Paralytic ileus is marked by the cessation of bowel motility. This condition is a major clinical concern that may lead to severe patient morbidity in orthopaedic surgery and trauma patients. Ileus most commonly occurs following spinal surgery, traumatic injury, or lower extremity joint reconstruction, but it may also occur following minor orthopaedic procedures. Clozapine is a prototypical second-generation antipsychotic, which has been found to be associated with various adverse effects. Although emphasis has mainly been placed on its haematological and metabolic side effects, less attention has been paid to its anticholinergic effects. One such problem is that of a paralytic ileus. Fifteen patients with severe scleroderma bowel disease began receiving home central venous hyperalimentation (HCVH) between 1979 and 1987. The major reasons for instituting HCVH were intestinal pseudo-obstruction, malabsorption, and malnutrition. Eleven patients had an improved quality of life. Serious complications encountered over these 15,700 catheter-use days were 2 episodes of septicemia and 2 episodes of superior vena cava obstruction. Seven patients died, but none directly from their gastrointestinal disease or from the HCVH.

Gastrointestinal involvement in systemic lupus erythematosus (SLE) usually occurs in the form of mesenteric vasculitis, protein-losing enteropathy, intestinal pseudo-obstruction, and pancreatitis. Small bowel diverticulosis occurs in 0.3-4.4% of all autopsies. BACKGROUND: Paralytic ileus (PI) also known as functional ileus or adynamic ileus occurs when intestinal motor activity is impaired. Cessation of ingesta passage leads to the accumulation of fluid and gas causing intestinal dilatation. With this type of ileus, intestinal transit is functionally impaired in the absence of a physical obstruction.
