Introgressive hybridization of human and rodent schistosome parasites in western Kenya.

Hybridization and introgression can have important consequences for the evolution, ecology and epidemiology of pathogenic organisms. We examined the dynamics of hybridization between a trematode parasite of humans, Schistosoma mansoni, and its sister species, S. rodhaini, a rodent parasite, in a natural hybrid zone in western Kenya. Using microsatellite markers, rDNA and mtDNA, we showed that hybrids between the two species occur in nature, are fertile and produce viable offspring through backcrosses with S. mansoni. Averaged across collection sites, individuals of hybrid ancestry comprised 7.…

"Delivering" on the MDGs?: equity and maternal health in Ghana, Ethiopia and Kenya.

OBJECTIVES: The Millennium Development Goals (MIDGs) have put maternal health in the mainstream, but there is a need to go beyond the MDGs to address equity within countries. We argue that MDG focus on maternal health is necessary but not sufficient. This paper uses Demographic and Health Survey (DHS) data from Kenya, Ethiopia and Ghana to examine a set of maternal health indicators stratified along five different dimensions. The study highlights the interactive and multiple forms of disadvantage and demonstrates that equity monitoring for the MDGs is possible, even given current data limitati…

Comparative acceptability of combined and progestin-only injectable contraceptives in Kenya.

OBJECTIVE: We compared 12-month continuation rates, menstrual bleeding patterns and other aspects of acceptability between users of Cyclofem and users of Depo-Provera. METHODS: The life-table method was used to calculate quarterly continuation rates. In all, 360 Kenyan women were randomly assigned to one of the two contraceptives. User-satisfaction questionnaires were administered at 6 and 12 months or at discontinuation, whichever occurred first. RESULTS: The 1-year continuation rate was 75.4% for Depo-Provera users versus 56.5% for Cyclofem users (p<.001). Main reasons for discontinuation …

Neurological and developmental outcome of neonatal jaundice and sepsis in rural Kenya.

Neonatal jaundice (NJ) and sepsis are common causes of neonatal mortality in sub-Saharan Africa, but little is known about the long-term morbidity in this setting. This study aimed to describe the neurological and developmental sequelae of severe neonatal hyperbilirubinaemia and neonatal sepsis (NS) in a district hospital in rural Kenya. Twenty-three term infants with NJ [total serum bilirubin (TSB) >300 mumol/l] and 24 infants with a history of NS were identified from hospital records. These children were compared to 40 children from the community (CC) without neonatal problems. At ages 18-32…

Laboratory-confirmed Mediterranean spotted fever in a Japanese traveler to Kenya.

A Japanese traveler returning from Kenya became ill, presenting with fever and a prominent, generalized rash without an eschar. Results of the immunofluorescence antibody assay of the patient's sera performed in Japan were compatible with illness due to a spotted fever group (SFG) rickettsia, and a presumptive diagnosis of African SFG rickettsiosis, probably either Mediterranean spotted fever (MSF) or African tick-bite fever (ATBF), was rendered. To further define the disease diagnosis, sera were examined in France by Western immunoblotting combined with cross-adsorption, which confirmed the d…

The effects of climatic factors on the distribution and abundance of malaria vectors in Kenya.

Since 1988 malaria epidemics have occurred in multiple sites in western Kenya highlands. Climatic variability has been associated with some of the recent epidemics. We examined influences of climatic factors on the distribution and abundance of three malaria vector species, Anopheles gambiae, Anopheles arabiensis, and Anopheles funestus in western Kenya and in the Great Rift Valley. Mosquito samples were collected from the lowland and highland areas with various climatic conditions. The three vector species were abundant in the lower part of western Kenya. An. arabiensis was not found in the a…

The potential of rapid screening methods for Schistosoma mansoni in western Kenya.

Data from 46 schools in western Kenya were used to investigate the performance of school-based questionnaires, on reported blood in stool and water-contact patterns, as indicators of the prevalence of human infection with Schistosoma mansoni. Prevalence of infection was associated with the prevalence of self-reported blood in stool, recent history of swimming and recent history of fishing. It was shown that use of a threshold of 30% of subjects reporting blood in stool would identify 42.9% of the 'high-prevalence' schools (i.e. prevalence > or = 50%) and 87.5% of the 'low-prevalence' schools (…

Policy implications of the inadequate support systems for orphans in western Kenya.

This paper describes the support systems available for orphans in a rural Luo community in Nyang'oma sub-location in Bondo District of Western Kenya. Qualitative data were collected through in-depth interviews with orphaned children and their caretakers as well as key informants, and through focus group discussions with orphaned children, widows and community elders. Quantitative data were obtained by questionnaires administered to 100 caretakers of orphaned children. The most serious problem was inability of the orphan households to afford school fees, although lack of food, medicare and clot…

An evaluation of surveillance of malaria at primary health care level in Kenya.

