Molecular Aspects of Colorectal Adenomas: The Interplay among Microenvironment, Oxidative Stress, and Predisposition.

The development of colorectal cancer (CRC) is a multistep process initiated by a benign polyp that has the potential to evolve into in situ carcinoma through the interactions between environmental and genetic factors. CRC incidence rates are constantly increased for young adult patients presenting an advanced tumor stage. The majority of CRCs arise from colonic adenomas originating from aberrant cell proliferation of colon epithelium. Endoscopic polypectomy represents a tool for early detection and removal of polyps, although the occurrence of cancers after negative colonoscopy shows a signifi

Gastrointestinal Tract Perineuriomas and Benign Fibroblastic Polyps: Case Report and Comprehensive Systematic Review.

Background: Perineurioma is a rare benign peripheral nerve sheath tumor that can arise in various body locations. In the gastrointestinal (GI) tract, perineuriomas are uncommon and have only been reported in case reports and case series. In addition, a new classification suggests reclassifying benign fibroblastic polyps as perineurioma when they show positive markers of perineurial differentiation. Objective: This study aims to enhance understanding of GI tract perineuriomas by presenting a new case and conducting a systematic literature review. Methods: We described a new case of colonic peri

[Peutz-Jeghers syndrome manifested as massive melæna at CHU-JRA Madagascar hospital: a case report].

Peutz-Jeghers syndrome (SPJ) is characterized by intestinal hamartomatous polyps in association with mucocutaneous lentiginosis. Patients are exposed to mechanical and bleeding complications. It is a cancer predisposition syndrome. Our study highlights the diagnostic criteria for Peutz-Jeghers syndrome (SPJ), the complications and the therapeutic progresses in patient care. We report the case of a 32-year-old male presenting with a massive melaena. It was hospitalized in the surgical intensive care unit with hypovolemic shock difficult to control. This required surgical intervention to stop bl

Heterotopic gastric mucosa together with intestinal metaplasia and moderate dysplasia in the gall bladder: report of two clinically unusual cases with literature review.

We report the clinicopathological findings of two patients with ectopic gastric mucosa within the gall ladder. The first patient, a 78 year old man, was asymptomatic. He was admitted to hospital for a colon adenocarcinoma. Intraoperatively, a firm nodule was palpable in the gall bladder. Histological examination of the resected specimen revealed a body type gastric mucosa in the submucosa, adjacent to which were extensive pyloric gland and intestinal metaplasia with mild to moderate dysplasia. The remaining gall bladder mucosa demonstrated changes of chronic cholecystitis. The second patient w

Location in the large bowel influences the APC mutations observed in FAP adenomas.

The right colon differs from the left, in embryological origin, luminal environment, and function. In both sporadic colorectal cancer and Familial Adenomatous Polyposis (FAP), polyp density and cancer susceptibility vary markedly by colonic site. Adenomas in FAP have a different mutational spectrum in small intestine versus colon. This study aimed to investigate whether colonic location also influences the APC mutation spectrum in FAP. 127 1-2 mm mildly dysplastic adenomas from 5 patients with a codon 1309 germline mutation, and 41 from 3 patients with mutations proximal to codon 1265, were an

Ileocecal intussusception secondary to giant ileal lipoma in patient with chronic constipation: a case report.

INTRODUCTION: Intussusception in adults is rare, comprising 5% of cases, and is often associated with pathological lesions like polyps or tumors. This report highlights a case of ileocolic intussusception due to a giant terminal ileal lipoma incidentally found during a colonoscopy. CASE PRESENTATION: A 53-year-old male presented with chronic constipation, mild abdominal pain, and rectal bleeding for 2 years. The patient used laxatives in the past for constipation but it did not improve. A colonoscopy revealed protrusion of the small intestine through the ileocecal valve into the cecum with a

Familial Juvenile Polyposis Syndrome: A Case Report of a Cosegregation of SMAD4 Germline Pathogenic Variant c.403C>T and Review of the Literature.

Juvenile polyposis syndrome (JPS) is an inherited autosomal dominant disease that is distinguished by the emergence of numerous juvenile polyps within the gastrointestinal (GI) tract and has an increased risk of cancer. JPS is associated with germline mutations of the BMPR1A or SMAD4 genes. We aim to describe a case of familial occurrence of the SMAD4 germline pathogenic variant c.403C>T in a Brazilian family. We present a case of an 18-year-old female with rectal bleeding and chronic anemia who had a family history of colorectal issues-her mother had a total colectomy due to multiple polyps,

[MALT lymphoma of large intestine as multiple large polypoid lesions].

We report a case of mucosa associated lymphoid tissue (MALT) lymphoma in the large intestine in a 38-year-old Japanese female. She developed a dull pain in the right lower abdomen and was found to have ileocecal intussusception. The terminal ileum, cecum and ascending colon were resected. Macroscopically, multiple polypoid lesions were found. Although some authors reported that MALT lymphoma of the colon tend to be solitary, the present case showed seven lesions. Two of the polypoid lesions in the present case were marked large. No such large polypoid MALT lymphoma has been described to our kn

Vanek's tumor causing ileoileal intussusception in a middle-aged man.

