Infarcted intestinal volvulus detected by prenatal ultrasonography.

This article describes a prenatal ultrasonographic finding of an infarcted intestinal volvulus. Ultrasonography showed polyhydramnios, multiple dilated intestinal loops, increased transverse abdominal area, and ascites. After cesarean section due to premature rupture of membranes and fetal distress, derotation of the infarcted volvulus caused postoperative thrombocytopenia, hyperkalemia, and acidosis and a subsequent resection was required. A high output of intestinal juice from the jejunostomy caused severe hypovolemia and electrolyte imbalance with resultant death. Increased transverse abdom…

[Immediate oesojejunal anastomosis after total gastrectomy in caustic necrosis].

Eight patients underwent total gastrectomy for caustic ingestion. Oesophago-jejunostomy was performed in the same surgical session. One patient died from hemorrhage. Only one of the 7 survivors had a benign anastomotic fistula. Five healed without any sequelae, 2 underwent colonoplasty 3 months later, because of a secondary oesophageal stenosis. We conclude that it is feasible, in certain unusual cases, to restore gastrointestinal continuity after total gastrectomy for caustic necrosis: moderate oesophageal lesions (stage I or II) and ingestion of a caustic product that cause few oesophageal d…

An ex vivo comparison of jejunal transection angles and the effect on lumen diameter following end-to-end jejunojejunal anastomoses.

OBJECTIVE: To evaluate three different jejunal transection angles for end-to-end jejunojejunostomies, comparing construction time, lumen size, and suture number. STUDY DESIGN: Ex vivo cadaver study. SAMPLE POPULATION: Eight euthanized horses each had three mid-jejunal specimens harvested. METHODS: Jejunal segments were assigned to one of three groups (A30, A45, and A60) and transected at 30, 45, or 60°, respectively. Following transection, a standardized handsewn single-layer interrupted modified Lembert anastomosis was performed, and construction time and number of sutures placed were reco

A clinical consensus paper on jejunal tube feeding in children.

BACKGROUND: Feeding problems are common in children with complex medical problems or acute critical illness and enteral nutrition may be required. In certain situations, gastric tube feeding is poorly tolerated or may not be feasible. When feed intolerance persists despite appropriate adjustments to oral and gastric enteral regimens, jejunal tube feeding can be considered as an option for nutrition support. METHODS: A multidisciplinary expert working group of the Australasian Society of Parenteral and Enteral Nutrition was convened. They identified topic questions and five key areas of jejuna

Tutorial on adult enteral tube feeding: Indications, placement, removal, complications, and ethics.

Enteral nutrition, generally preferred to parenteral nutrition, is indicated when patients cannot meet their energy and metabolic demands. Gastrostomy tubes are placed directly into the stomach (either endoscopically, surgically, or radiologically) through the abdominal wall. Routine gastrostomy tube care is important to maintain well-functioning tubes. Postpyloric feeding tubes are preferable to gastric feeding tubes if patients have a history of aspiration, gastroesophageal reflux, severe gastroparesis, and/or recurrent nausea and vomiting. Feeding jejunostomy tubes are placed surgically and…

Single-Anastomosis Pylorus-Preserving Bariatric Procedures: Review of the Literature.

Single-anastomosis pylorus-preserving procedures (SAPPP) were recently introduced into bariatric surgery in order to combine the physiologic advantages of a post-pyloric reconstruction with the technical advantages of an omega loop. Surgery consists of a sleeve gastrectomy that is performed first, followed by a duodeno-enterostomy. Two main variants exist: proximal and distal SAPPP, with duodeno-jejunostomy and duodeno-ileostomy, respectively. This review describes the SAPPP reported in the literature and analyzes their outcomes in comparison with the most frequently performed bariatric techni…

Esophageal perforation associated with fontan operation: a complication of transesophageal echocardiography.

A 10-year-old girl with unbalanced atrioventricular septal defect and aortic arch interruption with interrupted inferior vena cava and dextrocardia had undergone a modified Norwood procedure followed by a Kawashima operation in 2002 and 2005, respectively. In 2012, she underwent Fontan operation with diversion of hepatic veins to left pulmonary artery. Intraoperative transesophageal echocardiography (TEE) was used. She was noted to have an esophageal perforation, diagnosed on the 10th postoperative day. She was managed with thoracic drainage, cervical esophagostomy, and feeding jejunostomy. Es…

Delivery of enteral nutrition.