As less than twenty five per cent of persons suffering from malaria seek formal treatment in most of sub-Saharan Africa, Facility-based morbidity statistics are inadequate for monitoring malaria control programmes. This explorative study assessed whether a health centre equipped with a microscope and trained personnel could monitor malaria transmission within its catchment area. The study was conducted at Chemase Health Centre in Nandi District in Kenya, an area holoendemic for malaria with Anopheles gambiae as the main vector and Plasmodium falciparum as the commonest cause of malaria. From f…

Severe anaemia in children living in a malaria endemic area of Kenya.

Severe anaemia is an important cause of morbidity and mortality in African children, but the causes, particularly falciparum malaria, are difficult to determine. We assessed the contribution of falciparum malaria to anaemia in Kenyan children by clinical examination and measurement of parasitaemia and haemoglobin (Hb) concentration in 559 children in the community and in 2412 children admitted to Kilifi district hospital during a 2-year period. We also attempted to characterize severe malarial anaemia by examining the causes and pathophysiology of anaemia in 101 children admitted with Hb conce…

Sensorineural hearing loss in patients with sickle cell anaemia in Kenya.

The auditory function of sixty two Kenyan sickle cell anaemia patients aged seven to thirty years was compared to age-matched fifty five healthy controls with haemoglobin AA. Sensorineural hearing loss of 30 db and above was demonstrated in 25 (40%) of sickle cell anaemia patients and in three (5.5%) control subjects. Both sexes were equally affected. Bilateral lesion registered in 16%. Hearing threshold level was normal in 97% of the sickle cell group. High frequencies were commonly affected with hearing loss of 30-40 db. Two cases with severe unilateral deafness at all frequencies had severe…

The fall and rise of cost sharing in Kenya: the impact of phased implementation.

The combined effects of increasing demand for health services and declining real public resources have recently led many governments in the developing world to explore various health financing alternatives. Faced with a significant decline during the 1980s in its real per capita expenditures, the Kenya Ministry of Health (MOH) introduced a new cost sharing programme in December 1989. The programme was part of a comprehensive health financing strategy which also included social insurance, efficiency measures, and private sector development. Early implementation problems led to the suspension in…

Dental health of the Rendille and Samburu of the northern frontier district of Kenya.

Nomadic groups inhabiting the Kaisut desert region of Kenya were treated and studied during two successive trips. It was found that dental decay was relatively rare and advanced periodontal disease was present mostly in older patients. The mechanical action and contents of the locally available miswak stick (toothbrush tree) seem to play a significant role in control of dental diseases.

Increased recurrence of tuberculosis in HIV-1-infected patients in Kenya.

There is evidence that in human immunodeficiency virus 1 (HIV-1) infected patients with tuberculosis the rate of recurrence of tuberculosis is increased in those patients treated with a standard thiacetazone-containing regimen. To assess the impact of HIV-1 on tuberculosis in Kenya, patients with tuberculosis were studied prospectively. After treatment with either a standard thiacetazone plus isoniazid regimen or a short-course thiacetazone-containing regimen, overall recurrence rate of tuberculosis was 34 times greater in 58 HIV-1-positive patients than in 138 HIV-1-negative patients (adjuste…

Phlebotomus guggisbergi (Diptera: Psychodidae), a vector of Leishmania tropica in Kenya.

Sand flies were collected in light traps and on oiled papers at four active case sites of human cutaneous leishmaniasis due to Leishmania tropica at Muruku Sublocation, Laikipia District, Kenya. Nearly 5,200 females of five species, including Phlebotomus guggisbergi, were dissected and examined for flagellates. Of 3,867 P. guggisbergi females collected at a multiple case site, 168 (4.3%) harbored mature infections (to include metacyclic promastigotes) of flagellates morphologically identical to Leishmania, while all other flies were negative. Of the infected flies, 164 were collected in a cave…

Guillain-Barré syndrome in Kenya: a clinical review of 54 patients.

During an 8-year period, Guillain-Barré syndrome (GBS) was diagnosed in 54 Kenyans. Overall, the outcome was poor. Only 26 patients (48%) walked unaided within 3 month of the height of their paralysis. Five patients (9%) died from respiratory failure and 14 (26%) remained severely disabled from bilateral foot drop by the end of 16 months. Paralysis of lower cranial nerves and sphincter dysfunction were frequently associated with respiratory failure. Whether these features can predict the likelihood of respiratory failure developing requires further evaluation.