Intestinal obstruction is a common surgical emergency requiring urgent intervention. Small bowel obstruction secondary to intussusception is rarely encountered especially when inflammatory fibroid polyp (IFP) is the lead point. A 41-year-old gentleman with intestinal intussusception secondary to IFP presented to us with a classic symptom of intestinal obstruction. Computed tomography revealed a target or sausage-shaped soft tissue mass with a layering effect, which was confirmed by intraoperative findings. Histopathology was consistent with IFP and supported by immunoreactivity of CD34 and neg

Inflammatory fibroid polyps of the duodenum: a review of the literature.

BACKGROUND/AIM: An inflammatory fibroid polyp (IFP) is a rare benign submucosal gastro-intestinal lesion with an uncertain origin and natural history. An IFP was first described in the stomach and is rare in the duodenum. Presenting signs and symptoms vary according to anatomical site. This systematic literature review was prompted by the recent presentation of a patient with biliary obstruction caused by a duodenal IFP. METHODS: A review of all duodenal IFPs identified in the PubMed database from 1971 onwards is presented within the context of what is known about the lesion at other gastro-i

First endoscopic-histologic follow-up in patients with body-predominant atrophic gastritis: when should it be done?

BACKGROUND: Body-predominant atrophic gastritis is considered a risk factor for gastric cancer and carcinoid. Timing of follow-up for patients with this disorder has not been defined. This study was undertaken to determine the optimal time for the first endoscopic/histologic follow-up in patients with body-predominant atrophic gastritis. METHODS: Forty-two patients with body-predominant atrophic gastritis were randomly assigned to 1 of 2 follow-up intervals: group A (n = 22) at 24 months and group B (n = 20) at 48 months. At baseline and follow-up patients underwent gastroscopy at which biops

[Diagnosis of inflammatory and tumorous changes of the large intestine using colonic sonography].

By water instillation into the large intestine, sonographic visualization of the whole length of the colon from the recto-sigmoid to the caecum can be achieved. Detailed evaluation of the lumen, the wall and the surrounding connective tissue may be carried out. Diagnostic sonography of the colon in 41 patients with pathological changes confirmed by colonoscopy showed that colonic polyps and carcinomas can be detected by this method. In Crohn's disease, typical changes can be seen, which allow the degree of inflammatory infiltration to be assessed. Thus, sonography of the colon is a diagnostic

Alimentary Tract Neoplasia in Captive Bearded Dragons (Pogona spp).

There are multiple reports of gastric neuroendocrine carcinoma in bearded dragons (Pogona spp), but other types of alimentary neoplasia are rarely reported. In a retrospective study, we identified 51 cases of neoplasia, including gastric (n = 26), oral (n = 18), intestinal (n = 13), oesophageal (n = 3) and cloacal (n = 2) neoplasms. Round cell neoplasia was diagnosed in the alimentary tract in 14 cases, all of which had extra-alimentary involvement, most commonly in the liver, lung, spleen and kidney, and was considered to be the cause of death or euthanasia in all cases. Oral neoplasms includ

Endoscopic Mucosal Resection in Children.

OBJECTIVES: Endoscopic mucosal resection (EMR) for removal of large polyps is well established in adults. EMR technique in the pediatric population is less utilized due to lower incidence of large intestinal polyps in pediatric patients and limited EMR training for pediatric gastroenterologists. The aim of this study is to retrospectively review safety and efficacy of pediatric EMR cases at two large, tertiary referral centers with adult and pediatric EMR expertise. METHODS: A retrospective chart review was conducted at Cedars-Sinai Medical Center and Cincinnati Children's Hospital Medical Ce…

A syndrome of progressive muscle spasm, alopecia, and diarrhea.

A syndrome of progressive muscle spasm, alopecia, and diarrhea was seen in 15 patients. The syndrome was characterized by painful intermittent muscle spasm, alopecia, amenorrhea, and malabsorption, and was sometimes associated with epiphyseal destruction and retarded growth. Symptoms began at age 10 and were more common in women than men. Muscle cramps affected the limbs first and then, several years after onset, the neck, trunk, and masticatory muscles. The course was progressive and led to malnutrition. Four patients died from 5 to 18 years after onset. Autopsy revealed polypoid changes thro

Crosstalk between the growth hormone/insulin-like growth factor-1 axis and the gut microbiome: A new frontier for microbial endocrinology.

Both the GH/IGF-1 axis and the gut microbiota independently play an important role in host growth, metabolism, and intestinal homeostasis. Inversely, abnormalities in GH action and microbial dysbiosis (or a lack of diversity) in the gut have been implicated in restricted growth, metabolic disorders (such as chronic undernutrition, anorexia nervosa, obesity, and diabetes), and intestinal dysfunction (such as pediatric Crohn's disease, colonic polyps, and colon cancer). Over the last decade, studies have demonstrated that the microbial impact on growth may be mediated through the GH/IGF-1 axis,
