Nutrition delivery in infants with kidney failure on extracorporeal kidney replacement therapy.

BACKGROUND: Infants with kidney failure who initiate kidney replacement therapy (KRT) within 3 months of life require optimal fluid and nutritional management to support growth. Continuous or prolonged intermittent KRT (CKRT or PIKRT) is often used as a bridge to peritoneal dialysis facilitating earlier nutrition provision but is associated with protein losses. Current guidelines recommend ≥ 3-3.5 g/kg/day of protein during KRT. We aimed to assess nutritional prescriptions and growth in these infants. METHODS: We conducted a retrospective review of infants with kidney failure managed with ext…

Metabolic disorder-associated kidney disease and diabetic kidney disease in developing countries.

There is a paucity of literature addressing the epidemiology of metabolic disorder-associated kidney disease in developing countries, but extrapolating from the high prevalence of metabolic syndrome in low- and middle-income countries, and the known high prevalence of diabetic kidney disease in the same, the health and economic impact of metabolic disorder-associated kidney disease and diabetic kidney disease is substantial. Resource constraints in developing countries amplify the challenges in preventing, detecting, and treating metabolic disorder-associated kidney disease and diabetic kidney…

Apolipoproteins in Chronic Kidney Disease and Kidney Transplant: A Long Unfinished Story.

Chronic kidney disease (CKD) is a growing global health burden, strongly associated with cardiovascular disease, the leading cause of mortality in this population. Dyslipidemia is a key metabolic abnormality in CKD, but traditional lipid measures such as total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides often fail to capture the complexity of lipid disturbances in CKD and after kidney transplantation. Apolipoproteins have emerged as more reliable markers of cardiovascular and renal risk. Elevated apolipoprotein B (ApoB), reduced apolipoprotein A1 (ApoA1), and a higher ApoB…

Future Therapy for End-Stage Kidney Failure: Gene-Edited Pig Kidney Xenotransplantation.

In patients with end-stage kidney disease, kidney transplantation from a living or deceased human donor offers a much-improved quality and length of life. Gene-edited pigs might provide an alternative source of kidneys for clinical transplantation (xenotransplantation). The major pathobiological barriers to successful pig kidney xenotransplantation have steadily been overcome by the following methods: (1) genetic engineering of the organ-source pig and (2) the administration of novel immunosuppressive agents. Pig kidney transplants have now supported immunosuppressed (anephric) nonhuman primat…

Navigating Challenges and Adapting Innovations in Acute Kidney Injury and Kidney Failure.

Acute kidney injury and kidney failure represent critical health challenges, requiring continuous and specialized care. The impact of natural disasters, such as earthquakes, floods, and hurricanes, alongside man-made crises like armed conflicts and pandemics, exacerbates the complexities of managing these conditions. Disasters disrupt health care infrastructure, cause resource shortages, and pose ethical dilemmas in care allocation, often leading to surges in acute kidney injury cases and interruptions in kidney failure treatment. This article examines these challenges and underscores the impo…

Targeting Dysregulated Epigenetic Modifiers With Kidney-Targeted Nanotherapeutics for Polycystic Kidney Disease.

Autosomal dominant polycystic kidney disease (ADPKD) is the most common genetic kidney disease worldwide. The one small molecule drug available to patients, tolvaptan, is associated with off-target side effects and high discontinuation rates, necessitating the development of new therapeutic strategies. Previous work has shown that the epigenome is altered in ADPKD; however, the identification and targeting of dysregulated epigenetic modulators has yet to be explored for human ADPKD therapy. Using cells derived from cysts of ADPKD patients, we tested the gene expression of several epigenetic mo…

Growth in neonates with congenital kidney failure requiring continuous kidney replacement therapy.

BACKGROUND: With advanced technology, survival of neonates with congenital kidney failure (CKF) requiring continuous kidney replacement therapy (CKRT) has improved. Nutrition is essential but difficult to attain as CKRT removes proteins and micronutrients, and many patients have multiple co-morbidities. Scant data exist to guide clinicians on appropriate energy requirements for growth. METHODS: We performed a retrospective study of neonates with CKF admitted to Children's of Alabama between 2016 and 2022 who required KRT within 10 days. We evaluated risk factors and growth in the 18/24 (75%) …

Acute Kidney Failure in Limited Scleroderma Reveals Anti-GBM--Associated Kidney Disease.

Although scleroderma renal crisis (SRC) is the most common form of kidney injury in patients with scleroderma, consideration of other autoimmune conditions is warranted in patients with atypical presentation. A 27-year-old woman with a history of limited scleroderma presented to the hospital with 1 month of uremic symptoms and oliguria. Initial laboratory tests revealed significantly elevated serum urea nitrogen and creatinine levels, suggesting kidney failure. Suspicion for SRC was low, and kidney biopsy was performed to evaluate further. Histopathology revealed severe diffuse crescentic glom…

Melatonin attenuates kidney injury by alleviating lysosomal damage in diabetic kidney disease.

Proteinuria-induced damage to renal tubular epithelial cells is one of the main causes of diabetic kidney disease (DKD), and the clearance of overloaded albumin by lysosomes is crucial for maintaining the homeostasis of renal tubular epithelial cells. Therefore, lysosomal damage is closely related to the pathogenesis of DKD, but effective prevention and treatment measures are still lacking. Melatonin (MLT) is secreted by the pineal gland and can not only regulate circadian rhythms but also maintain lysosomal homeostasis. In this study, we demonstrate the presence of significant lysosomal damag…

[Acute Kidney Injury in the Context of Chronic Kidney Disease: Interdisciplinary Aspects].