There is increasing evidence that enteral feeding is superior to parenteral nutrition with regard to maintaining gut structure and function. Selection of the enteral access route depends on the type and anticipated duration of nutrient delivery. At present, enteral feeding devices can be divided into two major categories: those entering the gastrointestinal tract through the oral or nasal cavity (oroenteric or nasoenteric tubes) and those entering through the abdominal wall including gastrostomy, duodenostomy, or jejunostomy tubes. This article provides a review of methods to insert and confir…

[Temporary minimal esophagostomy in the surgical treatment of malignant esophago-respiratory fistulas].

Oesophago-bronchial and oesophago-pleural fistulae are serious complications of oesophageal cancers. They occur earlier than is generally believed and, if untreated, result in death caused by septic and respiratory complications. An original two-stage surgical method is reported and illustrated by 3 cases. The first stage consists of excluding the oesophagus at both ends and introducing, through a small oesophagostomy, a Foley's catheter to aspirate the secretions; the cardia is excluded by stapling; jejunostomy is performed by the catheter technique. The second stage consists of oesophageal r…

[Our experience with reparative and derivative operations for non-cancerous lesions in the extrahepatic bile ducts].

17 reparative operations on the bile ducts, 7 of which were immediate (5 for peroperative lesions and 2 for "spontaneous" lesions) and 10 reoperations to deal with sclero-cicatricial lesions, were studied. 7 reconstructive operations (plastic or end-to-end anastomosis) and 10 derivative operations with a preference for hepato- or choledocho-jejunostomy on defunctionalized loop were carried out. The former were reserved for lesions without or with minimum loss of substance, the latter for extensive lesions. Operative mortality is high and higher still in immediate interventions. Long term resul…

Enteral nutrition and quality of life in patients undergoing chemoradiotherapy for esophageal carcinoma: a comparison of nasogastric tube, esophageal stent, and ostomy tube feeding.

BACKGROUND AND AIMS: This study prospectively recruited esophageal squamous cell carcinoma patients who received esophageal stent, nasogastric tube (NGT), or jejunostomy/gastrostomy feeding to compare the changes in nutritional status and quality of life during chemoradiation therapy (CRT). METHODS: In total, 81 patients were analyzed (stent, 7; surgical ostomy, 26; NGT, 19; oral intake, 29). An NGT was inserted when, despite medication, dysphagia or pain worsened with oral feeding during CRT. Serial body weight and daily narcotic demand were recorded. Marginal Artery of Drummond Masquerading as a Fistulous Tract Resulting in Recurrent Lower Gastrointestinal Bleeding.

Lower gastrointestinal bleeding (LGIB) can be caused by a variety of causes. Pseudoaneurysms have been described as a rare etiology of LGIB and are associated with pancreatic pseudocysts that involve adjacent vasculature. Our study describes a 38-year-old man with recent severe coronavirus disease 2019 and necrotizing pancreatitis presenting with hematochezia and blood clots by gastrostomy-jejunostomy. Initial flexible sigmoidoscopy did not elicit an etiology for the LGIB. Recurrent hematochezia prompted colonoscopy and angiography, which demonstrated a pseudoaneurysm in the marginal artery of…

Ductus Aneurysm Ruptured Into the Esophagus With Massive Bleeding.

We report a patient with life-threatening hematemesis caused by the rupture of a ductus aneurysm into the esophagus, which was successfully treated by coil embolism for the esophageal fistula through the aorta and subsequent thoracic endovascular aortic replacement. Second-stage therapy was performed surgically after proactive antibiotic treatment and in consideration of the patient's improved general condition. This included debridement with drainage, aortic encasement with remnant aneurysmal wall, omentopexy, and jejunostomy. The esophagus was preserved, and satisfactory healing and natural …

Management of duodenal trauma.

The surgical management of fifteen cases of duodenal trauma is reported. Blunt injury of the abdomen was the commonest mode of injury. Thirteen patients (86.6%) had associated intra-abdominal and/or extra-abdominal injuries. Diagnosis of duodenal injury was made at laparotomy in all patients. In the majority of patients (n = 11), either duodenal repair, or anastomosis with the pylorus or jejunum was done. Duodenal diverticulization or pancreaticoduodenectomy were undertaken in 2 patients each. Feeding jejunostomy was an important adjunct for nutritional support. Septicemia with multisystem org…

Congenital dilatation of the common bile duct in children.--The etiologic significance of the narrow segment distal to the dilated common bile duct.

Radiographic studies of 26 choledochal cyst patients were evaluated. Abnormal junction of the pancreatico-biliary system (PBS) was demonstrated in all patients. The transverse diameter of dilated common bile ducts was closely related to the length of the narrow segment of the distal common bile duct. Common bile ducts had not dilated despite the anomalous junction of PBS during the follow-up period, ranging from 2 to 12 years after choledochocysto -jejunostomy in 7 patients. The present studies indicate that no abnormal junction of PBS but stenosis of the distal common bile duct may be etiolog…
