Painful thickening of the soles: importance of keeping a wide differential.

Acquired plantar keratoderma refers to the hyperkeratotic plaques on the soles of the feet, which can result from a variety of underlying conditions. Psoriasis, allergic or irritant contact dermatitis and eczema are several causes of acquired plantar psoriasis. This case report describes a female patient in her 50s with plantar keratoderma, initially attributed to psoriasis that was recalcitrant to systemic treatments. Subsequent evaluation suggested the possibility of contact dermatitis triggered by footwear. Allergen avoidance and supportive management led to clinical improvement. This case

A case of Schöpf-Schulz-Passarge syndrome presented with widespread apocrine hidrocystomas showing variable clinical features.

Dear Editor, Genodermatoses are usually diagnosed in early childhood but diagnosis may be delayed in rare cases. Schöpf-Schulz-Passarge syndrome (SSPS) first described in 1971 is a rare type of autosomal recessive ectodermal dysplasia characterized by palmoplantar keratoderma, periocular and eyelid apocrine hidrocystomas, hypodontia, hypotrichosis, and nail dystrophy (1). Fewer than 40 cases have been reported up to 2018 (2) and the number has slightly increased with subsequent case reports. We report here a 73-year-old male patient whose parents were third-degree cousins presented to dermatol

Pachyonychia Congenita: A Research Agenda Leading to New Therapeutic Approaches.

Pachyonychia congenita (PC) is a dominantly inherited genetic disorder of cornification. PC stands out among other genodermatoses because despite its rarity, it has been the focus of a very large number of pioneering translational research efforts over the past 2 decades, mostly driven by a patient support organization, the Pachyonychia Congenita Project. These efforts have laid the ground for innovative strategies that may broadly impact approaches to the management of other inherited cutaneous and noncutaneous diseases. This article outlines current avenues of research in PC, expected outcom

An intronic splice-site variant in MBTPS2 underlies ichthyosis follicularis with atrichia and photophobia syndrome.

Ichthyosis follicularis with atrichia and photophobia (IFAP) syndrome is a rare genodermatosis characterized by a classic triad of follicular ichthyosis, alopecia, and photophobia. We report a Chinese patient displaying features of IFAP triad along with painful palmoplantar keratoderma, recurrent infections, periorificial keratotic plaques, nail dystrophy, and pachyonychia. Whole-exome sequencing revealed an intronic variant (NM_015884.3: exon7:c.970+5G>A) in the gene MBTPS2. Sanger sequencing confirmed that the variant segerated with phenotype in the family. Sequencing of cDNAs derived from t

Water-induced dermatosis: Aquagenic keratoderma. A case report.

Aquagenic keratoderma is a benign entity that is characterized by producing whitish or translucent papules a few seconds after contact with water. We present the case of a 16-year-old patient with multiple asymptomatic confluent papules that appeared on both hand palms after contact with water and which disappeared after drying. The histopathological findings in a skin biopsy after water exposure showed changes in the superficial layers of the stratum corneum and dilatation of sweat gland ducts. This entity of unknown etiology has been related to neuronal and eccrine gland dysfunction. Recentl

A Mal De Meleda patient with severe flexion contractures of hands and feet: A case report in West China.

RATIONALE: Palmoplantar keratoderma (PPK) is a genetically heterogeneous group of skin diseases, which is characterized by erythema and hyperkeratosis. Mal de Meleda (MDM) is a rare type of PPK with an estimated prevalence in the general population of 1 in 100,000. PATIENT CONCERNS: In this study, we report a MDM patient with severe lesion in skin and flexion contractures of fingers and toes. DIAGNOSES: MDM was diagnosed based on clinical manifestations and gene test. INTERVENTIONS: This patient was treated with oral acitretin and topical tazarotene. OUTCOMES: Physical examinations indicat

Richner-Hanhart syndrome (tyrosinemia type II): a case report of delayed diagnosis with pseudodendritic corneal lesion.

Richner-Hanhart syndrome (tyrosinemia type II) is a rare autosomal recessive disease associated with high serum tyrosine levels caused by the deficiency of tyrosine aminotransferase enzyme. We report a 15-year-old female patient with complaints of bilateral photophobia and tearing, which started during the infancy period. Biomicroscopic examination revealed bilateral circular corneal opacities on the inferior quadrant and small dendritic lesions at the center of the circular opacities. Blood tests showed a tyrosine level of 508 micromol/L (normal range: 30-150). On her dermatologic examination

Recurrent dysphonia and acitretin.

We report the case of a woman complaining of dysphonia while she was treated by acitretin. Her symptoms totally regressed after drug withdrawal and reappeared when acitretin was reintroduced. To our knowledge, this is the first case of acitretin-induced dysphonia. This effect may be related to the pharmacological effect of this drug on mucous membranes.

Genetic disorders of palm skin and nail.

The outer part of the skin, the epidermis, is specialized to protect the human body from its environment. Because of the high levels of physical stress experienced by the human hand in everyday use, the epidermis of the hand is especially toughened. In particular, the epidermis of the palm is highly specialized to resist mechanical trauma. Like the epidermis, the nails are composed of specialized epithelial cells and are especially strong. In recent years it has become apparent that the physical strength of epithelial cells comes from the keratin cytoskeleton--a dense meshwork of filaments ext

Jadassohn-Lewandowski syndrome (pachyonychia congenita).

Pachyonychia congenita is an uncommon autosomal dominant disorder with variable expression. Symmetrical nail hypertrophy, present in nearly all cases, is accompanied by dyskeratosis and dysplasia of other ectodermal tissues. This article reviews the genetics, clinical manifestations, histopathology, and treatment of pachyonychia congenita. Many clinical features have been reported in association with this syndrome. From a review of the literature, we propose criteria for the diagnosis of pachyonychia congenita using the more important of these clinical manifestations.

Porokeratosis plantaris, palmaris, et disseminata.

A 58-year-old man and his son presented with the clinical and histologic characteristics of porokeratosis plantaris, palmaris, et disseminata. Porokeratosis plantaris, palmaris, et disseminata also occurred in two other family members. Marked improvement was demonstrated with oral aromatic retinoid (Tigason) therapy. The differential diagnosis from the other types of porokeratosis is discussed. It is suggested that punctate porokeratosis is a form fruste of porokeratosis plantaris, palmaris, et disseminata.

Porokeratosis plantaris, palmaris, et disseminata. Report of a case and treatment with isotretinoin.

Porokeratosis plantaris, palmaris, et disseminata is a unique clinical entity characterized by the familial occurrence of multiple porokeratoses predominantly on the palms and soles but also on both sun-exposed and non-sun-exposed areas of the body. Since its original description in 1971 by Guss and colleagues, there has been only one other report of this syndrome. We have discovered a third family with this disorder and have used isotretinoin successfully to control the condition in one of the family's symptomatic members.

[Functional anomalies of polymorphonuclears in Papillon-Lefèver disease (author's transl)].

Polymorphonuclear functions were studied in 3 patients of the same brotherhood with Papillon-Lefèvre disease (hyperkeratosis palmaris and plantaris and acute periodontosis resulting in loss of teeth) who developed severe and recurrent infections. Chemotaxis, oxygen consumption and production of H2 O2 were investigated by polarography, O2 production by quantitative reduction of nitroblue tetrazolium and iodination by the Pinus and Klebanoff technique. functional anomalies of polymorphonuclears involving chemotaxis, induced O2 consumption and H2 O2 production were detected in all three patients.