Acute respiratory diseases (ARDs) are among the most common illnesses of the 21st century and serve a leading cause of temporary disability. They are also a frequent trigger for the development of acute kidney injury (AKI), which, when progressing to acute kidney failure (AKF), requires costly treatment, including hemodialysis. The risk of AKF increases with age, especially in the presence of comorbidities. The proportion of AKF patients in intensive care units reaches 10-15%. This article presents clinical observations of two female patients of middle and advanced age who developed AKF follow…

Kidney protection strategy lowers the risk of contrast-associated acute kidney injury.

We developed a comprehensive kidney protection strategy (KPS), which comprises left ventricular end-diastolic pressure-guided saline hydration, ultralow contrast coronary angiography, and a staged coronary revascularization procedure under suitable conditions. This study aimed to investigate KPS's effect on the risk of developing contrast-associated acute kidney injury (CA-AKI) among persons with moderate-to-advanced kidney insufficiency (KI). Seventy patients who had undergone cardiac catheterization with an estimated glomerular filtration rate (eGFR) of 15-45 mL/min/1.73 m2 were investigated…

Longitudinal mortality risks and kidney functional outcomes in Japanese living kidney donors.

OBJECTIVES: Previous studies suggested that living kidney donors do not have a higher risk of death or kidney failure than the general population. However, living kidney donor risk is controversial. Furthermore, only a few studies have evaluated long-term kidney function after kidney donation. METHODS: This study evaluated Japanese kidney donor' long-term outcomes, including mortality and kidney function. From 1965 to 2015, 230 donors (76 males, 154 females, and a median age of 54) were enrolled in this study. The median observation period was 11.0 (range, 0.3-41.0) years. RESULTS: In total,…

Highly Sensitized Kidney Transplant Outcomes After the 2014 Kidney Allocation System Change.

Introduction: Kidney Allocation System (KAS) was implemented by United Network for Organ Sharing in 2014 to reduce allocation disparities. Research Questions: Outcomes of highly sensitized patients (calculated panel reactive antibody (cPRA) ≥ 97%) before and after KAS were compared to low-risk recipients (cPRA <10%) in the post-KAS era were examined. The impact on racial disparities was determined. Design: This was a retrospective study of national registry data. Two cohorts of adult candidates waitlisted for deceased donor transplantation during 3-year periods before and after KAS were identi…

Kidney transplant outcomes in children with simultaneous versus sequential heart-kidney transplants.

BACKGROUND: Heart transplant recipients frequently require kidney transplantation for concomitant advanced chronic kidney disease. Data on simultaneous (heart and kidney transplants performed simultaneously) versus sequential (heart transplant performed before kidney) heart-kidney transplants in children are limited. Herein, we compare kidney transplant outcomes between the two groups. METHOD: We used the Scientific Registry of Transplant Recipients to identify all pediatric (age <21 years) heart transplant recipients who also received a kidney transplant within 10 years of the heart transpla…

A kidney-brain neural circuit drives progressive kidney damage and heart failure.

Chronic kidney disease (CKD) and heart failure (HF) are highly prevalent, aggravate each other, and account for substantial mortality. However, the mechanisms underlying cardiorenal interaction and the role of kidney afferent nerves and their precise central pathway remain limited. Here, we combined virus tracing techniques with optogenetic techniques to map a polysynaptic central pathway linking kidney afferent nerves to subfornical organ (SFO) and thereby to paraventricular nucleus (PVN) and rostral ventrolateral medulla that modulates sympathetic outflow. This kidney-brain neural circuit wa…

Stopping kidney protection in the elderly following acute kidney injury: think mortality.

Chronic kidney disease (CKD) is projected to become the fifth most common global cause of death by 2040. This illustrates a key consequence of CKD, i.e. premature mortality. Since nephroprotective drugs such as renin-angiotensin system (RAS) blockers and sodium-glucose transport protein 2 (SGLT2) inhibitors decrease glomerular hyperfiltration, they may be stopped following an episode of acute kidney injury (AKI). This may theoretically modify the risks of subsequent events, ranging from hyperkalaemia to CKD progression to cardiovascular events, but the evidence so far has been inconsistent. Ro…

Kidney stone formation in a novel murine model of polycystic kidney disease.

Individuals with autosomal dominant polycystic kidney disease have a higher incidence of stone formation than the general population. However, there are no cystic animal models known to develop stones. Cystic mice compound heterozygous for hypomorphic Pkd1V and Pkd1RC alleles develop cystic kidneys within a few weeks of birth but live beyond 20 wk of age, allowing for the study of cystic comorbidities including stone formation. Cystic Pkd1V/RC mice were euthanized at 3, 13, or 26 wk of age, and their kidneys were analyzed by microcomputed tomography (µCT) for stone formation. Mice had occasion…

Urinary Citrate Is Associated with Kidney Outcomes in Early Polycystic Kidney Disease.

Low urinary citrate and crystal deposition accelerated cystogenesis in an experimental model of polycystic kidney disease (PKD).Hypocitraturia, frequently observed in patients with autosomal dominant PKD (ADPKD) could contribute to disease progression.Present findings suggest lower urinary citrate in early PKD was associated with faster eGFR decline and worse kidney survival.
