTS: Data were re

Admission to neonatal intensive care with respiratory morbidity following 'term' elective caesarean section.

Delaying the timing of elective caesarean section (ELCS) from 37/38 weeks to 39/40 weeks reduces the risk of respiratory morbidity in otherwise healthy neonates. The aims of this study were to clarify the number and clinical details of term infants delivered by ELCS who were admitted to neonatal intensive care (NIC) from 1st April 2001 to 31st March 2002 in Northern Ireland, to record the level of care required by these infants and to identify infants who were diagnosed with transient tachypnoea of the newborn (TTN) and/or Respiratory Distress Syndrome (RDS). Two hundred and ten (8%) of babies

Volume expansion during neonatal intensive care: do we know what we are doing?

Although volume expansion is liberally used in newborn intensive care, we know little about its effects on hemodynamics or outcomes. Given appropriately to a truly hypovolemic baby, it can be life-saving, but the clinical diagnosis of hypovolemia is probably very inaccurate. We know that volume expansion has less effect on blood pressure than dopamine, and although it seems to produce immediate increases in systemic blood flow, we do not know for how long these increases are sustained. There is evidence to show that the routine use of volume expansion in preterm babies has no effect on outcome

Familial asphyxiating thoracic dysplasia: clinical variability and impact of improved neonatal intensive care.

We describe 3 siblings with asphyxiating thoracic dysplasia whose neonatal symptoms range from mild respiratory distress to asphyxia and death. The youngest sibling received aggressive modern respiratory intensive care, survived, and at 2 years showed no respiratory symptoms. Improved neonatal intensive care has implications for clinical decision making and genetic counseling.

Intravenous midazolam infusion for sedation of infants in the neonatal intensive care unit.

BACKGROUND: The need for sedation for neonates undergoing uncomfortable procedures in the neonatal intensive care unit (NICU) has often been overlooked. Proper sedation may reduce stress and avoid complications during procedures such as mechanical ventilation. Midazolam is a short acting benzodiazepine that has been increasingly used in the NICU. However, the effectiveness of intravenous midazolam as a sedative in neonates has not been systematically evaluated. OBJECTIVES: To determine whether intravenous midazolam infusion is an effective sedative, as evaluated by behavioural and/or physiolo

Bloodstream infections in neonatal intensive care unit patients: results of a multicenter study.

For identification of risk factors for bloodstream infection (BSI) among neonatal intensive care unit patients, prospective 6-month studies in three neonatal intensive care units were conducted. BSI was diagnosed in 42 of 376 (11.2%) enrolled infants. Pathogens included coagulase-negative staphylococci, Candida sp., Group B streptococci and Gram-negative species. Patients with BSIs were more likely to die during their neonatal intensive care unit stay than were patients who did not acquire BSIs (6 of 42 vs. 11 of 334, P = 0.007). BSI rate was highest in infants with birth weight < 1500 g (rela

Neonatal intensive care: is it worth it? Developmental sequelae of very low birthweight.

A significant portion of the health care dollar has been spent on neonatal intensive care since the early 1970s when technologic developments permitted salvage of very small premature infants. The wisdom of allocating so much for so few has been challenged, especially if the result is an increase in the number of severely mentally and motorically disabled children. Studies from around the world of the secular trends in mortality and morbidity for very and extremely low-birthweight babies uniformly indicate that there have been dramatic decreases in mortality and morbidity in the past 20 years.

Percutaneous central venous catheterization. Three years' experience in a neonatal intensive care unit.

Prolonged venous access is desirable in very-low-birth-weight infants and infants for whom feedings are contraindicated. We prospectively evaluated 481 small-diameter venous catheters placed percutaneously in 317 patients over 3 years. Of 478 catheters, 241 (50%) were placed in infants weighing 1 kg or less. Mean catheter stay was 13 days (range, less than 1 to 77 days). Almost half (49%) of the central and thoracic catheters (91% of placements) were removed nonelectively: 43% due to problems such as leaking or clotting and 6% to suspicion of sepsis or venous occlusion. Of the 23 episodes of p

The pharmacology of neonatal resuscitation and cardiopulmonary intensive care. Part I--Immediate resuscitation.

Resuscitation of a neonate requires both immediate cardiopulmonary resuscitation and extended intensive care. Initial resuscitation of the neonate, as for adults, must include support of the airway, breathing and circulation. Because of the unique physiology of a newborn infant, some aspects of drug therapy differ significantly from their counterparts in the resuscitation of adults, and hypoglycemia and hypothermia pose special threats to a distressed neonate. Epinephrine and atropine can be administered via an endotracheal tube, but vascular access, which is most easily obtained by cannulatin

[Echovirus 11 outbreak among premature newborn infants in a neonatal intensive care unit].

During a period of 3 weeks, 17 premature neonates 4 of them weighing less than 1000 g at birth became infected with echovirus 11 in a neonatal intensive-care unit. Beside 3 inapparent infections, severe 'septicaemic' illness was observed among 5 premature neonates combined with meningitis and apnoeic attacks. 4 neonates presented meningitis as the predominant clinical feature and 3 showed gastrointestinal symptoms without neurological involvement. 2 children experienced a febrile illness with apnoea and bradycardia. All babies recovered. Echovirus 11 could be isolated in 51 of 74 (68.9%) speci

Neonatal Factors Associated with Mortality Among Preterm Infants Admitted to Neonatal Intensive Care in a Peruvian National Hospital.

Background/Objectives: Preterm birth is a leading cause of neonatal mortality, especially in low- and middle-income countries. Despite advances in neonatal care, mortality among preterm infants in intensive care units remains high, and specific risk factors are not fully understood. This study aimed to identify neonatal factors associated with mortality among preterm infants admitted to the neonatal intensive care unit (NICU) of a Peruvian national hospital. Methods: An analytical cross-sectional study was conducted at Guillermo Almenara National Hospital in Lima, Peru, including all preterm n
