Duodenocolic fistula healing by pentadecapeptide BPC 157 in rats. A cytoprotection viewpoint.

Using duodenocolic fistula in rats, this study attempts to highlight the particular cytoprotection aspects of the healing of fistulas and therapy potential of the stable gastric pentadecapeptide BPC 157, a cytoprotection mediator (i.e. upgrading minor vessels to induce healing at both fistula's sides). Upon duodenocolic fistula creation (two 'perforated' lesions put together) (assessed at 3, 6, 9, 12, and 15 min), BPC 157, given locally at the fistula, or intragastrically (10 μg/kg, 10 ng/kg), rapidly induces vessel 'recruitment', 'Выполняется' toward the defect, simultaneously at duodenum and col

The role of endoscopy in the correct and rapid positioning of feeding tubes.

The recognition of the importance of the enteral route for (hyper) alimentation led us to develop three methods for enteral placement of feeding tubes using endoscopic techniques. With these three treatment modalities it was possible to carry out enteral administration of nutrients in a group of patients, in whom otherwise only total parenteral feeding or enteral feeding by needle catheter jejunostomy was considered feasible. The use of endoscopic equipment permitted correct placement of tubes beyond the stomach either in the duodenum, at the ligament of Treitz or into the jejunum. Also simult

Repair of a recurrent rectovaginal fistula with a biological graft.

This case involves a patient with the congenital absence of the lower third of the vagina. While undergoing surgical restoration of the vagina, she sustained a laceration, which ultimately led to the development of a rectovaginal fistula. After two unsuccessful attempts at repair, the recommendation was for a diverting colostomy with another attempted repair, and she presented to our clinic to discuss other possible surgical options. The patient underwent repair of the fistula using a porcine-derived small intestinal submucosal extracellular matrix graft, which resulted in the repair of the re

[Entero-uterine fistual as the first symptom of intestinal neoplasms. Description of a case].

Vaginal elimination of stool due to sigmoid-uterine fistula was the first symptom in a case of neoplasia of the sigma. Few instances of a similar fistula appear in the literature. Bouskela, Chérisié, Yourde and Taieb observed one case starting from the uterus. McGregor & Bacon have described a fistula starting from the colon due to diverticulitis, as have Colceck & Staumann, Smalley et al., and Johnston & Stubbs. No previous example of tumour of the large intestine as the cause of a fistula with the corpus uteri could be found. On examination, the patient presented losss of stool via the vagin

Implantation of rectal cancer in an anal fistula: report of a case.

A 53-year-old man who had had an anal fistula for 20 years was admitted to our hospital with a large intestinal obstruction. Barium enema and colonoscopy confirmed advanced rectal cancer and we palpated a soft tumor, 3 cm in diameter, with inflammatory induration on the right side of the rectum. After draining a perianal abscess caused by the anal fistula, we performed low anterior resection. Histological examination of the perianal necrotic tissue obtained during resection of the perianal tumor encompassing the anal fistula revealed adenocarcinoma. Since the histology of the perianal lesion w

[Diagnostic and therapeutic guidelines for entero-cutaneous fistulas. Personal experience and literature review].

The entero-cutaneous fistulas (ECF) are abnormal communications between intestine and abdominal skin. They can occur spontaneously, or after an injury or a surgical procedure. They are associated with a high rate of morbidity and mortality. Spontaneous fistulas can mainly occur in patients affected by cancer, inflammatory bowel disease, diverticulitis, appendicitis, as a result of radiotherapy or injuries. Surgical procedures, carried out in case of neoplastic diseases, inflammatory bowel disease, adhesions removal, represent the primary cause in the development of a postoperative fistulas. Ma

Complications of colonic diverticulitis.

About half of the patients in whom diverticulitis develops have subsequent attacks. Recurrence increases the likelihood both of complications, such as perforation, abscess, fistula, obstruction, and hemorrhage, and of the necessity for surgical intervention. For these reasons, patients who have had two or more attacks of acute diverticulitis should be considered candidates for prophylactic resection.

"Not All That Fistulizes Is Crohn's Disease": A Rare Case of Ischemic Jejunocolic Fistula.

Enterocolic fistulae lead to abnormal diversion of gastrointestinal contents from the small intestine to the colon, causing diarrhea and protein energy malnutrition. We describe a rare case of a 40-year-old patient presenting with chronic diarrhea, unintentional weight loss, sitophobia, and severe abdominal pain in the setting of extensive thrombosis due to JAK2 V617F mutant disorder and associated tobacco use. Computed tomography enterography demonstrated an enterocolic fistula (jejunum to cecum), and a colonoscopy confirmed a 4 mm fistula in the cecum. The patient was nutritionally optimized

Treatment of an enteric fistula with somatostatin in a premature.

The authors present the case of an extremely premature baby affected by severe Necrotizing Enterocolitis (NEC) needing intestinal resection and ileostomy. An enteric fistula developed 3 days after ostomy closure. The baby was started on Total Parenteral Nutrition (TPN), and 7 days later the fistula output remained constant. Somatostatin (SM) was then given intravenously (3.0 micrograms/Kg/hr) and the fistula closed on the 3rd day of treatment. Since SM was introduced in 1986 as an adjunct treatment to TPN in enteric fistulas, the authors believe that theirs is the first report of a successful

Roentgen findings in aorto-enteric fistulae.

Six cases of aorto-enteric fistulas are discussed. In three patients the aorta ruptured into the duodenum and in three there was rupture into the esophagus. Aorto-enteric fistula is usually caused by atherosclerosis, but may also be due to syphilis or tuberculosis. Following insertion of an aortic graft, fistula is usually the result of breakdown of the anastomosis. Gastrointestinal bleeding usually heralds the problem. The roentgen signs of rupture are pressure on the esophagus or intestine, bleeding into the wall or lumen, and demonstration of the fistula. The abnormal connection may not be
